Immune checkpoint inhibitors (ICIs) have recently gained recognition as valuable treatment options for a variety of cancers. Pembrolizumab is a monoclonal antibody that acts as an inhibitor of programmed cell death receptor-1 (PD-1). This helps release host T-cells from regulatory inhibition by tumor neoantigens, therefore mediating antitumor effects.

Leiomyosarcomas are malignant soft tissue tumors that can arise nearly anywhere in the body. These tumors may rarely originate within the retroperitoneum and may be found incidentally or present with nonspecific symptoms due to mass effect on abdominal or pelvic organs. The presentation of a leiomyosarcoma with a massively elevated CA 19-9 level is exceedingly rare with only one other report in the literature.

Article Synopsis
  • Invasive pneumococcal disease primarily affects high-risk individuals, including those with asplenia and certain immune deficiencies, and can lead to severe complications like DIC and purpura fulminans, which have high mortality rates.
  • A case study details an immunocompetent woman in her 50s with an intact spleen who developed septic shock and quickly progressed to multiorgan dysfunction and purpura fulminans.
  • Despite receiving intensive treatment—including broad-spectrum antibiotics, factor replacement, and supportive therapies—she ultimately died from multiorgan failure.

We present a case of new-onset haemoptysis and associated lung lesion on chest imaging in the setting of the COVID-19 pandemic. This was believed to be due to dental care avoidance after tooth fracture and long-term use of temporary dental filler, with subsequent aspiration and bronchial injury. Our patient underwent bronchoscopy due to persistent haemoptysis with findings of mild traumatic injury.

(PA), a gram-negative rod-shaped bacterium, is one of the most common pathogens causing colonization and infection of the respiratory tract and lungs in patients with cystic fibrosis (CF). Antibiotic therapy is the mainstay treatment for PA infection, and tobramycin is one of the widely used antibiotics in intravenous or inhalation form. This review aims to explore if there is any advantage of adding systemic antibiotics to tobramycin inhalation therapy by comparing the combination regimen to tobramycin inhalation monotherapy in CF patients with PA infection.

Updates on the Management of Neurologic Complications of Post-Cardiac Arrest Resuscitation.

Semin Neurol

August 2021

Division of Neurosciences Critical Care, Neurology, Neurosurgery and Critical Care Medicine, Johns Hopkins University School of Medicine, Baltimore, Maryland.

Sudden cardiac arrest (SCA) is one of the leading causes of mortality and morbidity in the United States, and survivors are frequently left with severe disability. Of the 10% successfully resuscitated from SCA, only around 10% of these live with a favorable neurologic outcome. Survivors of SCA commonly develop post-cardiac arrest syndrome (PCAS).

Genetic meta-analysis of twin birth weight shows high genetic correlation with singleton birth weight.

Hum Mol Genet

September 2021

Department of Biological Psychology, Amsterdam Public Health Research Institute, Vrije Universiteit, Amsterdam, The Netherlands.

Birth weight (BW) is an important predictor of newborn survival and health and has associations with many adult health outcomes, including cardiometabolic disorders, autoimmune diseases and mental health. On average, twins have a lower BW than singletons as a result of a different pattern of fetal growth and shorter gestational duration. Therefore, investigations into the genetics of BW often exclude data from twins, leading to a reduction in sample size and remaining ambiguities concerning the genetic contribution to BW in twins.

After the novel coronavirus SARS-CoV-2 was declared a pandemic in mid-March, it challenged healthcare systems to provide care while mitigating the risk to both patients and staff. The high transmissibility of the virus combined with PPE shortages made this an even more difficult task. A Catholic health system in Sioux Falls, South Dakota, embraced healthcare technology in order to bridge the gap between providing patient care, exposure to the virus, and lack of PPE.

It is estimated that 25-30 percent of known pregnancies result in early pregnancy loss, which can contribute to significant mental health issues for women. In a local health system, it was identified that there was no standard of care for mental health resources offered to patients experiencing a first trimester miscarriage. To create a model of care and a set of resources that the health system could offer, miscarriage patients nationwide were surveyed about resources and care they received or did not receive, and how that contributed to their satisfaction.

Chimerism in health and potential implications on behavior: A systematic review.

Am J Med Genet A

June 2020

Netherlands Twin Register, Department of Biological Psychology, Vrije Universiteit, Amsterdam, The Netherlands.

In this review, we focus on the phenomenon of chimerism and especially microchimerism as one of the currently underexplored explanations for differences in health and behavior. Chimerism is an amalgamation of cells from two or more unique zygotes within a single organism, with microchimerism defined by a minor cell population of <1%. This article first presents an overview of the primary techniques employed to detect and quantify the presence of microchimerism and then reviews empirical studies of chimerism in mammals including primates and humans.

Division of Pulmonary, Critical Care, Sleep and Allergy, University of Nebraska Medical Center, Omaha, Nebraska.

Swyer-James-Macleod syndrome is a rare radiologic entity with radiolucent lung fields which could include the entire lung, a lobe or a subsegment. The pathogenesis is unclear but is thought to be related to childhood pulmonary viral and non-viral infections with subsequent vascular and alveolar distortion. It usually presents incidentally in asymptomatic patients on imaging, but could present with recurrent pulmonary infections, shortness of breath, cough, hemoptysis or bronchiectasis.
