Introduction Acute monoarthropathies that present in emergency settings include septic arthritis, where urgent joint arthrocentesis is the diagnostic gold standard. Literature indicates low confidence among trainee doctors in performing knee aspirations. Simulation-based teaching can be used to supplement procedural skills training and improve their confidence in performing such procedures.

Utility of virtual reality in medical training: Clinical management of a patient with knee monoarthritis.

Reumatol Clin (Engl Ed)

December 2024

Departamento de Anatomía Humana, Facultad de Medicina, Universidad de Salamanca, Salamanca, Spain.

The use of technological environments with virtual reality (VR) techniques for the practice of arthrocentesis offers an effective and safe way to learn and improve the necessary skills to carry out a medical procedure and patient care. This article presents an interactive simulator using immersive VR, in which the participant experiences practicing clinical management and decision-making in the face of a person presenting with monoarthritis in a medical consultation. The objective with this development is for the user to acquire the competence to perform, correctly and safely, a knee arthrocentesis and to differentiate the types of synovial fluids that can be found in a joint.

We examined the effect of preoperative antibiotic exposure and duration on synovial fluid samples from patients with native joint septic arthritis of the hip/knee. While exposure before diagnostic arthrocentesis did not affect fluid parameters, increased duration was associated with a decreased total nucleated cell count, underscoring the complex antibiotic effects on synovial fluid parameters.

Background: Synovial hemangiomas are rare benign vascular anomalies surrounded by a synovial lining and were first described by Bouchut in 1856. These neoplasms can develop in the intra-articular region, resulting in effusions and knee pain. However, their cause remains unknown.

  • The article emphasizes a multimodal approach to pain management in hemophilia, recommending intra-articular injections when other treatments fail and surgery isn't needed yet.
  • It provides a detailed overview of ultrasound-guided procedures for common joints affected by hemophilia, like the elbow, knee, and ankle, focusing on techniques like arthrocentesis and injections.
  • Experts believe that advancements in hematological treatments allow for effective incorporation of these ultrasound-guided techniques, leading to better pain relief outcomes for hemophilia patients.

  • * A case of a 52-year-old man with right knee pain was treated using point-of-care ultrasound (POCUS) to assist in a procedure that provided immediate relief without pain during the process.
  • * Although POCUS shows promise in enhancing procedure efficiency, challenges to its widespread use exist, but they are manageable.

Introduction: Temporomandibular joint (TMJ) osteoarthritis is a degenerative disease of the TMJ. It is characterized by progressive degradation of the extracellular matrix components of articular cartilage, with secondary inflammatory components leading to pain in the temporomandibular region and reduced mouth opening. Current treatments do not halt disease progression, hence the need for new therapies to reduce inflammation and, consequently, improve symptoms.

A 54-year-old man with a history of hypertension, atrial fibrillation, chronic kidney disease, nonischemic cardiomyopathy, osteoarthritis, and gout presented to the emergency department (ED) with dysuria, painful scrotal swelling, severe bilateral flank pain, back pain, atraumatic right arm (elbow and distally) pain and swelling, and bilateral knee pain. His physical exam was notable for fever, tachycardia, bilateral costovertebral angle (CVA) tenderness, exquisite pain, erythema, and swelling of bilateral knees and the right arm (elbow and distally). He met Systemic Inflammatory Response Syndrome (SIRS) criteria, was placed on Ceftriaxone for presumed septic pyelonephritis, and was admitted to the medicine team.

Objectives: This study used hydrogen nuclear magnetic resonance spectroscopy for the first time to examine differences in the metabolomic profile of stifle joint synovial fluid from dogs with cranial cruciate ligament rupture with and without meniscal injuries, in order to identify biomarkers of meniscal injury. Identifying a biomarker of meniscal injury could then ultimately be used to design a minimally invasive diagnostic test for meniscal injuries in dogs.

Materials And Methods: Stifle joint synovial fluid was collected from dogs undergoing stifle joint surgery or arthrocentesis for lameness investigations.

Septic arthritis of the knee is the most common form of septic arthritis in children and can lead to irreversible damage to the joint. is the primary pathogen associated with septic arthritis, although other causative pathogens may be isolate in children with specific risk factors. The diagnosis of knee septic arthritis is based on comprehensive evaluation, including the patient's medical history, physical examination, blood tests, and arthrocentesis.

Bilateral septic arthritis (SA) is a rare presentation that is often associated with sepsis syndrome in patients with underlying risk factors. We present the case of a 25-year-old female with laryngeal oedema due to para phenylene diamine-containing hair-dye poisoning. She was receiving treatment with intravenous (IV) methylprednisolone and IV antibiotics along with airway management.

How drainage of septic arthritis should be performed remains controversial. The aim of the present study was to compare arthrocentesis (Ac) using double intra-articular needle lavage to arthrotomy (At) as first-line drainage treatment for pediatric hip and knee septic arthritis. The secondary objective was to identify risk factors of second articular drainage.

Procalcitonin as a Predictor of Septic Knee Arthritis: A Retrospective Cohort Study.

J Am Acad Orthop Surg Glob Res Rev

January 2023

From the Department of Orthopaedic Surgery, (Dr. West, Fisher, Dr. Rounds, and Dr. Simpson); the Clinical Research Institute (Dr. Almekdash), Texas Tech University Health Science Center, Lubbock, TX; and the Department of Orthopaedics and Rehabilitation, University of Florida School of Medicine, Gainesville, FL (Dr. Murphree).

Introduction: Differentiating septic arthritis from aseptic arthritis (AA) of the knee is difficult without arthrocentesis. Although procalcitonin (PCT) has shown diagnostic value in identifying bacterial infections, it has not been established as a reliable marker for identifying septic arthritis (SA). Recent studies have shown promise in the use of PCT as a useful systemic marker for identifying septic arthritis versus AA.

Objective: Knee arthrocentesis can be performed by landmark (LM) or ultrasound (US) guidance. The goal of performing knee arthrocentesis is to obtain synovial fluid, however, it is also important to consider the number of attempts required and accidental bone contacts that occur. This study evaluates procedural success without bone contact in knee arthrocentesis and compares both LM and US guided techniques in a cadaver model.

  • Azacitidine (AZA) is a key treatment for patients with myelodysplastic syndromes (MDS) and acute myeloid leukaemia (AML) who cannot receive stem-cell transplants, but side effects like arthritis are not well-documented.
  • A 71-year-old patient with Chronic Lymphocytic Leukaemia developed therapy-associated AML and experienced a good hematological response to AZA but later presented symptoms indicating reactive arthritis after the ninth treatment cycle.
  • The patient was diagnosed with reactive arthritis after joint assessment, and symptoms were managed with conservative treatments; this case suggests a potential link between AZA and arthritis flare-ups in MDS patients, warranting further research.
