Background: Multi-drug resistant organisms, especially Vancomycin-Resistant Enterococcus (VRE) and Carbapenam Resistant Klebsiella pneumoniae (KPC), are serious health threat. Early detection of resistant bacteria colonization among patients in intensive care units (ICUs) not only enables effective treatment but more importantly prevents disease and limits transmission. Therefore, we aimed to to assess the frequency of VRE and KPC colonization via rectal swab sampling.

Objective: In this study, we aimed to determine whether admission hemoglobin versus post-percutaneous coronary intervention (PCI) hemoglobin level at 24 hours is a predictor of in-hospital mortality for patients with ST elevation myocardial infarction (STEMI) without evidence of clinical hemorrhage who underwent primary PCI.

Methods: In this study, we included 1,444 consecutive patients with STEMI who underwent primary PCI at a tertiary heart hospital. The primary outcome of the study was the in-hospital all-cause mortality.

Reconstruction of the lower extremity, especially the proximal lower leg, is known to be a challenge for reconstructive surgeons. When there is extensive vascular damage, the use of local flaps and microsurgical methods will be limited, so there are few reconstructive options available. We want to define the use of medial sural artery-based cross-leg flap for the reconstruction of the proximal lower leg.

Article Synopsis
  • The study evaluated the predictive value of the C-reactive protein/albumin ratio (CAR) for acute kidney injury (AKI) in non-ST-segment elevation myocardial infarction (NSTEMI) patients with chronic kidney disease (CKD).
  • A total of 420 NSTEMI patients were analyzed, and it was found that a higher CAR ratio (especially in the T3 group) significantly correlated with an increased incidence of AKI, with 34% of T3 patients developing AKI.
  • The study highlighted that CAR is a promising indicator for predicting AKI in this patient population, with CAR levels above 0.20 showing 74% sensitivity and 45% specificity for AKI development.

Is Septal Cartilage Graft via Transmastoid Approach Safe Enough in the Repair of Large Tegmen Defects?

ORL J Otorhinolaryngol Relat Spec

May 2021

Department of Otorhinolaryngology Head and Neck Surgery, Faculty of Medicine, Ankara Yildirim Beyazit University, Ankara, Turkey.

Introduction: Tegmen defect (TD) has a potential of intracranial spread of middle ear infection, meningoencephalic herniation (MEH), and cerebrospinal fluid leakage (CSFL). Especially the defects >1 cm with MEH or CSFL are generally repaired via the classical middle fossa or minicraniotomy technique. The aim of this study was to show the efficiency of the intracranial, extradural placement of the septal cartilage graft in the closure of the TD larger than 1 cm via the transmastoid (TM) approach.
