Article Synopsis
  • Transthyretin amyloidosis with cardiomyopathy (ATTR-CM) is a serious, progressive disease, and vutrisiran is a new treatment that works by reducing the production of transthyretin in the liver.
  • In a double-blind trial involving 655 patients, those receiving vutrisiran had a lower risk of death and cardiovascular events compared to those on placebo, demonstrating significant efficacy.
  • Vutrisiran also improved patient outcomes, showing less decline in walking distance and quality of life measurements over the study period compared to placebo.

Isolated intraocular relapses of primary cerebral lymphomas: An LOC network study.

Hematol Oncol

December 2022

Neurology, Assistance Publique - Hôpitaux de Paris (APHP), Sorbonne Université, IHU, ICM, Groupe Hospitalier Pitié-Salpêtrière, Paris, France.

Article Synopsis
  • - Most relapses of primary central nervous system lymphoma (PCNSL) typically occur in the brain and have a poor prognosis, while isolated intraocular relapses (IIORs) are rare and not well understood.
  • - A study analyzed 55 PCNSL patients with IIOR and found that a significant number had high IL-10 levels in their eye fluid, and the majority received systemic chemotherapy, with some undergoing high-dose chemotherapy followed by stem cell transplantation (HCT-ASCT).
  • - After an average follow-up of nearly 6 years, 76% of patients relapsed, especially those who didn't have HCT-ASCT; however, prognosis was better for IIOR compared to brain relapses, undersc

Global guideline for the diagnosis and management of rare yeast infections: an initiative of the ECMM in cooperation with ISHAM and ASM.

Lancet Infect Dis

December 2021

National Institute for Communicable Diseases (Centre for Healthcare-Associated Infections, Antimicrobial Resistance and Mycoses), a Division of the National Health Laboratory Service, Johannesburg, South Africa; School of Pathology, Faculty of Health Sciences, University of the Witwatersrand, Johannesburg, South Africa.

Uncommon, or rare, yeast infections are on the rise given increasing numbers of patients who are immunocompromised or seriously ill. The major pathogens include those of the genera Geotrichum, Saprochaete, Magnusiomyces, and Trichosporon (ie, basidiomycetes) and Kodamaea, Malassezia, Pseudozyma (ie, now Moesziomyces or Dirkmeia), Rhodotorula, Saccharomyces, and Sporobolomyces (ie, ascomycetes). A considered approach to the complex, multidisciplinary management of infections that are caused by these pathogens is essential to optimising patient outcomes; however, management guidelines are either region-specific or require updating.

Use of an external fixator during one-step surgical management of missed chronic Monteggia injuries.

Orthop Traumatol Surg Res

February 2022

Service de Chirurgie Orthopédique et Réparatrice de l'Enfant, hôpital Trousseau, Université Sorbonne, Paris, France.

The incidence of untreated Monteggia fracture-dislocations is not insignificant. But there is no consensus as to the surgical indication and best surgical technique to use. Single-stage reduction of the radial head through an elbow arthrotomy and ulnar osteotomy is a complex surgical procedure that requires stabilization of the ulnar segments during every step of the intervention.

Article Synopsis
  • * Results showed that 40 out of 57 patients achieved a complete response after chemotherapy, with no severe toxicity or treatment discontinuation due to side effects noted.
  • * Although the treatment improved overall survival rates, a significant recurrence of the disease was observed, particularly in the eyes and brain, indicating a need for better strategies to manage local disease control and to monitor specific biomarkers like aqueous humor IL-10.

The perihepatic space is frequently involved in a spectrum of diseases, including intrahepatic lesions extending to the liver capsule and disease conditions involving adjacent organs extending to the perihepatic space or spreading thanks to the communication from intraperitoneal or extraperitoneal sites through the hepatic ligaments. Lesions resulting from the dissemination of peritoneal processes may also affect the perihepatic space. Here we discuss how to assess the perihepatic origin of a lesion and describe the magnetic resonance imaging (MRI) features of normal structures and fluids that may be abnormally located in the perihepatic space.
