Objectives: Lateral sagittal infraclavicular approach is frequently used because it has less risk of complications and provides rapid and adequate regional anesthesia. Due to the fact that the brachial plexus is deeper in the infraclavicular region and the approach angle is sharper, it can be technically challenging. In this study, we aimed to compare the costoclavicular approach, which is a newly defined approach, with the lateral sagittal infraclavicular brachial plexus block.

Background: Metoclopramide, a dopamine antagonist employed for its antiemetic effects, can precipitate neuropsychiatric adverse effects, including extrapyramidal symptoms and, in a few instances, acute psychosis. Although there have been reports of metoclopramide-induced psychosis in elderly individuals, there is no documentation of such incidents in children as far as we are aware.

Case Presentation: This case report describes an 11-year-old girl with a history of mild intellectual disability and attention deficit hyperactivity disorder, managed with 10 mg of methylphenidate daily.

Hajdu-Cheney syndrome (HCS), caused by a heterozygous gain of function variant of the NOTCH2 gene, is a rare skeletal dysplasia. Although the main presentation is acro-osteolysis, osteoporosis, and facial dysmorphism, having a wide range of clinical manifestations creates diagnostic difficulties. Here, a 15-year-old male patient with HCS who had no complaints until this age except for two short bone fractures and one vertebral collapse fracture due to a fall was reported.

Extreme hyperleukocytosis (Leukocyte count >200 × 10/L) in an adolescent young adult (AYA) patient with B-ALL could result in mild symptoms of leukostasis. Hyperleukocytosis requires prompt initiation of therapy with adequate hydration, cytoreduction and prevention of tumor lysis. Ph + B-ALL may present with extreme hyperleukocytosis and may be resistant to initial pre-phase therapy.

Article Synopsis
  • Mitochondrial DNA depletion syndromes (MDDSs) are a group of complex genetic disorders being studied for their occurrence in young children with liver problems.
  • The research involved 40 children under 5 years old, mostly showing symptoms like jaundice, liver enlargement, and elevated lactate levels, with a significant diagnosis rate using genetic tests.
  • The findings suggest that common demographic and lab tests aren't enough for diagnosing these inherited conditions, as only 22.5% were diagnosed with MDDS while around 42.5% were diagnosed with more comprehensive genetic analysis.

  • The study assessed the effectiveness of hybrid aortic surgery in 24 patients with complex aortic conditions, focusing on technical success and postoperative outcomes like morbidity and mortality.
  • Different surgical strategies were utilized based on the location of the aortic issue, with notable findings that the femoral incision approach resulted in shorter hospital stays despite higher infection rates in some areas.
  • Overall, the results suggest that hybrid aortic surgery is both a safe and effective method for treating complex cases where standard procedures may not be adequate.

Outcomes by Retrospective Eligibility for Maintenance Therapy of Patients With Advanced Urothelial Carcinoma: Post Hoc Analysis of the Phase 3 KEYNOTE-361 Trial.

Clin Genitourin Cancer

October 2024

Barts Cancer Centre, St Bartholomew's Hospital, London United Kingdom; Barts Cancer Institute, Barts Health NHS Trust, Queen Mary University of London, London, United Kingdom.

Introduction: The phase 3 KEYNOTE-361 trial of first-line pembrolizumab with or without chemotherapy versus chemotherapy alone in patients with locally advanced or metastatic urothelial carcinoma (la/mUC) completed enrollment before the approval of postchemotherapy maintenance avelumab for patients without progressive disease. This post hoc analysis evaluated the outcomes of patients who received chemotherapy alone in KEYNOTE-361 by retrospective eligibility for subsequent maintenance therapy.

Patients And Methods: Patients in the chemotherapy alone arm were retrospectively categorized as maintenance eligible (received ≥4 cycles of chemotherapy and did not die or experience disease progression within 10 weeks of chemotherapy completion), maintenance ineligible (received <4 cycles of chemotherapy or had progressive disease or died within 0-10 weeks after completion of ≥4 cycles of chemotherapy), and indeterminate eligibility for maintenance therapy (if neither maintenance eligible or ineligible).

  • The study investigates the link between eminential morphometry (the shape of a part of the knee) and non-contact anterior cruciate ligament (ACL) ruptures, aiming to clarify the relationship.
  • Researchers analyzed knee MRIs from 400 patients, dividing them into two groups: those with ACL ruptures and those without, measuring various morphometric parameters.
  • The results revealed significant differences in eminential height and width between the groups, with male patients and lower eminential width identified as risk factors for non-contact ACL injuries.

The Relationship Between DNA Mismatch Repair Status and Clinicopathologic Characteristics in Colon Cancer.

Turk J Gastroenterol

August 2024

Department of Pathology, Ankara Yıldırım Beyazıt University Faculty of Medicine, Ankara Bilkent City Hospital, Ankara, Türkiye.

DNA mismatch repair (MMR) proteins are essential for repairing genetic mutations that occur during DNA replication. Deficiency of MMR proteins results in a phenotype called microsatellite instability (MSI), which occurs in Lynch syndrome as well as sporadic colorectal cancers (CRC), and it is associated with several clinicopathological features. We aimed to investigate the association of the loss of MMR proteins with clinicopathologic considerations in our CRC series.
