Article Synopsis
  • The study highlights that five-year survivors of diffuse large B-cell lymphoma (DLBCL) are at a higher risk of developing subsequent malignant neoplasms (SMNs), particularly among younger patients and those treated with higher doses of certain chemotherapy drugs.
  • After a median follow-up of 13.8 years, 321 out of 2373 survivors developed SMNs, with significant increases noted for lung and gastrointestinal cancers.
  • The research indicates that treatment including rituximab may lower the risk of certain cancers, underscoring the importance of long-term monitoring of DLBCL survivors and the need for further studies to assess risks associated with newer treatments.

Objective: Endoscopic mucosal resection (EMR) is the preferred treatment for non-invasive large (≥20 mm) non-pedunculated colorectal polyps (LNPCPs) but is associated with an early recurrence rate of up to 30%. We evaluated whether standardised EMR training could reduce recurrence rates in Dutch community hospitals.

Design: In this multicentre cluster randomised trial, 59 endoscopists from 30 hospitals were randomly assigned to the intervention group (e-learning and 2-day training including hands-on session) or control group.

Background: Questions asked by patients with primary progressive multiple sclerosis (PPMS) during patient-initiated MS nurse consultations may contain salient information that can help health care providers understand their needs, which, in turn, can help tailor counseling and treatment.

Methods: Records of all patients with PPMS visiting the MS center of a large teaching hospital in the Netherlands between January 2007 and January 2021 were studied retrospectively. Number and type (scheduled or patient initiated) of MS nurse consultations, reasons for consultations (in prespecified categories), and frequency of subsequent referrals were registered.

Article Synopsis
  • Since 2017, pembrolizumab and avelumab have become standard treatments for metastatic bladder cancer in Europe, with the study focusing on their usage in the Netherlands.
  • Out of 1,525 patients diagnosed with metastatic bladder cancer between 2018 and 2021, only about 34.7% received systemic treatment, primarily after first-line chemotherapy.
  • Despite the introduction of maintenance avelumab in 2021, the use of immune checkpoint inhibitors has not significantly increased, indicating a need for further research on treatment patterns.

Objectives: Psychologic screening is often included as a mandatory component of evaluation of the impact of psychopathology disorders on the predicted outcome of spinal cord stimulation (SCS) for patients with chronic pain due to persistent spinal pain syndrome type 2 (PSPS type 2). The conclusion of such screenings can influence the decision to offer SCS therapy to a patient. However, evidence on the impact of psychopathology on SCS outcomes is still scarce.

Background: Guidelines are inconclusive on whether contrast-enhanced MRI using gadoxetic acid and diffusion-weighted imaging should be added routinely to CT in the investigation of patients with colorectal liver metastases who are scheduled for curative liver resection or thermal ablation, or both. Although contrast-enhanced MRI is reportedly superior than contrast-enhanced CT in the detection and characterisation of colorectal liver metastases, its effect on clinical patient management is unknown. We aimed to assess the clinical effect of an additional liver contrast-enhanced MRI on local treatment plan in patients with colorectal liver metastases amenable to local treatment, based on contrast-enhanced CT.

Article Synopsis
  • High-risk multiple myeloma (MM) patients, especially those who relapse early after therapy, benefit significantly from the combination of daratumumab with either bortezomib/dexamethasone or lenalidomide/dexamethasone.
  • In the CASTOR and POLLUX studies, daratumumab significantly improved progression-free survival (PFS) and overall survival (OS) for relapsed/refractory MM patients across both early and late relapse groups.
  • Despite not fully overcoming the risks associated with early relapse, the findings support using daratumumab for patients with one prior line of therapy, enhancing complete response rates and minimal residual disease negativity.

Article Synopsis
  • The study evaluates the limitations of using an Injury Severity Score (ISS) > 15 to define major trauma (MT) and its impact on trauma care networks.
  • A multicenter analysis of the Dutch National Trauma Registry data from 2015 to 2019 involved patients aged 16 and older who were severely injured and examined their in-hospital mortality and clinical outcomes based on their admission to level I vs. non-level I trauma centers.
  • The results show no significant difference in in-hospital mortality between patients treated at level I and non-level I facilities, although those at non-level I hospitals had shorter stays and a higher likelihood of being discharged home, suggesting that all trauma care levels yield similar outcomes for severely injured patients.
