Background: Cleft lip and palate (CLP) are among the most common congenital anomaly that affects up to 33,000 newborns in India every year. Nasoalveolar moulding (NAM) is a non-surgical treatment performed between 0 and 6 months of age to reduce the cleft and improve nasal aesthetics prior to lip surgery. The NAM treatment has been a controversial treatment option with 51% of the cleft teams in Europe, 37% of teams in the USA and 25 of cleft teams in India adopting this methodology.

Article Synopsis
  • - Alkaptonuria is a rare genetic disorder that affects the metabolism of homogentisic acid, leading to a condition called ochronotic arthropathy, which impacts major joints like the spine and knees, often going undiagnosed until adulthood.
  • - A case study details a 43-year-old man who was diagnosed with this condition during surgery and successfully underwent joint replacements in multiple areas over 10 years, maintaining full mobility without implant issues after 11 years.
  • - The findings suggest that total joint arthroplasty can yield positive long-term outcomes for patients with ochronotic arthropathy, but surgeons must be vigilant about inherent surgical difficulties and potential complications during treatment.

Background: Currently, two classification systems, namely Stephens and Sanders, based on axial CT images, and Zwipp and Rammelt, which consider deformities, are used for calcaneus malunions. Existing classifications have limitations due to their pure anatomical basis, and the complexity of the problem, involving both bone and soft tissues. As a solution, the senior author proposed a novel ADIENS classification system for calcaneal malunion, based on pain generators.

Instability of the distal tibiofibular syndesmosis.

J Orthop Surg (Hong Kong)

July 2023

Department of Foot & Ankle Surgery, Amandeep Hospital, Amritsar, India.

The distal tibiofibular syndesmosis (DTFS) is more frequently injured than previously thought. Early diagnosis and appropriate treatment is essential to avoid long term complications like chronic instability, early osteoarthritis and residual pain. Management of these injuries require a complete understanding of the anatomy of DTFS, and the role played by the ligaments stabilizing the DTFS and ankle.

Background: Medial aspect of foot is unexplored due to its complex anatomy. Masterknot of Henry is an important landmark in this region, which plays a key role during tendon transfer procedures especially in those involving the flexor hallucis longus and flexor digitorum longus. We aim to determine the exact anatomical location of masterknot of Henry with respect to the bony prominences of the medial aspect of the foot and compare these measurements to the length of the foot.

Introduction: Volar locking plate fixation of distal radius fractures is commonly performed because of its good clinical outcomes. The flexor carpi radialis (FCR) approach is one of the most popular approaches to dissecting the volar side of the distal radius because of its simplicity and safety. We describe an extremely rare case of an absent FCR identified during a volar approach for fixation of a distal radius fracture.

Article Synopsis
  • Surgical management of Extraarticular Distal-third diaphyseal Humerus Fracture (EADHF) requires careful consideration of surgical techniques and implant options, with studies suggesting modified use of anatomic locking plates as an effective fixation method.
  • The case report outlines a successful application of a Distal Medial Tibial locking Plate (DMTP) in a 45-year-old female patient, leading to bony union and excellent recovery over two years.
  • The 3.5 mm DMTP enables rigid fixation and contributes to improved healing, decreased complications, and quicker recovery, making it a recommended alternative for treating EADHF.

Alveolar bone grafting and gingivoperiosteoplasty in bilateral cleft lip and palate.

Curr Opin Otolaryngol Head Neck Surg

August 2021

Department of Plastic, Cosmetic, Reconstructive and Microvascular Surgery, Amandeep Hospital, Amritsar, India.

Purpose Of Review: Gingivoperiosteoplasty (GPP) and alveolar bone grafting (ABG) procedures have a vast history. There have been many publications regarding ABG and secondary ABG, with and without presurgical infant orthopedics (PSIO), in bilateral cleft lip and palate patients. There is little long-term data available describing results of both techniques.

Choice of the implant during revision total hip arthroplasty (THA) is crucial in younger patients due to the impaired quality of bone and deficient bone stock. The short femoral stem provides an appealing unorthodox alternative implant in young patients undergoing primary total hip arthroplasty. A classic revision total hip arthroplasty predominantly describes the use of conventional or long stem for revision of the femoral component.

Introduction: Periprosthetic patellar fracture after total knee arthroplasty (TKA) is an uncommon yet devastating complication, and management of these fractures is challenging with unreliable results. An isolated bilateral patellar fracture is often associated with systemic diseases, steroid usage, and repeated microtrauma. Isolated simultaneous bilateral periprosthetic patellar fracture after TKA is seldom reported in the literature.

The awareness of anterior cruciate ligament (ACL) injuries and their treatment is increasing among athletes and within the general population, so patients report early to orthopaedic surgeons. Because we encounter a thick ACL stump during arthroscopic ACL reconstruction, an attempt is made to preserve this stump. Remnant preservation-although promising in terms of graft healing because it enhances cell proliferation, revascularization, and regeneration of the proprioceptive organs in the reconstructed ACL-does not come without complications, such as cyclops lesions, impingement, and extension loss.

Indian J Plast Surg

January 2019

Department of Plastic and Reconstructive Surgery, Amandeep Hospital and Clinics, Amritsar, Punjab, India.

Management of composite defects of leg following trauma requires a planned ortho-plastic approach right from the outset. Timely, planned intervention results in reduced amputation rates and improved limb salvage and function. Right from the time of presentation of the patient to the emergency with such injury, the process of decision making in terms of salvage or amputation, local flap cover/free flap cover, bone reconstruction first or soft tissue or both combined, come into play.
