Article Synopsis
  • - The study analyzed the OXYGEN trial, which compared high levels of oxygen (80% FiO2) vs. lower levels (30% FiO2) for preventing infections in patients with specific types of fractures and aimed to see if using an "as-treated" approach changed the results compared to the "intention-to-treat" method.
  • - Conducted at 29 trauma centers, 1,231 patients with tibial plateau, tibial pilon, or calcaneus fractures were randomly assigned to either oxygen treatment; adherence was evaluated using two specific criteria based on the percentage of surgery time at different oxygen levels.
  • - Results showed no significant differences in primary and deep infection rates, but the treatment group had fewer

Background: Microscopic colitis is a relatively new diagnosis that was first described in the 1980s. Patients usually present with chronic watery and non-bloody diarrhea and are typically characterized by an unremarkable gross appearance of the colon on lower endoscopy while having evidence of lymphocytic infiltration of the lamina propria and the epithelium on histology. Two subtypes have been described in the literature: Collagenous colitis, with marked thickening of the subepithelial layer, and Lymphocytic colitis.

The objective of this analysis was to compare clinician-based and formally calculated risk assessments by REVEAL Lite 2 and COMPERA 2.0 and to characterize parenteral prostacyclin utilization within 90 days of baseline in high-risk patients. A multisite, double-blind, retrospective chart review of patients with pulmonary arterial hypertension (PAH) was conducted with an index period of January 2014-March 2017.

Article Synopsis
  • Incidental durotomy (ID) is a common issue in spine surgeries that can lead to longer hospital stays and complications like pseudomeningocele formation, prompting a study comparing traditional open vs. minimally invasive (MIS) microdiscectomy techniques.
  • The study included 192 MIS patients and 2902 open surgery patients, ultimately analyzing 156 matched patients from both groups for ID occurrences and complications.
  • Results showed a similar incidence rate of ID in both methods (3.1% total), with one re-operation in the open cohort for pseudomeningocele, but no significant difference in re-operation rates between the two groups, highlighting the need for more extensive future research.

Article Synopsis
  • Historically, women with aortic stenosis have been underdiagnosed and faced worse outcomes compared to men, leading to the need for better treatment recognition and participation in clinical trials.
  • The SMART trial aimed to compare the clinical and hemodynamic outcomes of women with small aortic annuli receiving either self-expanding valves (SEVs) or balloon-expandable valves (BEVs) during transcatheter aortic valve replacement.
  • In a study of 621 women, no significant differences were observed in the main clinical outcomes between the SEV and BEV groups after 12 months, but SEVs showed a lower rate of bioprosthetic valve dysfunction.

  • Proximal femoral replacement (PFR) is a surgical technique used after tumor removal or failed hip replacements, but the risk of prosthetic instability is higher when there’s significant bone or soft tissue loss.
  • This study analyzed the outcomes of PFR among 106 patients over 15 years, categorizing them by the type of implant used: bipolar, constrained, or dual mobility.
  • Results showed that dual mobility implants had a longer time to dislocation and that smoking history and BMI differed among the groups, but all types had similar rates of early dislocation overall.

P-15 is a 15-amino-acid-long biomimetic peptide widely demonstrated to enhance osteogenesis in vivo. Despite the prevalence of polyether-ether-ketone (PEEK) in interbody device manufacturing, a growing body of evidence suggests it may produce an unfavorable immune response. The purpose of this preliminary study was to characterize the immune response and new bone growth surrounding PEEK implants with and without a P-15 peptide-based osteobiologic.

Dual-frame transcatheter mitral valve replacement has emerged over the last decade as an alternative to mitral valve surgery in patients with severe mitral regurgitation, especially in the presence of prohibitive operative risk. This paper presents a case of massive superinfected thrombosis of the Tendyne valve tether along with its risk factors, diagnosis, and management.

Article Synopsis
  • The study investigates the link between hospital volumes for aortic valve replacement (AVR) surgeries—either transcatheter (TAVR) or surgical (SAVR)—and patient outcomes, specifically in-hospital mortality and 30-day readmission rates.
  • It classified hospitals based on their procedural volumes and analyzed data from over 72,000 patients across 400 U.S. hospitals, finding that higher hospital volumes are associated with lower in-hospital mortality but do not affect 30-day readmissions.
  • The conclusions suggest that patients with aortic stenosis have better outcomes when treated in high-volume hospitals that perform both TAVR and SAVR procedures frequently.

This study aims to systematically review case reports and case series in order to compare the postoperative course of conservative, endovascular and surgical treatments for traumatic dural arteriovenous fistulas predominantly supplied by the middle meningeal artery (MMAVFs), which usually occur following head trauma or iatrogenic causes. We conducted a comprehensive search of PubMed, Embase, Scopus, Web of Science, and Google Scholar until June 23rd, 2024. Three cohorts were defined based on the treatment modality employed.

Background: Black and Latino youth are disproportionately affected by trauma from community violence, but to date, few data support the benefit of evidence-based treatments for these youth or of including peer support to engage these youth in mental health services.

Objective: From 2018 until 2020, a hospital and community-based violence intervention program in Philadelphia pilot tested the integration of home and community-based Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) with peer services and case management for youth impacted by community violence. This study describes the implementation of this intervention.

The menopause transition and post-menopause period marks a time of dynamic physiological and hormonal change. Cisgender women commonly experience vasomotor symptoms, genitourinary symptoms, and changes in bone health. The transgender population, including those assigned female at birth (AFAB) and those assigned male at birth (AMAB), has been understudied in terms of experiences through the menopause transition and midlife.

Purpose: Fractures and dislocations of the pelvic ring are complex injuries that when treating require meticulous attention to detail and often specialized technical skill. These injuries can be the result of high-energy trauma, particularly in younger patients, or low energy trauma more often found in the elderly. Regardless of mechanism, these injuries lie on a spectrum of severity and can be treated conservatively or surgically.
