Department of Pediatrics (RG Hernandez, NN Showell, and SB Johnson), Johns Hopkins University School of Medicine, Baltimore, Md; Department of Mental Health (X Qu, H Volk, and SB Johnson), Johns Hopkins Bloomberg School of Public Health, Baltimore, Md.

Article Synopsis
  • The study examined the impact of the COVID-19 pandemic on mothers with young children, focusing on factors like anxiety, stress, and financial issues before and during the pandemic.
  • Mothers reported significant increases in anxiety (9.4%), perceived stress (13.3%), and financial stress (41.7%) during the pandemic, with family routines being the most affected area (72.4%).
  • Interestingly, prepandemic psychosocial stress did not predict pandemic impact, indicating that new stresses arose during the pandemic that compounded existing inequalities rather than simply amplifying prior distress.

We report the case of a 14-year-old patient with a known history of Crohn's disease who was incidentally diagnosed with an asymptomatic cecal lipoma. A routine surveillance colonoscopy as part of the management of the patient's Crohn's Disease revealed a well-defined, submucosal, yellowish mass in the patient's cecum. Histopathological examination of a biopsy specimen revealed submucosal adipose tissue, consistent with the endoscopic images showing the characteristic appearance of the lipoma.

Prevalence rates of autism spectrum disorder (ASD) across the globe and lifespan suggest that law enforcement officers (LEOs) are likely to interact with autistic people in their professional duties. In developed countries, LEOs have professional encounters with autistic individuals, and research strongly supports LEOs need training to increase their knowledge about autism and confidence interacting with autistic people. The purpose of the present study is to compare three training approaches with LEOs: (a) a training presentation alone (P), (b) a training presentation combined with virtual reality simulation (P + VR), and (c) a training presentation combined with in-vivo simulation (P + SIM).

Article Synopsis
  • - The study analyzed injury patterns and outcomes for golf cart (GC) and all-terrain vehicle (ATV) accidents among children under 16, revealing a rise in injuries for both types of vehicles from January 2016 to June 2021.
  • - It found that GC patients tend to be younger and have a higher rate of intracranial injuries compared to ATV patients, but overall injury severity, ICU admission rates, and surgical needs were similar for both.
  • - The results suggest that despite being seen as safer, golf carts pose a significant injury risk similar to ATVs, indicating a need for improved safety measures for their use.

Importance: Observational studies often report that anemia and red blood cell (RBC) transfusions are associated with a higher risk of necrotizing enterocolitis (NEC) among extremely low-birthweight (ELBW) infants.

Objective: To evaluate whether there is a temporal association between 72-hour hazard periods of exposure to RBC transfusions and NEC among ELBW infants randomized to either higher or lower hemoglobin transfusion thresholds.

Design, Setting, And Participants: This post hoc secondary analysis of 1690 ELBW infants who survived to postnatal day 10 enrolled in the Transfusion of Prematures (TOP) randomized multicenter trial between December 1, 2012, and April 12, 2017, was performed between June 2021 and July 2023.

Article Synopsis
  • Post-transfusion survival of donor red blood cells (RBCs) is crucial for effective treatment in conditions like sickle cell disease (SCD) and can be measured using biotin labeling.
  • An observational study tested the survival of biotin-labeled RBCs in pediatric SCD patients receiving chronic transfusions, revealing one donor unit with microcytosis and alpha-thalassemia.
  • The findings indicated that RBCs from donors with alpha-thalassemia may have a shorter survival post-transfusion, suggesting a need for more research on how these traits affect overall transfusion effectiveness.

The interplay of psychosis and non-compliance with fatal outcome in an adult with MSUD.

Am J Med Genet A

September 2024

Division of Medical Genetics, Department of Pediatrics, Duke University Medical Center, Durham, North Carolina, USA.

Significant progress has been achieved in enhancing early outcomes for individuals with maple syrup urine disease (MSUD), a rare metabolic disorder that leads to the accumulation of branched-chain amino acids leucine, isoleucine, and valine, where leucine is known as the primary neurotoxic metabolite. Newborn screening is helpful in early diagnosis and implementation of dietary treatment, thus reducing neurological deterioration and complications in young children. However, patients face the life-long challenge of maintaining metabolic control through adherence to a strict low-leucine diet to avoid long-term consequences of chronic hyperleucinemia, which include cognitive deficits, mood disorders, and movement disorders.

Background: Up to 10% of children are reported to be allergic to penicillin, but many allergy labels are unverified and may require formal testing. Inaccurate drug allergy labels are associated with a range of adverse clinical outcomes. Patients with hematological disorders may experience frequent and severe infections; those who have been incorrectly labeled penicillin allergic may benefit from allergy de-labeling (ADL) efforts to facilitate access to beta-lactam antibiotics.
