  • Coxsackievirus A16 (CV-A16) is a major cause of hand, foot, and mouth disease (HFMD) and can lead to severe complications like encephalitis and myocarditis.
  • An epidemiological surveillance system in Beijing detected co-circulation of CV-A16 clades B1a and B1b from 2019 to 2021, with clade B1c emerging in 2022.
  • In a study involving 400 HFMD and 493 herpangina cases, CV-A16 was found in 18.89% of enterovirus-positive samples, highlighting the need for increased monitoring of the B1c clade in the region.

Coxsackievirus B1 (CVB1) is a common cause of acute and chronic myocarditis, dilated cardiomyopathy and aseptic meningitis. However, no CVB-vaccines are available for human use. In this study, we investigated the immunogenicity of virus-like particle (VLP) and inactivated whole-virus vaccines for CVB1 when administrated to mice via either subcutaneous or intranasal routes formulated with and without commercial and experimental adjuvants.

  • Mild encephalitis/encephalopathy with reversible splenial lesion (MERS) is a neurological condition primarily triggered by infections, particularly mumps in children, leading to reversible lesions in the brain.
  • A 23-year-old male patient exhibited typical mumps symptoms, alongside neurological issues such as dizziness, seizures, and meningeal syndrome, confirmed by MRI and cerebrospinal fluid analysis.
  • The patient received symptomatic and antiviral treatment, resulting in a positive outcome with full recovery and no lasting effects.

  • * A study analyzed CSF from COVID-19 patients with neurological issues, comparing it to cases of viral CNS infections and aseptic neuroinflammation, focusing on differences in metabolic profiles.
  • * Results showed low neuroinflammation in neuro-COVID, with specific metabolites like citrulline and ceramide significantly increased, while triglyceride levels were notably lower, indicating distinct metabolic pathways compared to other CNS infections.

New Insights Into the Therapeutic Management of Varicella Zoster Virus Meningitis: A Series of 123 Polymerase Chain Reaction-Confirmed Cases.

Open Forum Infect Dis

July 2024

Department of Infectious Diseases, Saint Louis-Lariboisière Hospital, Assistance Publique-Hôpitaux de Paris, Université Paris Cité, Paris, France.

Background: Varicella zoster virus (VZV) can reactivate and cause meningitis, but few studies have distinguished it from meningoencephalitis regarding treatment recommendations. The objective of this study was to assess the outcomes of a large series of patients with VZV meningitis according to their therapeutic management.

Methods: We conducted a bicentric retrospective cohort study, in Paris, France, including all adult patients with a cerebrospinal fluid sample positive for VZV by polymerase chain reaction between April 2014 and June 2022.

Tick-borne encephalitis (TBE) is usually diagnosed based on the presence of TBE virus (TBEV)-specific IgM and IgG antibodies in serum. However, antibodies induced by vaccination or cross-reactivity to previous flavivirus infections may result in false positive TBEV serology. Detection of TBEV RNA may be an alternative diagnostic approach to detect viral presence and circumvent the diagnostic difficulties present when using serology.

Mixed connective tissue disorder (MCTD) is the first overlap syndrome described with features of overlapping manifestations of at least two other autoimmune rheumatic conditions. It is an autoimmune disease of rarity and is strongly associated with specific antibodies to U1 small nuclear ribonucleoprotein (anti-U1-RNP). This disorder affects almost all organs of the body, and it has varied clinical presentations as it has an autoimmune and inflammatory background, causing heightened immune cell activation.

Background: Traditionally, 6-month courses of prednisolone are used to treat steroid-responsive meningitis-arteritis (SRMA), but this medication is associated with adverse effects that can lead to poor quality of life.

Hypothesis/objectives: Resolution of clinical signs and rate of relapse of SRMA would not be significantly different between a 6-month prednisolone protocol and a 6-week protocol.

Animals: Forty-four hospital cases from multiple referral centers in the United Kingdom (2015-2019).

Pediatr Rheumatol Online J

June 2024

Respiratory Department II, National Clinical Research Center for Respiratory Diseases, Beijing Children's Hospital, Capital Medical University, National Center for Children's Health, NO.56, Nanlishi Road, Beijing, 100045, China.

Objectives: To characterize the clinical features and to identify the predictors of recurrence of histiocytic necrotizing lymphadenitis (HNL) in Chinese children.

Study Design: This study retrospectively analyzed the clinical characteristics, laboratory and pathological findings, and recurrence status of children diagnosed with HNL at a single center in China from January 2018 to May 2023. Logistic regression analysis was employed to identify predictors of HNL recurrence.

Aseptic meningitis is a rare but serious complication of treatment with intravenous immunoglobulin (IVIG) and often mimics meningitis of infectious etiology which poses a challenge for timely diagnosis. Although there are published recommendations on the management of IVIG-induced complications, there are no clear guidelines on the continuation of IVIG use after resolution of aseptic meningitis. We present a case of IVIG-induced aseptic meningitis in a patient with a history of refractory dermatomyositis who had been treated with immunosuppressive therapy and IVIG infusions for over a year.

Article Synopsis
  • Acute aseptic meningitis has been linked to intravenous polyclonal human immunoglobulin therapy, with 71 documented cases found in a systematic review and meta-analysis.
  • Most cases (90%) occurred within three days of starting treatment and resolved quickly, especially in children, often within three days.
  • The analysis showed a 0.6% prevalence of meningitis among over 4,000 patients treated with immunoglobulins, indicating a possible association between rapid infusions and the onset of meningitis, which usually resolves after stopping the therapy.

The constellation of fevers accompanied by headache and vomiting is a red flag for clinicians that appropriately triggers evaluation for meningitis and other life-threatening diagnoses. When symptoms persist even after these conditions are ruled out, patient care becomes more challenging. We present the case of a 6-year-old male with a history of autism spectrum disorder who presented with 6 months of headaches and associated vomiting and intermittent fevers with negative infectious workup despite cerebrospinal fluid pleocytosis.

Aseptic meningitis and Fabry disease.

Ann Clin Transl Neurol

June 2024

Internal Medicine Department, Reference Centre for Lysosomal Diseases, Groupe Hospitalier Diaconesses Croix Saint-Simon, Paris, France.

Article Synopsis
  • Fabry disease results from a deficiency in the enzyme alpha-galactosidase A, leading to a buildup of glycosphingolipids and causing various health issues, including neurologic symptoms like neuropathic pain and strokes, with aseptic meningitis being a potential complication.
  • A study reviewed patients from 1995 to 2023, finding that 3.7% experienced aseptic meningitis, with additional data from published cases leading to a total of 29 patients analyzed, predominantly young adults.
  • The findings indicate that many of these patients were unaware they had Fabry disease before their meningitis episode, and the underlying causes of the inflammation in these cases are still not fully understood.
