Article Synopsis
  • Home time, or the duration patients with cardiovascular disease spend alive outside of healthcare institutions, is an essential aspect of recovery, particularly for heart failure (HF) patients, yet insufficiently studied in India where HF rates are increasing.
  • Current literature falls short in identifying hospital-level trends and predictors of home time, which complicates the development of personalized care strategies to enhance health outcomes, functional status, and reduce hospitalization risks.
  • While guideline-directed medical therapy, especially sacubitril/valsartan (an angiotensin receptor neprilysin inhibitor), is well-supported for improving life quality and reducing HF-related hospitalizations, its use remains low, indicating a critical need for better integration into clinical practice to improve patient recovery and home time post

Background/aims: Per-oral (PO) administration of tacrolimus (TAC) results in inadequate trough levels in the early postoperative period in liver-transplant (LT) recipients who undergo Roux-en-Y hepaticojejunostomy for biliary reconstruction. Sublingual administration (SL) of tacrolimus provides an alternative route in such patients. The objectives of this study were to assess the feasibility and safety of SL tacrolimus in adult LT-recipients in the early postoperative period and to compare therapeutic efficacy of SL administration of tacrolimus versus PO route.

Article Synopsis
  • Paroxysmal cold hemoglobinuria (PCH) is a rare autoimmune condition primarily affecting young children, characterized by an antibody that damages red blood cells at colder temperatures and causes lysis when warmed.
  • Diagnosis involves high clinical suspicion and confirming antibodies known as Donath-Landsteiner antibodies, with symptoms potentially leading to acute kidney injury and the need for treatments like hemodialysis.
  • The report highlights the importance of recognizing PCH to avoid misdiagnosis with generic autoimmune hemolytic anemia and unnecessary treatments, while also noting that it can sometimes require urgent renal replacement therapy despite being self-limiting.
