Background: The effective use of combination antiretroviral therapy (ART) has significantly improved the life expectancy of people living with the human immunodeficiency virus (HIV). However, complications have shifted from opportunistic infections to issues such as drug toxicity and resistance, as well as an increase in premature cardiovascular diseases (CVD). These conditions are attributed to chronic immune activation and persistent inflammation caused by HIV, along with lipid abnormalities and insulin resistance.

Background And Aims: The Endoscopic Resection Group of the Spanish Society of Endoscopy (GSEED-RE) model and the Australian Colonic Endoscopic Resection (ACER) model were proposed to predict delayed bleeding (DB) after EMR of large superficial colorectal lesions, but neither has been validated. We validated and updated these models.

Methods: A multicenter cohort study was performed in patients with nonpedunculated lesions ≥20 mm removed by EMR.

Article Synopsis
  • The study focused on how the length of hospitalisation (LOH) affects the outcomes of patients with acute heart failure (AHF) after they are discharged, comparing results across different hospital departments.
  • Out of 8,563 patients analyzed, 90-day outcomes showed that longer hospital stays led to higher post-discharge mortality rates, particularly for those staying over 15 days, but readmission rates remained constant regardless of LOH.
  • The research concluded that shorter hospital stays do not lead to worse outcomes, and higher mortality risks associated with longer stays were consistent across various hospital departments.

Article Synopsis
  • The study investigates how the risk of malnutrition affects 30-day mortality rates in elderly patients experiencing acute heart failure (AHF) in emergency departments.
  • It analyzed data from 749 patients aged 65 and older, finding that 79.3% were at risk of malnutrition and the overall 30-day mortality rate was 8.8%.
  • The results indicate that malnutrition is a significant independent factor for higher 30-day mortality, suggesting that routine screening for malnutrition could improve patient care and management in emergency settings.

Objective: To assess the effects of non-invasive ventilation (NIV) in emergency department (ED) patients with acute heart failure (AHF) on short term outcomes.

Methods: Patients from the EAHFE Registry (a multicenter, observational, multipurpose, cohort-designed database including consecutive AHF patients in 41 Spanish EDs) were grouped based on NIV treatment (NIV+ and NIV-groups). Using propensity score (PS) methodology, we identified two subgroups of patients matched by 38 covariates and compared regarding 30-day survival (primary outcome).

Introduction: The asthma-chronic obstructive pulmonary disease (COPD) overlap (ACO) is a clinical condition that combines features of those two diseases, and that is difficult to define due to the lack of understanding of the underlying mechanisms. Determining systemic mediators may help clarify the nature of inflammation in patients with ACO.

Objectives: We aimed at investigating the role and interaction of common markers of systemic inflammation (IL-6, IL-8, and tumor necrosis factor-α), Th2-related markers (periostin, IL-5, and IL-13), and IL-17 in asthma, COPD, and ACO.

Lipid-lowering therapy is one of the cornerstones of cardiovascular prevention and is one of the most effective strategies in the secondary prevention of ischemic heart disease. Nevertheless, the current treatment of lipid disorders, together with lifestyle changes, fails to achieve the targets recommended in clinical guidelines in a substantial proportion of patients. PCSK9 inhibitors have demonstrated safety and efficacy in the treatment of dyslipidemia.

MicroRNAs (miR) have considerable potential as therapeutic tools in cardiac diseases. Alterations in atrial miR are involved in the development of atrial fibrillation (AF), but the molecular mechanism underlying their contribution to atrial remodeling in chronic atrial fibrillation (CAF) is only partially understood. Here we used miR array to analyze the miR profile of atrial biopsies from sinus rhythm (SR) and CAF patients.

Francisco Romero, the first heart surgeon.

Ann Thorac Surg

September 1997

Cardiac Surgery Service, Hospital de la Santa Creu y Sant Pau, Barcelona, Spain.

Francisco Romero, a Catalonian physician, became the first heart surgeon when he performed an open pericardiostomy to treat a pericardial effusion in 1801. He presented his work at the Society of the School of Medicine in Paris in 1815, but the procedure was considered too aggressive and his work was silenced for many years. Proper credit should be given to Romero as the first man who directly approached the heart by incising the pericardium.
