Purpose: Gastroenteropancreatic -neuroendocrine tumours (GEP-NETs) are commonly treated with surgical resection or long-term therapies for tumour growth control. Lutetium [Lu]-DOTA-TATE was approved for the treatment of GEP-NETs after the phase III NETTER 1trial demonstrated improved progression free survival, objective response rates and health-related quality of life (HRQoL) compared to high-dose somatostatin analogues. No real-world data exist on prescribing habits and clinically significant endpoints for [Lu]Lu-DOTA-TATE treatment in Italy.

There is much debate about continuing antipsychotic medication in patients who need it when they become pregnant because benefits must be weighed against potential teratogenic and malformation effects related to antipsychotics themselves. To address this, we conducted a systematic review on the PubMed, PsycINFO and CINHAL databases and the ClinicalTrials.gov register using the following strategy: (toxicity OR teratogenicity OR malformation* OR "birth defect*" OR "congenital abnormality" OR "congenital abnormalities" OR "brain changes" OR "behavioral abnormalities" OR "behavioral abnormalities") AND antipsychotic* AND (pregnancy OR pregnant OR lactation OR delivery OR prenatal OR perinatal OR post-natal OR puerperium) on September 27, 2023.

  • Neoadjuvant chemotherapy (NAC) significantly affects breast cancer surgery, leading to a national Consensus Conference by the Italian Association of Breast Surgeons (ANISC) involving various specialists.
  • In November 2022, 164 specialists from 74 Breast Centers participated in an online conference to vote on eleven controversial NAC-related issues, achieving consensus on only three.
  • The results showed a general agreement on assessing tumor response via MRI, reassessing biological factors of residual tumors, and possibly skipping formal axillary node dissection for certain patients, highlighting the variability in surgical practices post-NAC.

Background: An increasing number of novel surgical treatments (NSTs) for benign prostatic hyperplasia (BPH) have been proposed over time to achieve similar functional outcomes, but better perioperative and sexual outcomes than traditional procedures.

Objective: To assess the trends in the utilization and costs of BPH surgical procedures over the past decade, and to analyze the need for surgical retreatment after each procedure.

Design, Setting, And Participants: A retrospective observational population-based analysis was conducted using the PearlDiver Mariner (PearlDiver Technologies, Colorado Springs, CO, USA) database, including all-payer nationally available claims records collected from 2011 to 2022.

Robotic thymectomy in thymic tumours: a multicentre, nation-wide study.

Eur J Cardiothorac Surg

May 2024

Unit of Thoracic Surgery, Department of Cardiac, Thoracic, Vascular Sciences and Public Health, University of Padua, Padua, Italy.

Article Synopsis
  • A large multicenter study across 15 Italian hospitals evaluated the early and long-term outcomes of robot-assisted thoracoscopic thymectomy for thymic epithelial tumors, enrolling 669 patients from 2002 to 2022.
  • The study found that most patients (98%) underwent complete thymectomy with a low incidence of open conversion (3.4%) and no perioperative mortality; however, 7.7% experienced postoperative complications.
  • Results showed effective oncological outcomes with only 2 patients dying from tumor-related causes during follow-up, and the 5- and 10-year recurrence rates were low at 7.4% and 8.3%, respectively, indicating the procedure's safety and effectiveness.

(1) Background: Prostate Cancer (PCa) may be incidentally diagnosed during the microscopic evaluation of resected tissue from BPH surgeries, characterizing the clinical condition known as incidental PCa (iPCa). This study aims to assess the prevalence of iPCa following BPH surgery to evaluate the associated surgical procedures and to scrutinize preoperative and postoperative management. (2) Methods: A retrospective analysis was conducted using the PearlDiver™ Mariner database, containing patient records compiled between 2011 and 2021.

Article Synopsis
  • The study aimed to determine the best treatment sequence for patients with advanced, well-differentiated pancreatic neuroendocrine tumors (pNETs) by analyzing data on progression-free survival (PFS) and overall survival (OS).
  • In total, 201 patients were examined, revealing that those with grade 1 pNETs, prior tumor surgery, and those treated with radioligand therapy (RLT) experienced significantly longer PFS.
  • The findings suggest that RLT is associated with better PFS than other treatments, and that a treatment sequence starting with somatostatin analogs (SSA) followed by RLT leads to improved outcomes.

Department of Internal Medicine, University of Genova, Genova, Italy; Cardiovascular Disease Unit, IRCCS Ospedale Policlinico San Martino, Genova, Italy. Electronic address:

Background: It has been postulated that cancer hampers the delivery of guideline-directed medical therapy (GDMT) for heart failure (HF). However, few data are available in this regard.

Methods: We performed a retrospective analysis from the HF Outpatient Clinic of the IRCCS Ospedale Policlinico San Martino in Genova, Italy.

  • Antibiotic prophylaxis (AB) reduces bacteriuria after invasive urodynamics (UDS) but does not significantly lower urinary tract infection (UTI) rates; guidelines for AB use in high-risk patients remain unclear.
  • A systematic review and expert consensus via a modified Delphi method identified key patient categories at risk for UTIs and proposed 16 statements regarding AB use.
  • The panel of 57 experts largely agreed (56.25%) on the necessity of AB for patients with neurogenic bladder and immunosuppression, while advising against AB for several other factors such as menopausal status and diabetes.

Background: Immune checkpoint inhibitors (ICIs), administered alone or combined with chemotherapy, are the standard of care in advanced non-oncogene addicted Non-Small Cell Lung Cancer (NSCLC). Despite these treatments' success, most long-term survival benefit is restricted to approximately 20% of patients, highlighting the need to identify novel biomarkers to optimize treatment strategies. In several solid tumors, immune soluble factors, the activatory CD137 Tcells, and the immunosuppressive cell subsets Tregs and MDSCs (PMN(Lox1)-MDSC and M-MDSCs) correlated with responses to ICIs and clinical outcomes thus becoming appealing predictive and prognostic factors.

Urinary tract infections (UTIs) are the second most frequent type of infection observed in clinical practice. Gram-negative are common pathogens in UTIs. Excessive antibiotic use in humans and animals, poor infection control, and increased global travel have accelerated the spread of multidrug-resistant strains (MDR).

Combined Anterior Odontoid Screw Fixation and C1-C2 Jefferson Fracture Anterior Fixation: A Step-by-Step Technical Note.

World Neurosurg

April 2024

Experimental Neurosurgery Unit, IRCCS Ist Neurologico Mediterraneo Neuromed, Pozzilli, Italy; Department of Neurosurgery, Policlinico Umberto I, Sapienza University of Rome, Rome, Italy. Electronic address:

Background: Combined triple atlas (C1)-axis (C2) fixation has been described in previous literature as a safe, effective, and minimally invasive procedure for complex atlas and odontoid fractures that allows for a greater range of motion compared with posterior approaches and atlanto-occipital fusion. However, it is rarely performed due to the occipital-cervical diastasis resulting from often-fractured C1 joint masses. No evidence-based consensus has been reached regarding the treatment of complex atlantoaxial fractures, and the choice of surgical strategy is based only on clinical experience.
