[Life-threatening puerperal sepsis caused by group A streptococci].

A case of postpartum streptococcal toxic shock syndrome due to group A streptococci is described. The patient suffered multiorgan failure, was critically ill and stayed in the intensive care unit for 18 days before recovering. The patient received massive antibiotic treatment as well as intravenous immunoglobulin therapy.

[Continuous venovenous hemodiafiltration in critically ill patients with acute renal failure].

The development of computerized machines with a simple user-friendly interface to perform continuous venovenous hemodiafiltration (CVVHDF) has resulted in a break-through of CVVHDF in the treatment of acute renal failure (ARF) in Danish intensive care units. During CVVHDF the blood is submitted to a combination of dialysis and ultrafiltration. In contrast to intermittent haemodialysis (HD), CVVHDF can be used in critically ill patients with unstable circulation.

Repaglinide acutely amplifies pulsatile insulin secretion by augmentation of burst mass with no effect on burst frequency.

Objective: Repaglinide is a new oral hypoglycemic agent that acts as a prandial glucose regulator proposed for the treatment of type 2 diabetes by stimulating insulin secretion. The aim of this study was to explore actions of repaglinide on the rapid pulsatile insulin release by high-frequency insulin sampling and analysis of insulin-concentration time series.

[Microbiological examination of patients hospitalized in Aarhus municipal hospital].

The purpose of this retrospective study was to evaluate 1) the nature, extent and relevance of the microbiological examination performed before institution of antibiotic treatment, 2) the relevance of the instituted treatment in the light of the microbiological findings or clinical diagnosis of infection. In patients with Menière's disease attacks of vertigo can be invalidating. We have tested the effect of instillation of gentamycin in the middle ear to minimize or stop the attacks of vertigo. The results of a prospective investigation of 14 patients with Menière's disease treated with gentamycin in the period from 1993 to 1996 are presented.

[Semen quality and fertility in a population-based follow-up study].

Little is known about the importance of semen quality in male infertility. We followed 430 first pregnancy planners with no previous reproductive experience recruited among more than 50,000 cohabiting trade-union members. The couples were consecutively enrolled into the study as they discontinued contraception and were followed up for six month menstrual cycles or until a pregnancy was verified.

[Pesticides and time to pregnancy among Danish farmers].

The aim of this study was to examine time to pregnancy among farmers who used pesticides (traditional farmers) and farmers who did not (organic farmers). A total of 904 men, selected from the Danish Ministry of Agriculture lists of traditional and organic farmers, participated in telephone interviews. The participation rate was 84%.

High-frequency oscillations in circulating amylin concentrations in healthy humans.

[The effect of pedicle screw instrumentation on posterolateral spinal fusion. A prospective, randomized study with a two-year follow-up].

The aim was to evaluate the effect of supplementary pedicle screw fixation (Cotrel-Dubousset [CD]) in posterolateral lumbar spinal fusion. The study comprises 130 patients undergoing lumbar or lumbosacral fusion for spondyloisthesis grades I-II or degenerative segmental instability conditions. The patients were randomly allocated for no instrumentation (n = 66) or CD instrumentation (n = 64) in posterolateral lumbar fusion.

[Results after surgical treatment of unstable thoracolumbar fractures].

Seventy-eight patients with unstable, one-level fracture of the thoracolumbar spine and no neurological impairment were treated with short segment fixation, transpedicular autologous bone transplantation and posterolateral fusion. Kyphotic deformity and anterior column height improved significantly. Complications consisted of one case of late deep infection, three cases of seroma, four cases with 5 mm schantz screw breakage and two cases with screw loosening.

[Metaphyseal chondrodysplasia].

Metaphyseal chondrodysplasia (MCD) is a heterogeneous group of diseases characterised by defective enchondral ossification, leading to metaphyseal changes. The different types of MCD can be distinguished by clinical findings, radiology and genetic tests. Based on a case story with MCD, pancreatic insufficiency and granulocytopenia (Shwachman's syndrome), we review the most common types of MCD with regard to clinical manifestations, radiological findings, and genetic background.

Transcapillary escape rate and albuminuria in Type II diabetes. Effects of short-term treatment with low-molecular weight heparin.

The relation between urinary albumin excretion rate (UAE), transcapillary escape rate of albumin (TERalb), haemostatic factors, ambulatory blood pressure, and metabolic variables was investigated in 45 Type II (non-insulin-dependent) diabetic patients without overt nephropathy or uncontrolled blood pressure. We enrolled 44 patients in a placebo controlled study to test the effects of 3 week long treatment with low-molecular weight heparin (tinzaparin) on the same variables. BMI, 24 h systolic and diastolic blood pressure, plasma concentrations of triglycerides, fasting glucose, factor VIII, von Willebrand factor (vWf), fibrinogen, alpha-2 macroglobulin, and fibronectin were notably higher in patients with increased albuminuria compared with normoalbuminuric patients, whereas the TERalb was similar in the two groups.

[Antibiotic prophylaxis in adult critically ill patients in intensive care units].
