Acute generalized exanthematous pustulosis (AGEP) is a rare, pustular rash that occurs most commonly after exposure to a medication (typically antibiotics or diltiazem). This case describes a patient who developed a widespread pustular eruption shortly after beginning empiric antibiotics for community-acquired pneumonia. Diagnosis of AGEP was difficult in this scenario due to the patient's pulmonary infection and atypical skin biopsy results.

Electronic address:

Background: Short courses of adjunctive systemic corticosteroids are commonly used to treat acute urticaria and chronic urticaria flares (both with and without mast cell-mediated angioedema), but their benefits and harms are unclear.

Objective: To evaluate the efficacy and safety of treating acute urticaria or chronic urticaria flares with versus without systemic corticosteroids.

Methods: We searched the MEDLINE, EMBASE, CENTRAL, CNKI, VIP, Wanfang, and CBM databases from inception to July 8, 2023, for randomized controlled trials of treating urticaria with versus without systemic corticosteroids.

Article Synopsis
  • - Pseudoarthrosis, a serious complication of spinal fusion surgery, has a high occurrence rate (35%-40%) and current revision options often fail and can cause additional issues like junctional kyphosis.
  • - An innovative technique using pedicled vascularized bone grafts (VBGs) helps improve spinal fusion by enhancing structural integrity and blood supply, and can be applied at various spinal levels with minimal extra time and no need for additional incisions.
  • - A case study involving a 68-year-old woman showed that bilateral rib and iliac crest VBGs effectively augmented her complex spinal surgery, leading to significant functional improvements and satisfaction without complications after three months of recovery.

Endometriosis is a prevalent and potentially debilitating condition that mostly affects individuals of reproductive age, and often has a substantial diagnostic delay. US is usually the first-line imaging modality used when patients report chronic pelvic pain or have issues of infertility, both common symptoms of endometriosis. Other than the visualization of an endometrioma, sonologists frequently do not appreciate endometriosis on routine transvaginal US images.

Abdominal-based free flaps are the mainstay of autologous breast reconstruction; however, the region may not be ideal for patients with inadequate soft tissue or history of abdominal surgery. This case describes the use of a novel conjoined flap based on the profunda artery perforator and upper gracilis pedicles, named the perforator and upper gracilis (PUG) flap. This flap design aims to maximize medial thigh flap volume while ensuring robust tissue perforation.

Electronic address:

Objective: Donation after circulatory death (DCD) donors offer the ability to expand the lung donor pool and ex vivo lung perfusion (EVLP) further contributes to this ability by allowing for additional evaluation and resuscitation of these extended criteria donors. We sought to determine the outcomes of recipients receiving organs from DCD EVLP donors in a multicenter setting.

Methods: This was an unplanned post hoc analysis of a multicenter, prospective, nonrandomized trial that took place during 2011 to 2017 with 3 years of follow-up.

Modern crops exhibit diverse sensitivities to ammonium as the primary nitrogen source, influenced by environmental factors such as external pH and nutrient availability. Despite its significance, there is currently no systematic classification of plant species based on their ammonium sensitivity. We conducted a meta-analysis of 50 plant species and present a new classification method based on the comparison of fresh biomass obtained under ammonium and nitrate nutrition.

Article Synopsis
  • Gastrointestinal (GI) bleeding is the leading cause of GI-related hospitalizations in the U.S., making timely diagnosis and treatment essential for better patient outcomes and reduced healthcare costs.
  • Various radiologic techniques, such as CT angiography and MR enterography, are used to assess GI bleeding and work alongside GI endoscopy, although existing management guidelines can create confusion about their use.
  • An expert panel has reviewed these radiologic methods, discussing their performance, pros and cons compared to endoscopy, and providing consensus recommendations for their technical application in GI bleeding cases.

Cone-Beam CT of the Extremities in Clinical Practice.

Radiographics

March 2024

From Mayo Clinic Alix School of Medicine, 5777 East Mayo Blvd, Scottsdale, AZ 85054 (M.K.D., E.V.); and Departments of Radiology (J.R.L., A.W., J.A.F.) and Orthopedic Surgery (K.P.), Mayo Clinic Arizona, Phoenix, Ariz.

Cone-beam CT (CBCT) is a promising tool with increasing applications in musculoskeletal imaging due to its ability to provide thin-section CT images of the appendicular skeleton and introduce weight bearing, which accounts for loading forces that typically interact with and affect this anatomy. CBCT devices include an x-ray source directly opposite a digital silicon detector panel that performs a single rotation around an object of interest, obtaining thin-section images. Currently, the majority of research has been focused on the utility of CBCT with foot and ankle pathologic abnormalities, due to the complex architectural arrangement of the tarsal bones and weight-bearing nature of the lower extremities.

Members of the Fleischner Society have compiled a glossary of terms for thoracic imaging that replaces previous glossaries published in 1984, 1996, and 2008, respectively. The impetus to update the previous version arose from multiple considerations. These include an awareness that new terms and concepts have emerged, others have become obsolete, and the usage of some terms has either changed or become inconsistent to a degree that warranted a new definition.

Imaging of Perirenal and Intrarenal Lymphatic Vessels: Anatomy-based Approach.

Radiographics

March 2024

From the Department of Radiology (H.T., A.I., T.A.P., N.T.) and Department of Medicine, Division of Rheumatology (Y.S.), Mayo Clinic, 200 First St SW, Rochester, MN 55905; Department of Radiology, Okayama City Hospital, Okayama, Japan (T.T.); Department of Radiology, Jichi Medical University, Tochigi, Japan (S.M.); Department of Diagnostic and Interventional Radiology, University of Tsukuba, Ibaraki, Japan (M.M.); and Department of Radiology, Mayo Clinic, Scottsdale, Ariz (A.K.).

Article Synopsis
  • The lymphatic system includes lymphoid organs and vessels, but findings related to intrarenal and perirenal lymphatic vessels are not well-documented in radiology.
  • Lymphatic pathways in the renal cortex allow lymph to travel towards the hilum and capsular plexus, which can be visualized through contrast medium during CT urography, indicating possible complications like pyelolymphatic backflow.
  • Conditions like congenital renal lymphangiectasia and acquired renal complications can affect imaging post-kidney transplant, with distinct fluid patterns in the urinary tract indicating issues like chyluria, highlighting the importance of understanding lymphatic pathways for effective cancer management.
