Extended approach in head and neck tumors: modified Schobinger incision.

J Craniofac Surg

September 2007

Plastic Surgical Department, Argerich Hospital, Buenos Aires, Argentina.

Surgical treatment of many tumors of the head and neck require extended approaches. Many cervicotomies have been described in the treatment of head and neck and cervical tumors; careful thought must be made by a surgeon when surgical treatment is planned for an extended tumor. Our purpose is to show the versatility of the modified Schobinger incision.

Treatment of acute pancreatic pseudocysts after severe acute pancreatitis.

J Gastrointest Surg

March 2007

Department of Surgery, Cosme Argerich Hospital, Ayacucho 1485, Ciudad de Buenos Aires, Argentina.

Treatment of acute pancreatic pseudocysts (APP) after an episode of severe acute pancreatitis (SAP) remains controversial. Both population heterogeneity and limited numbers of patients in most series prevent a proper analysis of therapeutic results. The study design is a case series of a large, tertiary referral hospital in the surgical treatment of patients with APP after SAP.

Treatment of linear and spider telangiectasia with an intense pulsed light source.

J Cosmet Dermatol

December 2004

Chouela EN, Unit of Dermatology, Dr Cosme Argerich Hospital, Buenos Aires, Argentina.

Background: The flashlamp-pumped pulsed dye laser (585 and 577 nm) has proved to be an effective and safe treatment option in the therapy of linear and spider facial telangiectasia. Nevertheless, the postoperative purpura, which most patients see as cosmetically disfiguring, has always been a matter of concern.

Aims: To test the effectiveness and safety of an intense pulsed light source (IPLS), which emits non-coherent light adjustable within the 515-1200 nm range, in the treatment of linear and spider facial telangiectasia.

The present study analyzed the recurrence rate of idiopathic atrial fibrillation (AF) after elimination by radiofrequency ablation of the substrate for a regular tachycardia. Forty consecutive patients with idiopathic AF and a history of regular palpitations or documented regular tachyarrhythmias were prospectively included. Regular tachyarrhythmias were induced in 82.

Endoscopic excision of frontozygomatic dermoid cysts.

J Craniofac Surg

July 2004

Department of Plastic Surgery, Argerich Hospital, Buenos Aires, Argentina.

A dermoid cyst is a tumor frequently located in the external third of the fronto-orbital region. It develops along the line of embryonic fusion of the naso-optic groove, infolding the ectoderm during embryonic development. The classic treatment is complete surgical resection by means of an external approach on the dome of the cyst; consequently, a visible scar is a permanent esthetic sequel.

Despite the availability of efficient tear substitutes, many patients with dry eye syndrome experience severe corneal injuries and a subsequent loss of vision. Surgical techniques using mayor salivary glands to provide a substitute for tears have been reported; with this technique the drainage of saliva goes into the conjunctival fornix, permitting corneal and conjunctival humidification. The authors describe a new surgical approach in which minor salivary glands are autotransplanted into the conjunctival fornix by means of a graft of the intraoral mucosa-transporting salivary glands.

Epidemiological surveillance of human hydatidosis by means of ultrasonography: its contribution to the evaluation of control programs.

Acta Trop

June 2001

Division of Internal Medicine-Hepatology, Argerich Hospital, Government of Buenos Aires City, Associated to the University of Buenos Aires, Alte Brown 240, 1171, Buenos Aires, Argentina.

Epidemiological surveillance for hydatidosis is based on initial determination and follow-up of the infestation rate in man, sheep and dogs. The use of ultrasonography (US) as a screening method has demonstrated its usefulness in detecting asymptomatic human carriers of abdominal hydatidosis. To evaluate the contribution of US to epidemiological surveillance within the framework of disease control programs, we performed 719 US studies in school children from 7 to 13 years of age, in rural areas of Río Negro, Argentina, subjected to canine deparasitization during 1984/1986 and 1997/1998.

The purpose of this investigation was to evaluate the degree of efficacy of eliminating crow's feet by means of direct injection of botulinum toxin A into orbicularis oculi muscles under direct surgical vision during either blepharoplasty or face lift operations. Eighteen patients were injected with Botox A-14 in each orbicularis oculi muscle. Dilution was obtained by adding 4 ml of preservative-free saline to 100 IU of Botox A.

Surgical treatment of the senile upper lip.

Plast Reconstr Surg

September 2000

Argerich Hospital, Buenos Aires, Argentina.

Aesthetic changes in the aging upper lip constitute a troublesome problem for modern women. During the process of aging, the following alterations appear in the upper lip: (1) vertical wrinkles, (2) reduction in height of the vermilion border along with lengthening of the skin area of the lip, and (3) "disappearance" of the Cupid's bow. In 1993, Guerrissi and Sanchez described a surgical technique that allowed them to correct the effects of these senile changes in 19 patients.

Background: Internal drainage of giant pancreatic pseudocysts secondary to acute pancreatitis is frequently complicated with postoperative retroperitoneal infection and hemorrhage. Recent data suggest that the risk factor is unrecognized pancreatic necrosis; presumably, pancreatic necrosis becomes infected with bacteria introduced by the cystoenteric anastomosis.

Hypothesis: Video-assisted pancreatic necrosectomy, performed at the time of internal drainage, may prevent postoperative retroperitoneal complications in patients with giant acute pseudocysts.

Reconstruction of large defects in the scalp with fasciocutaneous flaps.

Scand J Plast Reconstr Surg Hand Surg

June 1999

Service of Plastic Surgery, Argerich Hospital, Buenos Aires, Argentina.

Although recent reports have emphasised free microsurgical transfer for reconstruction of extensive defects in the scalp, in our experience a carefully planned scalp flap is a simpler and safer method than a free transfer. Twenty-one patients with defects as large as 10%-60% of the scalp surface area were reconstructed; the calvarium was resected in five cases and the dura mater in two. In 18 cases the flaps were based on a single pedicle: the superficial temporal artery.

A patient with Frey syndrome on the left cheek area as a complication of an Obwegeser osteotomy is reported. Flushing, sweating of skin, and hypoesthesia of buccal mucosae were present 6 months after surgery. An injury to the auriculotemporal nerve during desperiostization of the posterior border of the mandibular ramus is believed to be the principal cause.

Facial nerve paralysis after intratemporal and extratemporal blunt trauma.

J Craniofac Surg

September 1997

Service of Plastic Surgery, Cosme Argerich Hospital, Buenos Aires, Argentina.

Definitive facial nerve paralysis by intratemporal and extratemporal blunt injury is uncommon. Facial palsy as a result of closed temporal trauma is assumed to result if a petrous or temporal fracture is produced. Injury of the extratemporal trunk is extremely rare, but peripheral branches can be more easily injured.

The purpose of this clinical investigation is to confirm the efficacy of eliminating facial wrinkles by injecting botulinum toxin A into mimetic muscles. Fifty-four patients were injected with BOTOX A-14 in the corrugator superciliaris, 19 in the frontalis muscles, and 13 in the orbicularis oculis. Dilution was obtained by adding 4 ml preservative-free saline to 100 IU of BOTOX A.
