Division of High-Consequence Pathogens and Pathology, National Center for Emerging and Zoonotic Infectious Diseases, Centers for Disease Control and Prevention, Atlanta, GA, United States.

Introduction: Patients with Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) experience cognitive problems with attention, information processing speed, working memory, learning efficiency, and executive function. Commonly, patients report worsening of cognitive symptoms over time after physical and/or cognitive challenges. To determine, monitor, and manage longitudinal decrements in cognitive function after such exposures, it is important to be able to screen for cognitive dysfunction and changes over time in clinic and also remotely at home.

Background: Post-acute sequelae of SARS-CoV-2 infection (PASC) symptoms have broad impact, and may affect individuals regardless of COVID-19 severity, socioeconomic status, race, ethnicity, or age. A prominent PASC symptom is cognitive dysfunction, colloquially referred to as "brain fog" and characterized by declines in short-term memory, attention, and concentration. Cognitive dysfunction can severely impair quality of life by impairing daily functional skills and preventing timely return to work.

Article Synopsis
  • The MCAM study aimed to see if different clinicians recognized myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) as the same clinical condition by analyzing data from 465 patients across seven specialized clinics in the U.S.
  • Research found minimal significant differences in ME/CFS symptoms and functionalities among patients from various clinics, indicating a possible inherent illness heterogeneity.
  • The study suggests that representing data visually (like scatter plots) could better illustrate this variation, and emphasizes the need for more detailed subgroup analyses in future research to enhance understanding and reproducibility of ME/CFS findings.

The etiology and mechanism of myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) are poorly understood and no biomarkers have been established. Specifically, the relationship between the immunologic, metabolic, and gastrointestinal abnormalities associated with ME/CFS and their relevance to established symptoms of the condition remain unclear. Relying on data from two independent pairs of ME/CFS and control cohorts, one at rest and one undergoing an exercise challenge, we identify a state of suppressed acute-phase innate immune response to microbial translocation in conjunction with a compromised gut epithelium in ME/CFS.

Article Synopsis
  • SARS-CoV-2 infection can lead to long-term health issues known as post-acute sequelae of SARS-CoV-2 infection (PASC) or long COVID, which can manifest as ongoing or new symptoms after the initial infection.
  • The RECOVER-Adult study aims to better understand PASC by investigating its prevalence, symptoms, risk factors, and underlying biological mechanisms through a large cohort of nearly 15,000 adults.
  • Participants will provide ongoing data through questionnaires, physical examinations, and biological samples over several months, helping researchers gather critical insights into the complexities of long COVID.

Importance: SARS-CoV-2 infection is associated with persistent, relapsing, or new symptoms or other health effects occurring after acute infection, termed postacute sequelae of SARS-CoV-2 infection (PASC), also known as long COVID. Characterizing PASC requires analysis of prospectively and uniformly collected data from diverse uninfected and infected individuals.

Objective: To develop a definition of PASC using self-reported symptoms and describe PASC frequencies across cohorts, vaccination status, and number of infections.

Article Synopsis
  • Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a complex illness marked by severe fatigue, reduced functionality, and other symptoms, with variations in natural killer (NK) cell counts being explored as a potential biomarker.
  • In a study involving over 270 participants, including those with ME/CFS and healthy controls, researchers measured NK cell cytotoxicity but found no significant differences between groups, indicating that NK cell cytotoxicity might not serve as a reliable biomarker for this condition.
  • The study concludes that the assay used for measuring NK cytotoxicity is not ready for clinical application and emphasizes the need for further research into immune factors related to ME/CFS.

Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is characterized by unexplained debilitating fatigue, cognitive dysfunction, gastrointestinal disturbances, and orthostatic intolerance. Here, we report a multi-omic analysis of a geographically diverse cohort of 106 cases and 91 healthy controls that revealed differences in gut microbiome diversity, abundances, functional pathways, and interactions. Faecalibacterium prausnitzii and Eubacterium rectale, which are both recognized as abundant, health-promoting butyrate producers in the human gut, were reduced in ME/CFS.

Mental Health Practitioners (MHPs) have a unique opportunity to provide resources and support to those suffering from Long COVID (LC), the post infectious illness that often follows an acute SARS-CoV-2 infection. In working with these individuals, MHPs can learn from the experiences of patients with another post-infectious disease known as myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS). ME/CFS was once thought to be a psychologically mediated disorder caused by deconditioning and the fear of exertion following a precipitating event such as a viral infection.

Background: Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a complex multi-symptom disease with widespread evidence of disrupted systems. The authors hypothesize that it is caused by the upregulation of the corticotropin-releasing factor receptor type 2 (CRFR2) in the raphé nuclei and limbic system, which impairs the ability to maintain homeostasis. The authors propose utilizing agonist-mediated receptor endocytosis to downregulate CRFR2.
