We assessed the attitudes of UK Obstetrics and Gynaecology (O&G) trainees towards a caesarean delivery for maternal request (CDMR); and identified differences in attitude towards patients requesting CDMR and preferences for their own mode of delivery. An internet survey was constructed with questions covering trainees' personal preferences towards and experience of CDMR; attitudes to CDMR; and how they might treat patients making this request. Bilateral anterior shoulder dislocation is a rare clinical entity with few case reports and limited series published in the literature. Acute adult epiglottitis is a rare and potentially fatal upper airway condition, requiring prompt recognition, resuscitation, stabilization, and transfer to definitive care. Treatment centers on skilled airway management and early antimicrobial therapy. We describe a patient with acute adult epiglottitis in whom airway management using noninvasive positive pressure ventilation was successful allowing avoidance of tracheal intubation.

Primary breast lymphoma is a rare subtype of breast malignancy whose diagnosis relies on histopathological characteristics and a high index of suspicion in clinically unusual presentations of breast pathology. The authors report a rare case of diffuse primary large B cell lymphoma of leg type presenting as a single lesion confined to the breast. Unusually for this subtype of lymphoma, the lesion was surgically excised and the patient, to date, has made an uneventful recovery.

He was initially treated for his lymphoma and then, following no regression in his cervical lymphadenopathy was referred for further assessment. This revealed squamous cell carcinoma arising from the left tongue base with metastatic disease in the left neck.
