Article Synopsis
  • Endovascular thrombectomy (EVT) has proven safe and effective for patients suffering from large core strokes, but the effects of reperfusion quality and procedure details on outcomes are still unclear.
  • In the SELECT2 trial, findings indicated that 80% of patients experienced successful reperfusion, which correlates with better clinical outcomes, particularly in those who achieved near-complete reperfusion.
  • Longer procedure times negatively impacted patient outcomes, while the method of thrombectomy (aspiration vs stent-retriever) showed no significant differences in reperfusion success or functional recovery.

Macular Buckling for Myopic Traction Maculopathy: A Comprehensive Systematic Review and Meta-Analysis.

Am J Ophthalmol

October 2024

BayCare Clinic Eye Specialists (A.G.E.), Aurora BayCare Medical Center, Green Bay, Wisconsin, USA; Department of Ophthalmology (A.G.E.), Faculty of Medicine, Cairo University, Cairo, Egypt.

Article Synopsis
  • Myopic traction maculopathy is a significant cause of vision issues, especially in Asian populations, prompting the need to explore effective treatments like macular buckling.
  • A systematic review analyzed 13 studies involving 482 eyes, evaluating the impact of macular buckling on visual acuity, axial length, and the rates of retinal reattachment and macular hole closure.
  • The results indicated notable improvements in best-corrected visual acuity, consistent reductions in axial length, and high rates of retinal reattachment (94%) and macular hole closure (72%), suggesting that macular buckling is a beneficial treatment option.

Article Synopsis
  • The BE FAST acronym, which includes Balance and Eye symptoms along with the original FAST signs, was intended to help the public better recognize stroke symptoms, but previous studies showed mixed results in improving detection.
  • A pilot study with 174 participants found that while both BE FAST and FAST had similar overall recall rates at 30 days, BE FAST participants struggled significantly more to remember all symptoms shortly after learning them and over time.
  • The study concluded that using the original FAST acronym resulted in better retention of stroke symptoms compared to BE FAST, indicating that adding Balance and Eye signs may not be beneficial for public awareness.

Alignment in total ankle replacement is important for success and implant survival. Recently there has been the introduction and adoption of patient specific instrumentation for implantation in total ankle replacement. Current literature does not evaluate the effect of preoperative deformity on accuracy of patient specific instrumentation.

Introduction: Left ventricular assist devices (LVAD) are increasingly common among patients with heart failure. The unique physiologic characteristics of patients with LVADs present a challenge to emergency clinicians making treatment and disposition decisions. Despite the increasing prevalence of LVADs, literature describing emergency department (ED) visits among this population is sparse.

is a fungal genus that can cause opportunistic human infection in immunocompromised hosts. While the majority of clinical manifestations are cutaneous or subcutaneous infections, there are reports of oculomycosis, which is most commonly present as keratomycosis or endophthalmitis. Here, we present a rare case of Alternaria dacryocystitis with cutaneous fistula in a diabetic patient.

Ankle fusion is a well-studied, classical procedure for conditions affecting the ankle joint, and has a long history with predictable results. Although there are many different approaches available, a few options are commonly used. The two main focuses of modern literature are on anterior plating versus traditional screw fixation, and on arthroscopic versus open techniques for fusion.

Many different types of surgical repair for insertional Achilles tendinosis have been described. Strength after surgery is an essential factor for patient function and satisfaction. A retrospective series of patients that underwent surgery for insertional Achilles tendon surgery were contacted to come in for prospective strength testing, with the tester blinded to the type of surgery performed.

Surgical repair of insertional Achilles tendinosis is indicated in the face of failed conservative measures. Several methods for this repair have been described, and the optimal method is not universally agreed upon. In addition, the cost of medical care is important, and should be considered when determining the surgical repair for each patient.

Total ankle replacement (TAR) continues to increase in popularity as a motion-preserving option to ankle arthrodesis. TAR is indicated for primary, posttraumatic and inflammatory arthropathies as an alternative procedure to tibiotalar arthrodesis. Proper patient selection is paramount to a successful outcome in TAR.

Summit Orthopaedics, 2321 Coronado Street, Idaho Falls, ID 83404, USA.

The correction of the deformed arthritic foot and ankle is a complicated and somewhat controversial topic. After conservative methods fail, there is a wide range of possible bony procedures and arthrodesis that maybe performed. The appropriate work up and understanding of the pathomechanics is vital to the correct choice of procedures to correct these deformities.

De Quervain syndrome is the second most common compressive tendinopathy. Although the length of the first extensor compartment (FEC) has been studied previously, there is no documented reported comparison study of short-axis and long-axis sonographic measurements. The thread technique, or Guo Technique, has been applied to carpal tunnel syndrome, trigger finger, and superficial peroneal compressive neuropathy.
