Mucorales-Specific T Cells in Patients with Hematologic Malignancies.

PLoS One

July 2016

Section of Hematology, Department of Surgical and Medical Sciences, University of Modena and Reggio Emilia, Azienda Ospedaliero-Universitaria Policlinico, Modena, Italy.

Article Synopsis
  • Invasive mucormycosis (IM) is a serious fungal infection that is hard to diagnose and treat promptly, particularly in patients with hematologic malignancies.
  • A study used an enzyme-linked immunospot assay to detect Mucorales-specific T cells in blood samples from patients undergoing chemotherapy, finding that about 10% of patients had these cells.
  • The research identified two patient groups: those without invasive fungal disease (IFD), marked by certain T cell responses, and those with proven or possible IFD, showing higher levels of T cells that produced specific cytokines, suggesting these T cells could serve as a potential diagnostic marker for IM.

Practical concept of pharmacokinetics/pharmacodynamics in the management of skin and soft tissue infections.

Curr Opin Infect Dis

aInstitute of Clinical Pharmacology, Azienda Ospedaliero-Universitaria Santa Maria della Misericordia bDepartment of Experimental and Clinical Medical Sciences, University of Udine, Udine, Italy.

Purpose Of Review: This article gives an overview of the practical concept of pharmacokinetic/pharmacodynamic principles useful for clinicians in the management of skin and soft tissue infections (SSTIs).

Recent Findings: Recent studies suggest that distinguishing between bacteriostatic or bactericidal activity when choosing an antimicrobial for the treatment of severe infections could probably be clinically irrelevant. Conversely, what could help clinicians in maximizing the therapeutic efficacy of the various drugs in routine practice is taking care of some pharmacokinetic/pharmacodynamic principles.

The ability to flexibly switch between fast and accurate decisions is crucial in everyday life. Recent neuroimaging evidence suggested that left lateral prefrontal cortex plays a role in switching from a quick response strategy to an accurate one. However, the causal role of the left prefrontal cortex in this particular, non-verbal, strategy switch has never been demonstrated.

Purpose: Recent epidemiological data have confirmed the increasing problem of antimicrobial resistance not only for hospitalized, healthcare-associated patients, but also for outpatients. In particular, the progressive increase in resistance to broad-spectrum antibiotics, such as third-generation cephalosporins, fluoroquinolones or carbapenems in Enterobacteriaceae, is an alarming situation for all urologists and general practitioners. Here, we aimed to review the epidemiological data of multidrug-resistant bacteria in the urological setting, in order to summarize all diagnostic and therapeutic recommendations to use in everyday clinical practice.

Methicillin-resistant Staphylococcus aureus (MRSA) is one of the most challenging bacterial pathogens responsible for severe infections among hospitalized patients. In recent years there is increasing evidence that the clinical efficacy of vancomycin is progressively decreasing. Although daptomycin and linezolid are valuable alternatives to vancomycin for the treatment of MRSA-related bloodstream infections and pneumonia, respectively, a great deal of debate exists about their role in daily clinical practice due to cost-effectiveness issues.
