Background: Metastatic basal cell carcinoma (mBCC) is a rare condition with no effective second-line treatment options. Cemiplimab is an immune checkpoint inhibitor that blocks the binding of programmed cell death-1 (PD-1) to its ligands, programmed death-ligand 1 (PD-L1) and programmed death-ligand 2 (PD-L2). Here, we present the final analysis of cemiplimab in patients with mBCC after first-line hedgehog pathway inhibitor (HHI) treatment (NCT03132636).

Introduction: Treatment of hidradenitis suppurativa (HS) is challenging, and in Hurley stage I, it is based on topical measures. Our aim was to compare the efficacy and safety of topical resorcinol 10% with topical clindamycin 1% and no therapy, in Hurley stage I HS.

Methods: In this open, prospective, randomized trial, we studied 60 Hurley stage I patients with IHS4 ≤10.

Introduction: The basophil activation test (BAT) is a flow cytometry laboratory technique that assesses the level of activation indicators expressed on the surface of basophils. We conducted a real-life study in a prospective cohort of patients with reported drug hypersensitivity reactions to determine the true relevance of BAT as a diagnostic tool for assessing immediate hypersensitivity reactions to medicines.

Methods: We prospectively assessed individuals with clinical suspicion of immediate hypersensitivity reactions to drugs over a 2-year period.

Service de Dermatologie, Centre Hospitalier Lyon Sud, Hospices Civils de Lyon, Lyon, France; Université Claude-Bernard-Lyon Lyon, Lyon, France and Lyons cancer research center UMR INSERM U1052 - CNRS5286 - UCBL1 Lyon France.

Melanonychia striata longitudinalis might involve one or more fingers and/or toes and might result from several different causes, including benign and malignant tumors, trauma, infections, and activation of melanocytes that might be reactive or related to the pigmentary trait, drugs and some rare syndromes. This broad differential diagnosis renders the clinical assessment of melanonychia striata particularly challenging. Nail matrix melanoma is relatively rare, occurs almost always in adults involves more frequently the first toe or thumb.

Dermatol Pract Concept

October 2023

Skin Cancer and Melanoma Unit, 1st Department of Dermatology-Venereology, National and Kapodistrian University of Athens, Andreas Sygros Hospital, Athens, Greece.

Article Synopsis
  • Advanced basal cell carcinoma (BCC) patients, particularly those not eligible for surgery or radiotherapy, can use hedgehog pathway inhibitors (HHI) like vismodegib and sonidegib as first-line treatments, though high discontinuation rates exist (88-92%).
  • Factors influencing treatment discontinuation include how well the tumor responds, side effects, and the patient's choice, with many stopping after 8 to 12 months.
  • The review discusses the possibility of switching to immunotherapy (e.g., cemiplimab) after HHI treatment, covering its effectiveness and safety as a secondary option or in combination with other therapies.

Australas J Dermatol

February 2024

Faculty of Medicine, 1st Department of Dermatology-Venereology, National and Kapodistrian University of Athens, Andreas Sygros Hospital, Athens, Greece.

Background/objectives: To date, scientific data on the efficacy of botulinum toxin type A (BoNT-A) for primary plantar hyperhidrosis (PPH) are mainly derived from case reports and small case series. Herein, we sought to assess the efficacy and safety of BoNT-A for PPH on a large series of patients.

Methods: Medical records of patients who were referred to the outpatient department for hyperhidrosis of a tertiary care hospital and received BoNT-A for PPH from March 2003 until December 2022 were reviewed.

Article Synopsis
  • Changes in pigmented melanocytic lesions have been observed after laser or IPL hair removal treatments, with studies showing significant clinical and dermoscopic alterations.
  • In clinical evaluations, 20-100% of individuals experienced changes, while dermoscopic alterations were reported in 48%-93% of nevi, with common findings including bleaching and irregular pigmented areas.
  • Diagnostic approaches for these atypical lesions may involve complex techniques like confocal microscopy and digital dermoscopy, highlighting the need for caution during procedures to prevent potential melanoma risks.

Eur J Cancer

November 2023

Division of Cancer Sciences, University of Manchester, Manchester, UK; Department of Medical Oncology, Christie NHS Foundation Trust, Manchester, UK.

Article Synopsis
  • A multidisciplinary panel of experts from various European medical organizations has developed updated recommendations for treating invasive cutaneous squamous cell carcinoma (cSCC) based on a thorough literature review and expert consensus.
  • Treatment guidelines cover different stages of cSCC, recommending surgery with clear margins as the primary option for low and high-risk cases, while highlighting the lack of benefit from radiation for high-risk patients with clear margins.
  • For advanced cSCC that cannot be surgically treated, anti-PD-1 agents like cemiplimab are recommended as first-line therapy, with second-line options including cetuximab combined with chemotherapy or radiation.

Article Synopsis
  • The study investigates the transition from actinic keratosis (AK) to cutaneous squamous cell carcinoma (cSCC), highlighting unclear mechanisms driving disease progression despite observed genetic mutations.
  • RNA sequencing of 110 patient samples shows a progression from differentiated skin cells to a progenitor-like state, involving the suppression of genes responsible for skin cell differentiation and alterations in immune response.
  • The research identifies a specific combination of tumor suppressor gene inactivation and Ras signaling activation that contributes to the development and progression of cSCC, providing insights for potential therapeutic interventions.
