Article Synopsis
  • - Narcolepsy type 1 (NT1) is characterized by low hypocretin levels and changes in stress hormone activity, particularly a loss of corticotrophin-releasing hormone (CRH) linked to the body's stress response system.
  • - A study involving NT1 patients and healthy controls utilized the Trier Social Stress Test to measure physiological and psychological responses to stress, including changes in ACTH, cortisol, heart rate, and subjective stress levels.
  • - Results indicated participants with NT1 felt more stressed, but their hormone and heart rate responses were similar to controls, although men with NT1 had notably lower cortisol levels after stress than their control counterparts, suggesting further exploration is necessary for understanding CRH's role and gender effects

The socio-relational focus of youth peer support workers (YPSWs) poses a challenge when YPSWs are embedded in medical oriented contexts common to child and adolescent mental health services (CAMHS); as it requires YPSWs to find a balance between being a peer on one hand, and adhering to professional boundaries and medical standards set out by CAMHS on the other. To create a suitable position for YPSWs in CAMHS, this study investigated the unique socio-relational contributions YPSWs can make to CAMHS in addition to clinicians, and identified how these contributions can be embedded within CAMHS. This study reports on 37 semi-structured interviews conducted in the Netherlands with youth (n = 10), YPSWs (n = 10), and clinicians (n = 17).

How far should I manage acute optic neuritis as an ophthalmologist? A United Kingdom perspective.

Eye (Lond)

August 2024

The Medical Eye Unit, Guy's and St Thomas' NHS Foundation Trust, London, UK.

Optic neuritis (ON) is an inflammation of or around the optic nerve, frequently caused by infectious or immune-mediated inflammatory disorders. In the UK, its strongest association is with Multiple Sclerosis (MS), though the combined prevalence of other associated infectious and immune-mediated inflammatory diseases (I-IMID) is similar to that of MS-ON. Prompt identification and understanding of ON's underlying cause informs tailored management and prognosis.

Background: Ethiopia made a national licensing examination (NLE) for associate clinician anesthetists a requirement for entry into the practice workforce. However, there is limited empirical evidence on whether the NLE scores of associate clinicians predict the quality of health care they provide in low-income countries. This study aimed to assess the association between anesthetists' NLE scores and three selected quality of patient care indicators.

Reduced myeloid commitment and increased uptake by macrophages of stem cell-derived HPS2 neutrophils.

Life Sci Alliance

April 2024

Department of Molecular Hematology, Sanquin Research, Amsterdam University Medical Center (AUMC), University of Amsterdam, Amsterdam, Netherlands

Article Synopsis
  • Hermansky-Pudlak syndrome type 2 (HPS2) is a rare genetic disorder caused by mutations that lead to protein mis-sorting within cells, resulting in severe neutropenia.
  • Using iPSCs from an HPS2 patient, researchers found that while granulocyte development was impaired, the neutrophils produced displayed characteristics similar to healthy neutrophils.
  • The study revealed that despite normal viability, neutrophil yield was reduced due to macrophages phagocytosing them, indicating that both developmental delays and macrophage activity contribute to the limited neutrophil output in HPS2.

Diagnosis of childhood febrile illness using a multi-class blood RNA molecular signature.

Med

September 2023

Section of Paediatric Infectious Disease and Centre for Paediatrics & Child Health, Department of Infectious Disease, Imperial College London, London, UK. Electronic address:

Background: Appropriate treatment and management of children presenting with fever depend on accurate and timely diagnosis, but current diagnostic tests lack sensitivity and specificity and are frequently too slow to inform initial treatment. As an alternative to pathogen detection, host gene expression signatures in blood have shown promise in discriminating several infectious and inflammatory diseases in a dichotomous manner. However, differential diagnosis requires simultaneous consideration of multiple diseases.

Motile and non-motile cilia play critical roles in mammalian development and health. These organelles are composed of a 1000 or more unique proteins, but their assembly depends entirely on proteins synthesized in the cell body and transported into the cilium by intraflagellar transport (IFT). In mammals, malfunction of non-motile cilia due to IFT dysfunction results in complex developmental phenotypes that affect most organs.

Efficient complement-mediated clearance of immunosuppressed T cells by macrophages.

Front Immunol

June 2023

Department of Molecular Hematology, Sanquin Research, Amsterdam University Medical Center (AUMC), University of Amsterdam, Amsterdam, Netherlands.

Cancer is one of the leading causes of death worldwide. Treatment outcome is largely dictated by the tumor type, disease stage, and treatment success rates, but also by the variation among patients in endogenous anti-tumor responses. Studies indicate that the presence of neutrophils in the tumor microenvironment is associated with a worse patient outcome due to their ability to suppress local anti-tumor T cell activity.
