Middleton Memorial Veterans Hospital, Madison, WI, United States.

Article Synopsis
  • The study evaluates the effectiveness of the Geriatric Depression Scale (GDS) in detecting depression among older adults from various ethno-racial backgrounds, specifically focusing on White, Black/African-American, and American Indian/Alaska Native groups.
  • The analysis utilized data from the Wisconsin Alzheimer's Disease Research Center, with participants reporting depressive symptoms through multiple GDS versions and showing significant internal consistency and correlation with dementia-related metrics.
  • Findings indicate that Black participants reported higher depressive symptoms, while American Indian/Alaska Native participants reported fewer symptoms compared to Black participants, underscoring the need for tailored assessments considering ethno-racial differences.

Acute and chronic sodium-glucose cotransporter 2 (SGLT-2) inhibition increases endogenous glucose production (EGP). However, the organ-liver versus kidney-responsible for the increase in EGP has not been identified. In this study, 20 subjects with type 2 diabetes (T2D) and 12 subjects with normal glucose tolerance (NGT) received [3-3H]glucose infusion (to measure total EGP) combined with arterial and renal vein catheterization and para-aminohippuric acid infusion for determination of renal blood flow.

Unlabelled: Roux-en-Y gastric bypass (GB) and sleeve gastrectomy (SG) surgeries increase prandial insulin and glucagon secretion but reduce the endogenous glucose production (EGP) response to hypoglycemia in comparison with control subjects who had not undergone gastric surgery (CN), suggesting that parasympathetic nervous system (PNS) plays a role. Here, we investigated the effect of acute PNS blockade on the post-meal counterregulatory response to insulin-induced hypoglycemia in GB and SG compared with CN. Glucose kinetics and islet cell secretion were measured in nine subjects without diabetes with GB and seven with SG and five CN during hyperinsulinemic-hypoglycemic clamp (∼3.

Background: Real-world evidence is limited regarding the relationship between race and use of durvalumab, an immunotherapy approved for use in adults with unresectable stage III non-small cell lung cancer (NSCLC) post-chemoradiotherapy (CRT). This study aimed to evaluate if durvalumab treatment patterns differed by race in patients with unresectable stage III NSCLC in a Veterans Health Administration (VHA) population.

Materials And Methods: This was a retrospective analysis of White and Black adults with unresectable stage III NSCLC treated with durvalumab presenting to any VHA facility in the US from January 1, 2017, to June 30, 2020.

Purpose: Severe community-acquired pneumonia (sCAP) is associated with high morbidity and mortality, and whilst European and non-European guidelines are available for community-acquired pneumonia, there are no specific guidelines for sCAP.

Methods: The European Respiratory Society (ERS), European Society of Intensive Care Medicine (ESICM), European Society of Clinical Microbiology and Infectious Diseases (ESCMID), and Latin American Thoracic Association (ALAT) launched a task force to develop the first international guidelines for sCAP. The panel comprised a total of 18 European and four non-European experts, as well as two methodologists.

Article Synopsis
  • Severe community-acquired pneumonia (sCAP) lacks specific international guidelines, prompting a task force from major European and Latin American respiratory societies to create the first set of recommendations.
  • The task force, including experts from various fields, addressed eight key clinical questions regarding sCAP diagnosis and treatment through systematic literature reviews and meta-analyses, assessing evidence quality with the GRADE system.
  • The resulting guidelines offer evidence-based recommendations on diagnosis, antibiotic use, and treatment strategies for sCAP, while also identifying existing knowledge gaps and suggesting areas for future research.

Division of Pulmonary Diseases and Critical Care Medicine, Department of Medicine, UT Health San Antonio, 7703 Floyd Curl Dr., San Antonio, TX 78229, USA.

Background: Corticosteroids (CSs), specifically dexamethasone (DEX), are the treatment of choice for severe acute respiratory distress syndrome (ARDS) due to COVID-19 pneumonia (CARDS). However, data from both ARDS and relatively small CARDS clinical trials have suggested improved outcomes with methylprednisolone (MP) DEX. The objective of this retrospective cohort study was to compare the safety and effectiveness of MP and DEX in critically ill CARDS patients.

Background: Aberrant dopamine neuron activity is attributable to hyperactivity in hippocampal subfields driving a pathological increase in dopamine neuron activity, which is positively correlated with psychosis in humans. Evidence indicates that hippocampal hyperactivity is due to loss of intrinsic GABAergic (gamma-aminobutyric acidergic) inhibition. We have previously demonstrated that hippocampal GABAergic neurotransmission can be modulated by targeting α5-GABA receptors, which are preferentially expressed in hippocampal regions.

Article Synopsis
  • - The study explores a new sputum-based test called CyPath Lung, which combines flow cytometry and machine learning to help doctors make decisions regarding lung nodules identified through low-dose spiral CT scans.
  • - The test analyzes induced sputum samples for cancer-associated cells, achieving a predictive model with high accuracy (AUC of 0.89) and strong sensitivity and specificity, particularly for smaller nodules.
  • - The results suggest that CyPath Lung is a promising tool for identifying lung cancer risks in high-risk patients and maintains effectiveness across various sample processing conditions.

To better understand VA providers' approaches to and perspectives on providing care to women Veterans, providers (n = 97) in primary care and mental health settings were interviewed about women's perceived treatment needs, types of care provided, and perceptions of evidence-based treatments (EBTs) for this population. Providers perceived that women Veteran VA users are often diagnostically complex and require a coordinated approach to treatment planning. They struggled with decisions about how to offer services such as EBTs and collaborative care in light of comorbidity and psychosocial stressors, and endorsed the belief that a tailored approach and consideration of these factors is essential in providing care.

The prevalence of pain has been reported to be >60-70% among patients with advanced and end-stage kidney disease. Although the underlying etiologies of pain may vary, pain per se has been linked to lower quality of life and depression. The latter is of great concern given its known association with reduced survival among patients with end-stage kidney disease.

Chronic infections are associated with exacerbation in patients with chronic obstructive pulmonary disease (COPD). The major objective of the management of these patients is the prevention and effective treatment of exacerbations. Patients that have increased sputum production, associated with purulence and worsening shortness of breath, are the ones that will benefit from antibiotic therapy.

Background: Exacerbation frequency is related to disease progression, quality of life, and prognosis in COPD. Earlier diagnosis, along with interventions aimed at preventing exacerbations and delaying progression, may help reduce the global burden of disease. Long-acting inhaled bronchodilators are effective at maintaining symptom relief and are recommended as first-choice therapy for more symptomatic patients and those at risk of exacerbation.
