Marcus Tullius Cicero's concept of "ius gentium," or universal law, provides a timeless framework for understanding and defending fundamental human rights, particularly in the context of reproductive freedom. Cicero distinguished between "ius gentium" and "ius civile," emphasizing that while civil law governs specific communities, universal law, rooted in natural reason, applies to all humanity. This philosophical foundation resonates with modern discussions on reproductive rights, where universal principles of justice and bodily autonomy are at stake.

Maxillofacial trauma is a significant concern in emergency departments (EDs) due to its high prevalence and the complexity of its management. However, many ED physicians lack specialized training and confidence in handling these cases, leading to a high rate of facial trauma referrals and increased stress on consult services. Recent advancements in artificial intelligence, particularly in large language models such as ChatGPT, have shown potential in aiding clinical decision-making.

Risk and protective factors for psychiatric illnesses are linked to distinct structural and functional changes in the brain. Further, the prevalence of these factors varies across sexes and genders, yet the distinct and joint effects of sex and gender in this context have not been extensively characterized. This suggests that risk and protective factors may map onto the brain and uniquely influence individuals across sexes and genders.

Background: Pancreatic ductal adenocarcinoma (PDAC), a highly lethal cancer with a poor prognosis, is expected to become the second deadliest cancer in the United States by 2030. Despite advancements in treatment modalities, the survival rates of patients with PDAC have remained low. Immunotherapy has emerged as a promising treatment for various cancers; however, its utilization in PDAC has been limited due to various challenges, including resistance mechanisms and the advanced stage at which most patients are diagnosed.

Department of Pathology and Laboratory Medicine, North Shore University Hospital and Long Island Jewish Medical Center, Donald and Barbara Zucker School of Medicine at Hofstra/Northwell Health, Hempstead, New York, USA.

Introduction: Indeterminate oncocytic/Hürthle cell lesions on thyroid cytology are reflexed to molecular testing. This study aims to examine the cytologic characteristics of thyroid fine-needle aspiration (FNA) smears with oncocytes classified as atypia of undetermined significance (AUS) with particular molecular patterns that can aid in determining a more conclusive Bethesda category upfront thus decreasing unnecessary testing and associated costs.

Materials And Methods: Our pathology database was searched for thyroid FNAs with AUS for oncocyte predominance from 2019 to 2022.

Article Synopsis
  • Lichen planus (LP) is a chronic inflammatory disease affecting the skin and mucous membranes, with a low overall prevalence of about 0.15% among U.S. adults, higher in females compared to males.
  • Among over half a million patients studied, 1,098 had LP, with many (53%) receiving no treatment after a year; topical corticosteroids were the most common initial therapy.
  • The findings highlight the need for further research on treatment options since patients on systemic immunosuppressants were more likely to continue or switch treatments after a year.

Elevated levels of serum autoantibodies are a hallmark of systemic lupus erythematosus (SLE) and are produced by plasma cells in response to a variety of antigenic triggers. In SLE, the triggers are complex and may include both T cell-dependent/-independent and TLR-dependent/-independent mechanisms of immune activation, which ultimately contributes to the significant immune dysregulation seen in patients at the level of cytokine production and cellular activation (B cells, T cells, dendritic cells, neutrophils and macrophages). Interferon regulatory factor 5 (IRF5) has been identified as an autoimmune susceptibility gene and polymorphisms in IRF5 associate with altered expression and hyper-activation in distinct SLE immune cell subsets.

The Master Adaptive Clinician Educator: A Framework for Future Educational Leaders in Academic Medicine.

J Gen Intern Med

November 2024

Division of General Internal Medicine, Department of Medicine, Boston University Chobanian and Avedisian School of Medicine, Boston Medical Center, Boston, MA, USA.

Medical education continues to evolve with technologic advances, cultural changes, and progress in the application of learning theories. One example is Cutrer's concept of the Master Adaptive Learner (MAL), where the trainee self-directs learning and innovates to maintain a level of expertise in a domain. We suggest that this concept can be applied alongside ideas of adaptive and teacher expertise to represent the medical educator of the future-the Master Adaptive Clinician Educator (MACE).
