Successful Treatment of Wounds from Nonuremic Calciphylaxis with Acellular Piscine Dermis.

Plast Reconstr Surg Glob Open

July 2023

From the Department of Surgery, Division of Plastic, Reconstructive, Hand and Burn Surgery, University of New Mexico, Albuquerque, N.M.

In this report, we present a 57-year-old man with chronic bilateral lower extremity wounds from nonuremic calciphylaxis, which were successfully reconstructed using a piscine-derived acellular dermal matrix. The acellular dermal matrix incorporated quickly, providing a wound bed that was amenable to skin grafting. We demonstrate that this is an effective off-the-shelf solution for these chronic wounds, resulting in pain reduction and complete closure of the wounds, allowing the patient to return to his previous baseline activities, and improving his quality of life.

Added Value of FDG PET/MRI in Gynecologic Oncology: A Pictorial Review.

Radiographics

August 2023

From the Mallinckrodt Institute of Radiology, Washington University School of Medicine, 510 S Kingshighway Blvd, St. Louis, MO, 63110-8131 (Z.T., K.S.K., S.I., T.J.F., P.R.D., A.S.S., M.R.P., J.C.M., M.I.); Department of Radiology, Obstetrics and Gynecology, University of Wisconsin School of Medicine and Public Health, 600 Highland Ave, Madison, Wis (E.A.S.); Department of Diagnostic Radiology, The University of Texas MD Anderson Cancer Center, Houston, Tex (P.R.B.); and Department of Radiology, Mayo Clinic, Scottsdale, Ariz (M.Z.).

Fluorine 18-fluorodeoxyglucose (FDG) PET and MRI independently play a valuable role in the management of patients with gynecologic malignancies, particularly endometrial and cervical cancer. The PET/MRI hybrid imaging technique combines the metabolic information obtained from PET with the excellent soft-tissue resolution and anatomic details provided by MRI in a single examination. MRI is the modality of choice for assessment of local tumor extent in the pelvis, whereas PET is used to assess for local-regional spread and distant metastases.

  • The study explores whether balloon septal rupture can enhance the outcomes of aortic stent grafting for chronic aortic dissection, which is often hindered by retrograde false lumen perfusion.
  • Forty patients, averaging 56 years old, underwent thoracic endovascular aortic repair with septal rupture, leading to various types of aortic dissections, with some cases being emergencies.
  • Postoperative results showed a high survival rate of 97.5% at about 1.6 years, with many patients experiencing improvements in aortic size and successful false lumen thrombosis.*

  • - A study was conducted to test dexpramipexole, a new oral drug aimed at reducing eosinophils in patients with eosinophilic asthma, to see if it is safe and effective.
  • - In a trial involving 103 participants, those taking dexpramipexole (especially the 150 mg and 75 mg doses) showed significant reductions in eosinophil levels compared to the placebo group and maintained a good safety profile.
  • - The results indicate that dexpramipexole effectively lowers eosinophils, but more extensive studies are needed to evaluate its overall impact on asthma outcomes.

Quantitative imaging biomarkers of liver disease measured by using MRI and US are emerging as important clinical tools in the management of patients with chronic liver disease (CLD). Because of their high accuracy and noninvasive nature, in many cases, these techniques have replaced liver biopsy for the diagnosis, quantitative staging, and treatment monitoring of patients with CLD. The most commonly evaluated imaging biomarkers are surrogates for liver fibrosis, fat, and iron.

J Thorac Cardiovasc Surg

January 2024

Norton Thoracic Institute, St Joseph's Hospital and Medical Center, Phoenix, Ariz; Creighton University School of Medicine, Phoenix Regional Campus, Phoenix, Ariz.

Article Synopsis
  • A study investigated how the demanding work environment and burnout of cardiothoracic surgeons impact their spouses and significant others (SOs).
  • A survey sent to SOs revealed that a majority felt their surgeon partners' stress significantly influenced family life, with issues such as lack of time for family and intimacy being common.
  • The findings highlighted that family well-being is at risk, particularly when surgeons are early in their careers, work longer hours, or lack workplace support, pointing to the need for further research and potential solutions to address these challenges.

Multiplex immunofluorescence is a novel, high-content imaging technique that allows simultaneous in situ labeling of multiple tissue antigens. This technique is of growing relevance in the study of the tumor microenvironment, and the discovery of biomarkers of disease progression or response to immune-based therapies. Given the number of markers and the potential complexity of the spatial interactions involved, the analysis of these images requires the use of machine learning tools that rely for their training on the availability of large image datasets, extremely laborious to annotate.
