Diabetic kidney disease (DKD) is the leading cause of end-stage kidney disease (ESKD). Prognostic biomarkers reflective of underlying molecular mechanisms are critically needed for effective management of DKD. A three-marker panel was derived from a proteomics analysis of plasma samples by an unbiased machine learning approach from participants (N = 58) in the Clinical Phenotyping and Resource Biobank study.

Article Synopsis
  • The study aimed to create and validate a quick bedside tool that helps identify patients at high risk for long-term posttraumatic stress symptoms following a car accident.
  • Researchers collected data from over 1,500 adults who had been treated in emergency departments after such trauma, ultimately focusing on an 8-question tool that assesses various risk factors.
  • The tool showed good effectiveness in predicting significant posttraumatic stress symptoms, but further research and validation are needed to enhance its accuracy and usefulness in public health interventions.

Immune checkpoint inhibitors are becoming of more use as clinicians are prescribing them for patients with different malignancies. As their use continues to increase, clinicians must be aware of the side effects, which are autoimmune in nature. Autoimmune diabetes has been described in the past while patients were being treated with programmed cell death protein 1 (PD-1) inhibitors, but it usually occurs after the patient's fourth or fifth cycle.

Statin therapy is considered one of the main therapies indicated to reduce the risk of cardiovascular death in patients with atherosclerotic cardiovascular disease. Maximal risk reduction is linked to the degree of reduction of the low-density lipoprotein cholesterol; therefore, high-intensity dosing is required with coronary artery disease. Musculoskeletal side effects are reported with the use of statin, especially at high doses.

Importance: Colony-stimulating factors are prescribed to patients undergoing chemotherapy to reduce the risk of febrile neutropenia. Research suggests that 55% to 95% of colony-stimulating factor prescribing is inconsistent with national guidelines.

Objective: To examine whether a guideline-based standing order for primary prophylactic colony-stimulating factors improves use and reduces the incidence of febrile neutropenia.

Anxiety sensitivity, or fear of anxious arousal, is cross-sectionally associated with a wide array of adverse posttraumatic neuropsychiatric sequelae, including symptoms of posttraumatic stress disorder, depression, anxiety, sleep disturbance, pain, and somatization. The current study utilizes a large-scale, multi-site, prospective study of trauma survivors presenting to emergency departments. Hypotheses tested whether elevated anxiety sensitivity in the immediate posttrauma period is associated with more severe and persistent trajectories of common adverse posttraumatic neuropsychiatric sequelae in the eight weeks posttrauma.

Background: Sex-disaggregated data for coronavirus disease 2019 (COVID-19) reported higher hospitalized fatality rates among men than women.

Objective: To determine whether the risk factors for in-hospital mortality from COVID-19, present at the time of hospital admission, differed by patient sex.

Design And Setting: Single-center, retrospective cohort study at a tertiary-care urban academic center.

Background: It has been theorized that 75%-80% of febrile neutropenia (FN) is caused by endogenous pathogens, while up to 20% of cases are thought to be caused by a viral infection. It is unknown if precautions such as masking and social distancing reduce the risk of FN in susceptible populations.

Aim: To determine whether coronavirus disease 2019 (COVID-19) infection mitigation efforts, namely masking and social distancing, were associated with a reduction in the incidence of FN.

PLoS One

September 2022

Department of Internal Medicine, Division of Cardiovascular Medicine, University of Michigan, Ann Arbor, MI, United States of America.

Background: The COVID-19 pandemic has severely impacted healthcare delivery and patient outcomes globally.

Aims: We aimed to evaluate the influence of the COVID-19 pandemic on the temporal trends and outcomes of patients undergoing percutaneous coronary intervention (PCI) in Michigan.

Methods: We compared all patients undergoing PCI in the BMC2 Registry between March and December 2020 ("pandemic cohort") with those undergoing PCI between March and December 2019 ("pre-pandemic cohort").

Purpose: The prospective Neoadjuvant Breast Registry Symphony Trial compared the 80-gene molecular subtyping signature with clinical assessment by immunohistochemistry and/or fluorescence in situ hybridization in predicting pathologic complete response (pCR) and 5-year outcomes in patients with early-stage breast cancer.

Methods: Standard-of-care neoadjuvant chemotherapy combined with trastuzumab or trastuzumab plus pertuzumab was given to patients with human epidermal growth factor receptor 2 (HER2)-positive tumors (n = 295). pCR was the primary end point, with secondary end points of distant metastasis-free survival and overall survival at 5 years.

Introduction: The purpose of this report is to increase awareness of ceftriaxone as an alternative therapy for the prevention of congenital syphilis (CS) when the mother is allergic to penicillin, especially when desensitization to penicillin cannot be performed or is unsafe.

Case: A 37-year-old pregnant woman who was syphilis positive reacted to penicillin with Stevens-Johnson syndrome (SJS); her rapid plasma reagin (RPR) was 1:64 at presentation to the infectious disease clinic. CS was prevented with two courses of ceftriaxone: 10 days 1 g IV daily at week 12 followed by 10 days of 250 mg IM daily at week 28 achieved a 4-fold fall in RPR titer to 1:16, indicating cure.

Introduction: Atherosclerotic cardiovascular disease (ASCVD) is the major cause of morbidity and mortality worldwide. Statins provide primary and secondary ASCVD prevention. Intolerance due to statin-associated myalgias reduces long-term adherence, thus muting potential benefits.

Undifferentiated osteoclast-like giant cell pancreatic carcinoma (UC-OGC) is a rare pancreatic carcinoma that is composed of osteoclast-like giant cells among other cells and is hardly described in literature due to its infrequent presentation. With that, a rare symptom of pancreatic cancers is upper gastrointestinal (GI) bleeding. We report a 76-year-old African American male who presented with one episode of bloody emesis associated with intermittent episodes of severe abdominal pain and a 25 lbs of unintentional weight loss secondary to metastatic UC-OGC.

Esophageal schwannoma is a rare tumor and is classified as one of the esophageal subepithelial lesions (SELs). Endoscopic ultrasound (EUS) evaluation is the gold standard for evaluating subepithelial lesions. Differentiation through EUS-guided fine needle aspiration is sometimes important to exclude lesions with malignant potential.

Atypical presentations of acute coronary syndrome (ACS) have been commonly known to occur but are often excluded in the differential when other diagnoses seem more likely. Female gender, patients with diabetes, hypertension, age greater than 55, and a history of smoking are some of the risk factors that have been associated with noncharacteristic presentations of ACS. This often leads to misdiagnosis and overall increased mortality.

Article Synopsis
  • Trauma memories can vividly affect survivors, particularly in the context of PTSD, impacting normal functioning.
  • Most neuroscience research on trauma focuses on threat-processing, neglecting the role of visual circuitry, which is crucial for understanding PTSD symptoms.
  • A study of 278 participants found that structural changes in the ventral visual stream post-trauma are linked to PTSD symptoms like nightmares and cognitive issues, suggesting that this visual network might worsen trauma memory reactivation.

  • - A case study describes a 61-year-old male with a history of benign prostatic hyperplasia who presented with fever, diarrhea, and swelling in the genital area, leading to diagnosis and treatment with antibiotics and surgical debridement.
  • - The patient, who was immunocompetent, showed unusual pathogens in his wound cultures, highlighting that FG can occur even in patients without common predisposing factors.
