De-Escalation of Nodal Surgery in Clinically Node-Positive Breast Cancer.

Importance: Increasing evidence supports the oncologic safety of de-escalating axillary surgery for patients with breast cancer after neoadjuvant chemotherapy (NAC).

Highly selective inhibitors of cyclin-dependent kinase 4 and 6 (CDK4/6is) have emerged as a standart of care for first- and second-line therapies in combination with endocrine therapy (ET) for HR+/HER2- metastatic breast cancer (MBC) patients. It has been reported that combination therapy is more effective than ET alone and is safe in elderly patients as well as young patients. Nevertheless, elderly and very old patients with HR+/HER2-MBC treated with CDK4/6 inhibitor (CDK4/6i) combinations are relatively underrepresented in randomized controlled trials.

Background: The anatomy of the proximal tibia presents treatment challenges, and there is currently no widely accepted surgical fixation method for fractures in this region. The aim of this study is to evaluate the efficacy, benefits, and differences between intramedullary nailing (IMN) with a 4-hole Limited Contact Dynamic Compression Plate (LC-DCP) combination and IMN with an 8-hole LC-DCP combination in unstable fractures of the proximal metaphyseal region of the tibia.

Type II odontoid fractures are recognized as unstable fractures, often necessitating surgical intervention. The anterior transodontoid screw technique emerges as a commonly employed surgical approach in such cases, with factors like age, osteoporosis, and the extent of fracture line displacement influencing surgical success. Objective: This study aims to compare the impact of 3 surgical techniques-stand-alone cage placement (SAC), anterior cervical plate (ACP) fixation, and total disc replacement (TDR)-on cervical lordosis restoration and segmental height after anterior cervical discectomy at C5-6 level.
