Article Synopsis
  • - The study aims to determine how the presence of cribriform pattern 4 carcinoma/intraductal carcinoma of the prostate (CC/IDCP) affects the likelihood of having persistent prostate-specific antigen (PSA) levels after robot-assisted radical prostatectomy (RARP) in men with localized prostate cancer.
  • - Researchers analyzed data from 730 patients who underwent RARP between 2015 and 2021, excluding those with metastasis or previous cancer treatments, to assess the correlation between CC/IDCP and persistent PSA levels, defined as PSA ≥0.2 ng/mL one month after surgery.
  • - The findings showed 6.3% of patients had persistent PSA levels, with CC/IDCP identified

The relationship between sex differences and long-term outcomes after fractional flow reserve (FFR)- and instantaneous wave-free ratio (iFR)-guided deferral of revascularization has yet to be elucidated. From the J-CONFIRM registry (long-term outcomes of Japanese patients with deferral of coronary intervention based on FFR in a multicenter registry), this study included 432 lesions from 385 patients (men, 323 lesions in 286 patients; women, 109 lesions in 99 patients) with paired data of FFR and iFR. The primary endpoint was the cumulative 5-year incidence of target vessel failure (TVF), including cardiac death, target vessel-related myocardial infarction, and clinically driven target vessel revascularization.

Background: Temporal trends in the management of acute coronary syndrome complicated with cardiogenic shock after the revision of guideline recommendations for intra-aortic balloon pump (IABP) use and the approval of the Impella require further investigation, because their impact remains uncertain.

Methods And Results: Using the Japanese Percutaneous Coronary Intervention (J-PCI) registry database from 2019 to 2021, we identified 12 171 patients undergoing percutaneous coronary intervention for acute coronary syndrome complicated with cardiogenic shock under mechanical circulatory support. The patients were stratified into 3 groups: (1) IABP alone, (2) Impella, and (3) venoarterial extracorporeal membrane oxygenation (VA-ECMO); the VA-ECMO group was further stratified into (3a) VA-ECMO alone, (3b) VA-ECMO in combination with IABP, and (3c) VA-ECMO in combination with Impella.

Article Synopsis
  • The study analyzed outcomes of 151 acute myeloid leukemia (AML) patients with the rare genetic alteration der(1;7)(q10;p10) after undergoing allogeneic hematopoietic stem cell transplantation (allo-SCT).
  • Results showed that patients with der(1;7)(q10;p10) had significantly better transplant outcomes compared to 853 patients with monosomy 7 or chromosome 7q deletion (-7/del(7q)).
  • Factors like additional chromosomal abnormalities and poor performance status were found to impact survival in the der(1;7)(q10;p10) group, indicating that allo-SCT is a viable treatment option for these patients.

Background Coronary artery disease (CAD) patterns play an essential role in the decision-making process about revascularization. The pullback pressure gradient (PPG) quantifies CAD patterns as either focal or diffuse based on fractional flow reserve (FFR) pullbacks. The objective of this study was to evaluate the impact of CAD patterns on acute percutaneous coronary intervention (PCI) results considered surrogates of clinical outcomes.

Polyglycolic acid (PGA) felt has been used to prevent clinically relevant postoperative pancreatic fistula (CR-POPF) after pancreaticoduodenectomy (PD). However, there has been no related research on Blumgart anastomosis. Therefore, this study aimed to investigate the practical significance of Blumgart anastomosis with our new method involving PGA felt to prevent CR-POPF following PD.

Physiological assessments using fractional flow reserve (FFR) and resting full-cycle ratio (RFR) have been recommended for revascularization decision making. Previous studies have shown a 20% rate of discordance between FFR and RFR. In this context, the correlation between RFR and FFR in patients with renal dysfunction remains unclear.

Aim: Drain fluid amylase concentration (DFAC) has been reported as a predictor of clinically relevant postoperative pancreatic fistula (CR-POPF) after pancreatectomy. However, the clinical significance of measuring the total drain fluid amylase amount (DFAA) considering the daily drainage volume of CR-POPF remains unclear.

Methods: Data from 216 consecutive patients who underwent pancreaticoduodenectomy (PD) (n = 126) or distal pancreatectomy (DP) (n = 90) between August 2014 and November 2020 were reviewed.

In cases of decompensated heart failure related to cancer therapy-related cardiac dysfunction, ivabradine administration could lead to an increased stroke volume by reducing the sinus heart rate, resulting in favorable hemodynamics. Assessment of the overlap between the E- and A-waves facilitates understanding the effects of ivabradine in such cases.
