Article Synopsis
  • Mucosal melanomas, particularly in the oral and sinonasal regions, present diagnostic challenges due to their varied characteristics and differentiation patterns, including neuroendocrine features.
  • A study of 18 cases revealed that 72.2% displayed neuroendocrine differentiation, suggesting it is a common feature in these types of melanoma.
  • The research highlights the importance of recognizing neuroendocrine markers (like synaptophysin, chromogranin, CD56, and INSM-1) in preventing misdiagnoses in malignant melanomas.

Background: Adjuvant therapy with durvalumab, with or without tremelimumab, may have efficacy in patients with limited-stage small-cell lung cancer who do not have disease progression after standard concurrent platinum-based chemoradiotherapy.

Methods: In a phase 3, double-blind, randomized, placebo-controlled trial, we assigned patients to receive durvalumab at a dose of 1500 mg, durvalumab (1500 mg) plus tremelimumab at a dose of 75 mg (four doses only), or placebo every 4 weeks for up to 24 months. Randomization was stratified according to disease stage (I or II vs.

Respiratory Translational Research Group, Department of Laboratory Medicine, School of Health Sciences, College of Health and Medicine, University of Tasmania, Locked Bag - 1322, Newnham Drive, Launceston, TAS, 7248, Australia.

The WHO recently published a Tobacco Knowledge Summary (TKS) synthesizing current evidence on tobacco and COPD, aiming to raise awareness among a broad audience of health care professionals. Furthermore, it can be used as an advocacy tool in the fight for tobacco control and prevention of tobacco-related disease. This article builds on the evidence presented in the TKS, with a greater level of detail intended for a lung-specialist audience.

Background: It is unclear whether targeted monitoring of acute adrenal insufficiency (AI) related adverse events (AE) such as sick day episodes (SDEs) and hospitalization rate in congenital adrenal hyperplasia (CAH) is associated with a change in the occurrence of these events.

Aim: Study temporal trends of AI related AE in the I-CAH Registry.

Methods: In 2022, data on the occurrence of AI-related AE in children aged <18 years with 21-hydroxylase deficiency CAH were compared to data collected in 2019.

Background: One of the most commonly experienced symptoms of premenstrual disorder (PMD) is anxiety, and there is a notable rise in sympathomimetic activity in this patient group. Studies have linked fluctuations in systemic autonomic tone to electrocardiography (ECG) changes. This study aims to investigate the relationship between anxiety, a common symptom of PMD, and alterations in QT dispersion (QTd) and P-wave dispersion (Pd) in adolescent females.

Article Synopsis
  • Laser lithotripsy can break down cystine stones, potentially producing toxic gas byproducts, specifically during the use of thulium fiber laser (TFL) but this has not been thoroughly studied.
  • * An in vitro study was conducted to analyze the gases released when cystine stones were fragmented with TFL, revealing that gas formation only occurred with cystine stones, producing gases like sulfur and hydrogen sulfide.
  • * Analysis of the remaining stone fragments showed they contained various chemical elements, pointing to a risk of toxic gas emissions that could affect patients and operating room staff during procedures.

THE effect of P2X7 receptor activation on functional responses of human left internal mammary artery.

Naunyn Schmiedebergs Arch Pharmacol

September 2024

Department of Medical Pharmacology, Akdeniz University Medical Faculty, Dumlupinar Avenue, 07070, Antalya, Turkey.

The Purinoreceptor 7 (P2X7R) has become a promising drug target in many cardiovascular diseases, including coronary artery disease, since prolonged activation of P2X7R could promote vascular dysfunction, atherosclerosis, and thrombosis. Thus, we aimed to study the effects of P2X7R activation on vascular relaxation responses of the human left internal mammary artery (LIMA). Sections of redundant human LIMA were cut into 3-mm wide rings,, suspended in 20-mL organ baths containing physiologic salt solution, and attached to an isometric force transducer connected to a computer-based data acquisition system.

Article Synopsis
  • Steroidal mineralocorticoid receptor antagonists help patients with heart failure and reduced ejection fraction, but their effectiveness in those with mildly reduced or preserved ejection fraction is unclear, indicating a need for further research on finerenone.
  • In a double-blind study, patients with heart failure (ejection fraction 40% or greater) were assigned to receive either finerenone or a placebo to assess its impact on heart failure events and cardiovascular death.
  • Results showed that finerenone led to fewer worsening heart failure events and a lower overall rate of primary outcome events compared to placebo, although it also carried a higher risk of hyperkalemia.

Article Synopsis
  • This study analyzed 331 patients with primary brain abscesses (PBAs) from 39 centers in 16 countries, focusing on infection causes, risk factors, and mortality outcomes over a 12-year period.
  • Study results showed that the majority of patients were middle-aged men (average age 46.8), with symptoms like headaches and fever; Gram-positive cocci were the most common pathogens.
  • Key factors influencing mortality at 42 days included intravenous drug use and malignancy, while factors affecting 180-day mortality included temporal lobe involvement and the presence of unidentified bacteria.

Glioma resection is associated with prolonged survival, but neuro-oncological trials have frequently refrained from quantifying the extent of resection. The Response Assessment in Neuro-Oncology (RANO) resect group is an international, multidisciplinary group that aims to standardise research practice by delineating the oncological role of surgery in diffuse adult-type gliomas as defined per WHO 2021 classification. Favourable survival effects of more extensive resection unfold over months to decades depending on the molecular tumour profile.

Article Synopsis
  • The study focuses on developing a new drug, "IHNOCS," aimed at treating Head, Neck, and Oral Cancer by targeting specific proteins to suppress cancer progression.
  • Using advanced computational tools, researchers demonstrated that IHNOCS effectively inhibits the KRTAP2-3 protein and modulates TGF-β, which helps to slow down cancer metastasis.
  • While the drug shows promising results in theory, it will need to go through further experimental testing, including lab studies and clinical trials, to evaluate its effectiveness in real-world situations.

The combination of Lutetium-177 (Lu-177) PSMA-617 radioligand therapy (RLT) with androgen receptor pathway inhibitors (ARPIs) has shown promise in metastatic castration-resistant prostate cancer (mCRPC). However, real-world data on the efficacy and safety of this combination are limited. This study aimed to evaluate the impact of combination therapy with Lu-177 PSMA-617 RLT and ARPIs on progression-free survival (PFS) and overall survival (OS) in patients with mCRPC.

Article Synopsis
  • The study aimed to evaluate the disease burden of familial partial lipodystrophy (FPLD) caused by genetic variants from LMNA and PPARG, with a focus on understanding these rare conditions linked to selective fat loss and metabolic issues.
  • The research involved 157 patients from Turkey and Spain, revealing that symptoms typically appeared around 20 years after initial clinical signs, with notable differences in onset and severity between the two types of FPLD, particularly regarding diabetes and cardiovascular issues.
  • Key findings indicate that FPLD2 is generally more severe than FPLD3, but the latter still poses significant metabolic risks; Turkish patients showed worse health outcomes like lower body mass index and higher prevalence of liver fat, with cardiovascular
