Negative symptoms in CHR-P people are generally not responsive to treatments and commonly related to poorer functional outcome. However, less research attention has been dedicated to Persistent Negative Symptoms (PNS), defined as clinically stable negative symptoms of moderate severity evident for at least 6 months. This study aims to (a) determine the prevalence of PNS in a sample of young people at CHR-P; (b) investigate any association of PNS with functioning and clinical features; (c) examine longitudinal course of PNS across 2 years of follow-up and changes in PNS severity levels with specialized treatments.

Article Synopsis
  • - The European AAOCA Registry (EURO-AAOCA) is a study evaluating how different European centers manage patients with Anomalous Aortic Origin of a Coronary Artery (AAOCA), a rare heart defect, from January 2019 to June 2023.
  • - Out of 262 patients with a median age of 33 years, common symptoms included chest pain and syncope, with right-AAOCA being the most frequent type identified.
  • - There is currently no standard approach for diagnosing and treating AAOCA in Europe, though surgery has proven safe; future studies aim to improve patient selection for revascularization versus conservative treatment.

Article Synopsis
  • - Marginal zone lymphomas (MZL) are a group of related subtypes that currently lack a standardized prognostic score for patients requiring systemic therapy.
  • - Researchers developed a prognostic model based on a study of 501 patients, identifying five important factors that significantly affect progression-free survival (PFS) and creating three risk categories: low, intermediate, and high.
  • - The newly proposed MZL International Prognostic Index (MZL-IPI) has been validated in multiple patient cohorts, showing reliable predictive value for both PFS and overall survival (OS) among MZL patients undergoing treatment.

Article Synopsis
  • Antiplatelet therapy is crucial for preventing blood clots in patients with recent heart issues, but urgent surgeries pose challenges in balancing bleeding and clotting risks.
  • Current literature lacks a consensus on the best approach to manage antiplatelet therapy during surgery, particularly for neurosurgical cases, although some guidelines exist for other types.
  • In a reported case, a patient undergoing brain tumor surgery safely used an intravenous P2Y inhibitor (cangrelor) as a bridging therapy, showing no bleeding or heart-related problems during and after the procedure.

Background And Hypothesis: Antipsychotic (AP) prescription in clinical high risk for psychosis (CHR-P) subjects remains a divisive issue. Although official guidelines currently discourage AP treatment in CHR-P, it is common in clinical practice, especially for psychosis prevention. The aim of this study was to investigate whether baseline AP need (especially in high-dose) indexes a CHR-P subgroup with poorer prognosis and differs from AP-naïve subjects in terms of sociodemographic, clinical, and outcome parameters across a 2-year follow-up.

Purpose: Although the stability of current diagnostic criteria for people with First Episode Psychosis (FEP) is essential for treatment, it still remains poorly investigated. As its examination necessarily requires a prospective evaluation of diagnostic trajectories, the aims of the current longitudinal investigation were: (a) to assess diagnostic changes in an Italian FEP population treated within an "Early Intervention in Psychosis" service during a 2-year follow-up period, and (b) to identify potential sociodemographic and clinical moderators of diagnostic instability at entry.

Methods: All participants were FEP individuals, aged 12-35 years.

Age-related changes in intestinal microbiome composition and function are increasingly recognized as pivotal in the pathophysiology of aging and are associated with the aging phenotype. Diet is a major determinant of gut-microbiota composition throughout the entire lifespan, and several of the benefits of a healthy diet in aging could be mediated by the microbiome. Mediterranean diet (MD) is a traditional dietary pattern regarded as the healthy diet paradigm, and a large number of studies have demonstrated its benefits in promoting healthy aging.

Artificial Intelligence for Personalized Perioperative Medicine.

Cureus

January 2024

Department of Medicine and Surgery, Anesthesiology, Critical Care and Pain Medicine Division, Azienda Ospedaliero Universitaria di Parma, Parma, ITA.

The development of artificial intelligence (AI) is disruptive and unstoppable, also in medicine. Because of the enormous quantity of data recorded during continuous monitoring and the peculiarity of our specialty where stratification and mitigation risk are some of the core aspects, anesthesiology and postoperative intensive care are fertile fields where new technologies find ample room for expansion. Recently, research efforts have focused on the development of a holistic technology that globally embraces the entire perioperative period rather than a fragmented approach where AI is developed to carry out specific tasks.

The healthy properties of pomegranate fruit, a highly consumed food, have been known for a long time. However, the pomegranate supply chain is still rather inefficient, with the non-edible fraction, whose weight is roughly half the total and is endowed with plenty of valuable bioactive compounds, either disposed of or underutilized. A novel extract obtained from non-edible byproducts (called PPE), using hydrodynamic cavitation, a green, efficient, and scalable technique, was investigated for its cardiovascular effects in vivo.
