Best Pract Res Clin Gastroenterol

March 2024

Department of Gastroenterology & Hepatology, Department of Medicine, Al-Jahra Hospital, Al Jahra, Kuwait.

Endoscopic resection techniques have evolved over time, allowing effective and safe resection of the majority of pre-malignant and early cancerous lesions in the gastrointestinal tract. Bleeding is one of the most commonly encountered complications during endoscopic resection, which can interfere with the procedure and result in serious adverse events. Intraprocedural bleeding is relatively common during endoscopic resection and, in most cases, is a mild and self-limiting event.

Article Synopsis
  • The study investigates the risk of tuberculosis (TB) infection in patients undergoing biological therapies, specifically Adalimumab, Etanercept, and Tocilizumab, in a TB-endemic region of Saudi Arabia.
  • Over a 2-year follow-up period, the researchers analyzed data from 410 patients on Adalimumab, 271 on Etanercept, and 58 on Tocilizumab, primarily for conditions like rheumatoid arthritis and inflammatory bowel disease.
  • The results showed no reported cases of active TB infection during treatment, with only a small percentage of patients developing latent TB, suggesting that these biological therapies do not significantly increase the risk of TB infection.

Endocrinol Diabetes Metab Case Rep

April 2024

Department of Endocrinology and Diabetes, Sheikh Khalifa Medical City, Abu Dhabi, United Arab Emirates.

Summary: X-linked hypophosphatemic rickets (XLH), the most prevalent form of inherited hypophosphatemic rickets, is caused by loss-of-function mutations in the gene encoding phosphate-regulating endopeptidase homolog, X-linked (PHEX). This case series presents 14 cases of XLH from Gulf Cooperation Council (GCC) countries. The patients' medical history, biochemical and radiological investigative findings, as well as treatment responses and side effects from both conventional and burosumab therapy, are described.

Background: Mirroring the experience with colonic resections, cold snare-based techniques have been recently proposed for non-ampullary duodenal lesions to reduce the risk of adverse events (AEs). As the duodenal wall is thinner and more vascularized than in the colon, electrocautery-related AEs are relevant issues in this setting.

Aims: We performed a systematic review with pooled-analysis to evaluate the efficacy and safety of this technique.

Background: Biotin-thiamine-responsive basal ganglia disease (BTBGD) is a rare autosomal recessive neurometabolic disorder that is caused by biallelic pathogenic SLC19A3 variants and is characterized by subacute encephalopathy associated with confusion, convulsions, dysphagia, dysarthria, or other neurological manifestations.

Methods: A retrospective review of the data registry in Kuwait Medical Genetics Center for all cases diagnosed clinically and radiographically and confirmed genetically with BTBGD.

Background: Autosomal dominant polycystic kidney disease (ADPKD) is the most common renal monogenic disease, characterized by bilateral accumulation of renal fluid-filled cysts leading to progressive renal volume enlargement and gradual impairment of kidney function, often resulting in end-stage renal disease. Kuwait could provide valuable genetic insights about ADPKD, including intrafamilial phenotypic variation, given its large household size. This study aims to provide a comprehensive description of the pathogenic variants linked to ADPKD in the Kuwaiti population using multiple genetic analysis modalities and to describe and analyse the ADPKD phenotypic spectrum in terms of kidney function, kidney volume and renal survival.

Article Synopsis
  • Urethral catheterization (UC) is commonly performed in emergency settings to address issues like urinary retention and bladder obstruction, as shown in an audit of 81 male patients aged 18 and older.
  • The audit highlighted a major gap: although sterile gloves and pyodine were used, proper hand hygiene was neglected 80% of the time, despite CDC and Lippincott guidelines emphasizing its importance.
  • To enhance patient safety and reduce infections, there is a need for regular audits and training for healthcare staff on hand hygiene and appropriate catheterization practices.
