Until recently, the use of direct contrast examinations with oral or intracavitary contrast agents was widespread in all general radiology departments. It provided significant answers to multiple clinical questions on the enteric organs and their mucosal surfaces. With the increased availability of CT and MRI, which also allow for the study of walls, as well as the accessibility of endoscopic examinations, the request and execution of fluoroscopy contrast studies have fallen considerably.

Article Synopsis
  • Intravenous thrombolysis (IVT) and endovascular therapy (EVT) are effective treatments for acute stroke, but their safety and efficacy in patients with atrial fibrillation (AF) are debated.
  • A study analyzed data from two multicenter studies to compare outcomes of AF patients with acute ischemic stroke treated with reperfusion therapies versus those who weren't.
  • Results showed that AF patients treated with reperfusion therapies had better functional outcomes and lower mortality rates compared to those who received conservative treatment.

  • The Supera stent is a special medical device that helps improve treatments for difficult artery problems in the leg.
  • Researchers studied how effective this stent is after a procedure called subintimal angioplasty, which is used to open blocked arteries.
  • The results showed that using the Supera stent led to successful outcomes for almost all patients, and it helped them stay healthy over time with fewer medical resources needed.

Background: Carotid near-occlusion (CNO) represents an anatomical-functional condition characterized by severe (more than 90%) internal carotid artery stenosis which can lead to a distal lumen diameter greater or less than 2 mm. CNO can be divided into a less severe subgroup (without lumen full collapse: diameter >2 mm) and a more severe subgroup (with lumen full collapse: diameter <2 mm). The decision for revascularization is still highly debated in Literature.

The purpose of the study was to describe the characteristics of patients experiencing hypersensitivity reactions (HRs) to iodinated contrast media (ICM) in a large Italian population and to investigate potential risks factors in order to obtain a risk stratification, helpful in the management of these patients. Data of 407 patients investigated in 9 Italian Allergy Centers for suspected HRs to ICM were analyzed and compared with a control group of 152 subjects that tolerated one or more ICM-enhanced examinations. The univariate and multivariate logistic regression model was used to evaluate associated factors.

Family caregivers of patients with dementia are at high risk of stress and burden, and quarantine due to the coronavirus disease 2019 (COVID-19) pandemic may have increased the risk of psychological disturbances in this population. The current study was carried out during the national lockdown declared in March 2020 by the Italian government as a containment measure of the first wave of the coronavirus pandemic and is the first nationwide survey on the impact of COVID-19 lockdown on the mental health of dementia informal caregivers. Eighty-seven dementia centers evenly distributed on the Italian territory enrolled 4,710 caregiver-patient pairs.

Article Synopsis
  • The study investigates when it's best to start oral anticoagulants after an ischemic stroke caused by atrial fibrillation, particularly focusing on patients who received thrombolysis or thrombectomy.
  • It combines data from two studies conducted between 2012 and 2016, analyzing patients treated with either Vitamin K antagonists or non-vitamin K oral anticoagulants for secondary stroke prevention.
  • Among 2159 patients included, the timing for starting anticoagulants was similar for those treated with reperfusion therapies and those not treated, with low rates of stroke or bleeding complications observed within 90 days.

Heritable and non-heritable uncommon causes of stroke.

J Neurol

August 2021

Stroke Research Group, Department of Clinical Neurosciences, University of Cambridge, Cambridge, UK.

Despite intensive investigations, about 30% of stroke cases remains of undetermined origin. After exclusion of common causes of stroke, there is a number of rare heritable and non-heritable conditions, which often remain misdiagnosed, that should be additionally considered in the diagnosis of cryptogenic stroke. The identification of these diseases requires a complex work up including detailed clinical evaluation for the detection of systemic symptoms and signs, an adequate neuroimaging assessment and a careful family history collection.

The association between preceding treatment with antiplatelet agents (APs), vitamin K antagonists (VKAs) or direct oral anticoagulants (DOACs) and mortality after intracerebral hemorrhage (ICH) remains unclear. The aim of this multicenter, prospective cohort study was to assess the risk for death after ICH in consecutive patients who were on treatment with APs, VKAs, DOACs, or no antithrombotic agent. The primary outcome was in-hospital death by day 30.

Article Synopsis
  • * Researchers analyzed data from 1,810 patients, discovering that 20% underwent this bridging therapy, and found that bridged patients had worse outcomes, with a higher rate of ischemic events and major bleeding.
  • * The results indicate that bridging therapy significantly raises the risk of early complications compared to patients who did not receive it, prompting a reevaluation of its common use in clinical practice.

Article Synopsis
  • The study examined the link between types of atrial fibrillation (paroxysmal vs. sustained) and the occurrence of early ischaemic recurrences after an acute stroke in patients.
  • Out of 2150 patients, those with sustained atrial fibrillation experienced a higher percentage of early ischaemic recurrences (6.2%) compared to those with paroxysmal atrial fibrillation (3.3%).
  • However, after considering other health risks, sustained atrial fibrillation did not show a significantly increased risk for early recurrence, suggesting that other factors may be more influential.

Background: GENetics of mOyaMoyA (GEN-O-MA) project is a multicenter observational study implemented in Italy aimed at creating a network of centers involved in moyamoya angiopathy (MA) care and research and at collecting a large series and bio-repository of MA patients, finally aimed at describing the disease phenotype and clinical course as well as at identifying biological or cellular markers for disease progression. The present paper resumes the most important study methodological issues and preliminary results.

Methods: Nineteen centers are participating to the study.

Background In patients with acute ischemic stroke and atrial fibrillation, early anticoagulation prevents ischemic recurrence but with the risk of hemorrhagic transformation ( HT ). The aims of this study were to evaluate in consecutive patients with acute stroke and atrial fibrillation (1) the incidence of early HT, (2) the time to initiation of anticoagulation in patients with HT , (3) the association of HT with ischemic recurrences, and (4) the association of HT with clinical outcome at 90 days. Methods and Results HT was diagnosed by a second brain computed tomographic scan performed 24 to 72 hours after stroke onset.

After acute proximal deep vein thrombosis (DVT) the thrombotic mass decreases, especially during the first months of anticoagulation. The persistence of residual vein obstruction (RVO) may predict future recurrence in patients with cancer-associated DVT. We aimed to evaluate the proportion of patients with RVO after an episode of cancer associated isolated distal DVT (IDDVT), to identify variables associated with RVO, and to provide initial evidence of its association with recurrent VTE.

Article Synopsis
  • The study investigates the optimal timing for administering non-vitamin K oral anticoagulants (NOACs) in patients with acute ischemic stroke and atrial fibrillation, focusing on early recurrence of stroke and major bleeding within 90 days post-treatment.
  • A total of 1,127 patients participated, with diverse treatment groups receiving dabigatran, rivaroxaban, or apixaban; results revealed a low incidence of early recurrence (2.8%) and major bleeding (2.4%).
  • Patients starting NOACs within 2 days of stroke had higher rates of complications (12.4% composite rate), compared to delayed initiation (3-14 days had 2.1%, and >14 days

Introduction: Venous thromboembolism (VTE) is a common and potentially fatal disease. Fondaparinux is a synthetic agent able to act on single factors involved in the coagulation network, which could be administered at fixed doses and with a more predictable response.

Areas Covered: This review will focus on the efficacy and safety of fondaparinux in the treatment of major VTE (deep vein thrombosis and pulmonary embolism) and in the treatment of superficial vein thrombosis (SVT).
