Introduction: In teeth with irreversible pulpitis, successful local anesthesia is hard to achieve irrespective of the amount of local anesthesia and technique used. Such cases can be managed by concoction of pre-medications like anxiolytics, analgesics and effective local anesthesia. Background: Our objectives are to analyse and to compare the stress distribution and displacement of the craniofacial structures, following the application of forces from quad-helix and Nickel Titanium Palatal Expander-2 (NPE2) using finite element analysis.

Methods: Three-dimensional finite element models of young dried human skull, quad-helix appliance and NPE2 were constructed, and the initial activation of the expanders was stimulated to carry out the analysis and to evaluate the Von Misses stresses and displacement.

Results: Both the models demonstrated the highest stresses at the mid-palatal suture, with maximum posterior dislocation.

A 24-year-old male patient was referred to our department for expansion of the constricted maxillary arch as a presurgical procedure for the correction of congenital facial disfigurement. On examination, the patient had a convex profile, increased interlabial gap, tongue thrust, limited mouth opening, posterior crossbite, asymmetric 'V'-shaped maxillary arch with severe constriction, crowding of anterior teeth in the maxillary arch and a massive open bite. Radiographic investigations included orthopantomograph and occlusal radiographs.

Transmigration of mandibular canines.

The purpose of this paper is to present the first case of unusual reverse oblique (110 degrees to midsagittal plane) migration of mandibular right canine crossing the jaw midline and piercing the lower border of the mandible at the level of the opposite canine and also to report two more cases of transmigrated mandibular canine and one case of transmigrating mandibular canine. Mandibular canines are "cornerstone" of dental arch; their importance is manifested by their efficiency in masticatory function, stability of dental arch, and aid in maintaining natural facial expression. Early detection of this anomaly can help preserving these canines by orthodontic intervention or by surgical transplantation.

Multiple glass pieces in paranasal sinuses.

Here, a case has been reported of a road traffic accident with multiple glass pieces arranged in an unusual pattern in the left maxillary sinus, ethmoid sinus, nasopharynx and medial side of the orbit, as seen in the radiographs. The need to cover the exposed periostium has meant that a number of materials including mucosal and skin grafts are regarded as suitable for grafting in oral and maxillofacial surgery. To circumvent the disadvantages of other materials such as skin, biological membranes have been suggested as options, including fetal membrane.
