Article Synopsis
  • The study evaluated two treatment strategies for high-risk prostate cancer: robot-assisted radical prostatectomy (RARP) with extended pelvic lymph-node dissection (ePLND) and RARP with neoadjuvant chemohormonal therapy (NCHT) without ePLND.
  • A total of 452 patients were analyzed, revealing that the ePLND group had a significantly higher postoperative complication rate compared to the NCHT group.
  • Additionally, the NCHT group showed better biochemical recurrence-free and castration-resistant prostate cancer-free survival rates, suggesting that NCHT may be a safer option; however, further research is needed on the long-term outcomes.

Background: Colorectal liver metastases (CRLM) occur in roughly half of patients with colorectal cancer. Minimally invasive surgery (MIS) has become an increasingly acceptable and utilized technique for resection in these patients, but there is a lack of specific guidelines on the use of MIS hepatectomy in this setting. A multidisciplinary expert panel was convened to develop evidence-based recommendations regarding the decision between MIS and open techniques for the resection of CRLM.

Article Synopsis
  • Minimal invasive liver resections (MLRs) are safer alternatives to traditional open surgery, but the effects of neoadjuvant chemotherapy (NAT) on their difficulty are not well understood.
  • This study analyzed data from 1,034 patients across 57 centers who underwent laparoscopic and robotic MLRs for colorectal liver metastases (CRLM), comparing those who received NAT to those who did not using statistical matching methods.
  • Results showed no significant differences in perioperative outcomes after matching, although the NAT group experienced slightly higher blood loss but shorter hospital stays, indicating that NAT has minimal impact on the difficulty and overall outcomes of MLRs for CRLM.*

[Purpose] With the innovation of healthcare delivery systems, a need for early determination of patients' discharge outcomes arises after proximal femur fracture surgery, to reduce the burden on healthcare infrastructure. Several studies have examined the extent of walking ability early in the postoperative period to predict the outcome destination. In this study, as an additional validation of these studies, we examined the effect of walking ability on the hospital discharge prognosis of patients in the first week after proximal femur fracture surgery in Japan.

Impact of tumor size on the difficulty of laparoscopic left lateral sectionectomies.

J Hepatobiliary Pancreat Sci

May 2023

Department of Hepatopancreatobiliary and Transplant Surgery, Division of Surgery, Singapore General Hospital, Singapore, Singapore.

Background: Tumor size (TS) represents a critical parameter in the risk assessment of laparoscopic liver resections (LLR). Moreover, TS has been rarely related to the extent of liver resection. The aim of this study was to study the relationship between tumor size and difficulty of laparoscopic left lateral sectionectomy (L-LLS).

Introduction: Laparoscopic liver resection (LLR) has made remarkable progress over the past two decades, providing superior perioperative outcomes to open liver resection (OLR). The pros and cons of LLR for colorectal liver metastases (CRLMs) have been discussed along with the debate regarding optimal resection margins for CRLMs.

Evidence Acquisition: A literature review has been carried out (Pubmed and Embase) focusing on the resection margin status (R status) after LLR for CRLMs.

Article Synopsis
  • Scientists wanted to create a clear definition for what "Textbook Outcome in Liver Surgery" (TOLS) means, which shows the best possible results after liver surgery.
  • They used a method called the Delphi process, where expert liver surgeons answered surveys, and they agreed on the definition only if 80% of them agreed on it.
  • The final definition includes things like no serious incidents during surgery, no major problems after surgery, and a good recovery without re-hospitalization.
