Highly selective inhibitors of cyclin-dependent kinase 4 and 6 (CDK4/6is) have emerged as a standart of care for first- and second-line therapies in combination with endocrine therapy (ET) for HR+/HER2- metastatic breast cancer (MBC) patients. It has been reported that combination therapy is more effective than ET alone and is safe in elderly patients as well as young patients. Nevertheless, elderly and very old patients with HR+/HER2-MBC treated with CDK4/6 inhibitor (CDK4/6i) combinations are relatively underrepresented in randomized controlled trials.

Article Synopsis
  • Chronic obstructive pulmonary disease (COPD) is a major global health issue, and factors like biomass exposure, patient demographics, and other health conditions play a significant role in worsening patient outcomes during episodes of exacerbation.
  • A study analyzed medical records of 334 COPD patients from January 2021 to December 2023, focusing on how biomass exposure influences hospital outcomes like ICU admissions, length of stay, and in-hospital mortality, rather than looking at its direct effects.
  • Results showed that patients with biomass exposure were more likely to be female, had lower smoking rates, and experienced higher ICU admissions and mortality; older patients also faced greater health challenges during their hospital stays, although specific characteristics didn't significantly correlate with outcomes.

  • The study investigates the impact of adjusting positive end-expiratory pressure (PEEP) based on intra-abdominal pressure (IAP) in mechanically ventilated patients with respiratory failure, focusing on its effects on oxygenation and potential harms.
  • Researchers applied different PEEP levels (0, 50%, and 100% of IAP) to patients, measuring arterial blood gases and mean arterial pressures to evaluate outcomes.
  • Results indicated significant improvements in the PaO2/FiO2 ratio with increased PEEP levels, while ensuring that plateau pressures remained within a safe range, suggesting a beneficial effect on oxygenation without negative side effects.
