The Bruton's tyrosine kinase (BTK) inhibitor ibrutinib represents an effective strategy for treatment of chronic lymphocytic leukemia (CLL), nevertheless about 30% of patients eventually undergo disease progression. Here we investigated by flow cytometry the long-term modulation of the CLL CXCR4/CD5 proliferative fraction (PF), its correlation with therapeutic outcome and emergence of ibrutinib resistance. By longitudinal tracking, the PF, initially suppressed by ibrutinib, reappeared upon early disease progression, without association with lymphocyte count or serum beta-2-microglobulin.

Background: Combined triple atlas (C1)-axis (C2) fixation has been described in previous literature as a safe, effective, and minimally invasive procedure for complex atlas and odontoid fractures that allows for a greater range of motion compared with posterior approaches and atlanto-occipital fusion. However, it is rarely performed due to the occipital-cervical diastasis resulting from often-fractured C1 joint masses. No evidence-based consensus has been reached regarding the treatment of complex atlantoaxial fractures, and the choice of surgical strategy is based only on clinical experience.

Cardio-oncology is a rapidly growing field of cardiovascular (CV) medicine that has resulted from the continuously increasing clinical demand for specialized CV evaluation, prevention and management of patients suffering or surviving from malignant diseases. Dealing with CV disease in patients with cancer requires special knowledge beyond that included in the general core curriculum for cardiology. Therefore, the European Society of Cardiology (ESC) has developed a special core curriculum for cardio-oncology, a consensus document that defines the level of experience and knowledge required for cardiologists in this particular field.

The achievement of adequate blood pressure (BP) control and the management of resistant hypertension still represent two burning "unmet needs" in the field of cardiovascular diseases, thus originating a continuous search for more efficacious pharmacological and non-pharmacological strategies including radiofrequency and ultrasound renal denervation (RDN). The most recent guidelines recommend a combination therapy of two or more antihypertensive agents, preferably in a single-pill formulation, as first-line strategy to achieve an effective and early BP control also improving adherence to prescribed treatments. In the last few years several sham-controlled trials have confirmed both the BP-lowering efficacy and safety of RDN in a broad range of patients with hypertension, showing a significant reduction of office and ambulatory BP levels without a relevant increase of procedure-related adverse events such as renal artery stenosis and worsening renal function.

COVID-19 disease has a strong impact on hematological patients; those receiving autologous hematopoietic stem cell transplantation (aHSCT) represent a particularly vulnerable group, in which the effectiveness of vaccination is very variable. Acquired hemophilia A (AHA) is a rare autoimmune disease caused by autoantibodies against coagulation factor VIII and characterized by spontaneous hemorrhage in patients with no previous family or personal history of bleeding. We report here a case of AHA that occurred in the Department of Medicina D'Urgenza in Sant'Andrea Hospital in a patient with previous diagnosis of NSLC. The aim of this article is to allow a more comprehensive knowledge of AHA that both for the rarity and the poor literature is underdiagnosed; for all these reasons, it is important that different specialists, like emergency specialists, experts in internal medicine, hematologists, and oncologists, acquire a more complete knowledge of the clinical and laboratory features of this disease, allowing an early diagnosis crucial for the evolution of the coagulopathy.

In light of the establishment of a permanent technical work table to address issues relating to cannabis for medical use, it is necessary to reflect free from ideological conditioning on the effectiveness and perception of the use of these "products" on the populations indicated in the regulatory note current. Taking a cue from two recently published works, which photograph a poor level of knowledge on medical cannabis by patients, caregivers, and healthcare professionals, we believe it is necessary to overcome ideological preconceptions by building educational programs and national guidelines that offer therapeutic opportunities based on solid scientific evidence.
