Effectiveness of pediatric drug-induced sleep endoscopy for REM-predominant obstructive sleep apnea.

Sleep Breath

December 2020

Division of Pediatric Otolaryngology-Head and Neck Surgery, Cincinnati Children's Hospital Medical Center, 3333 Burnet Ave. MLC 2018, Cincinnati, OH, 45229-2018, USA.

Study Objectives: Because dexmedetomidine (DEX)-induced sedation mimics non-rapid eye movement (NREM) sleep, its utility in sedating children with REM-predominant disease is unclear. We sought to determine the effectiveness of pediatric drug-induced sleep endoscopy (DISE) using DEX and ketamine for children with REM-predominant OSA, specifically whether or not at least one site of obstruction could be identified.

Methods: A retrospective case series of children without tonsillar hypertrophy undergoing DISE at a tertiary pediatric hospital from 10/2013 through 9/2015 who underwent subsequent surgery to address OSA with polysomnography (PSG) before and after.

Shanghai expert consensus on totally implantable access ports 2019.

J Interv Med

November 2019

Fudan University Shanghai Cancer Center, Department of Oncology, Shanghai Medical College, Fudan University, Shanghai, 200032, China.

Totally implantable access ports (TIAPs) are used for patients with poor peripheral vascular support requiring central venous access. In recent years, TIAPs have been gradually accepted and promoted by patients, doctors, and nurses owing to their advantages of convenient carrying, a long maintenance period, low complications, and a high quality of life for patients. Currently, medical personnel that handle TIAP implantation and management in China are from different areas of healthcare, including surgery, internal medicine, radiology, nurse anesthesia, vascular access, etc.

Aims: To quantify epicardial adipose tissue (EAT) and visceral adipose tissue (VAT) in Chinese adults with impaired glucose regulation (IGR) or diabetes and compare the contributions of EAT and VAT to the occurrence of IGR and diabetes with those of traditional obesity indices.

Methods: Cardiac and abdominopelvic noncontrast computed tomographic images of 668 individuals were used to measure EAT and VAT volume. Multivariable logistic regression and area under the receiver operating characteristic (ROC) curve were used to illustrate the contributions of these tissues.

The study aims to examine drug-induced sleep endoscopy (DISE) in the decision-making process of pediatric obstructive sleep apnea (OSA) patients with small tonsils. This was a retrospective study of children who underwent awake flexible endoscopy, DISE, and adenoidectomy with/without tonsillectomy at the Shanghai Children's Medical Center between 03/2015 and 12/2016. Tonsillectomy was performed for tonsillar obstruction found by DISE.

Article Synopsis
  • Extraaxial lymphoma can mimic trigeminal schwannoma, leading to misdiagnosis and incorrect surgical treatment, as seen in two cases involving the trigeminal nerve.
  • The first patient experienced facial pain and was wrongly diagnosed with trigeminal neuralgia, while the second had a known lymphoma and also presented with facial pain.
  • Histopathological examinations confirmed both cases were diffuse large B cell lymphoma, highlighting the importance of considering lymphoma in trigeminal region lesions and the necessity of surgical biopsy for accurate diagnosis.

China is experiencing the world's largest urbanization. There are two primary types of urbanization in China: rural-to-urban migration and urbanization, represented by Zhejiang Shangyu (SY) and Jiangsu Nanjing (NJ), respectively. Our aim is to compare changes in the prevalence of metabolic disease between these two types of urbanization in China.

Sleep in Angelman syndrome: A review of evidence.

Sleep Med Rev

February 2018

Governor Kremers Center-Rett Expertise Center & MHeNs, Maastricht University, The Netherlands; Department of Genetics, Maastricht University Medical Centre, Maastricht, The Netherlands.

Sleep problems are reported to be extremely prevalent in individuals with developmental disabilities. The consensus guidelines for Angelman syndrome (AS) consider abnormal sleep-wake cycles and diminished need for sleep as associated features. We report an integrative research review and a meta-analysis of studies with sleep as the primary aim of investigation in an AS sample.

Article Synopsis
  • Histone modifications like lysine methylation play crucial roles in biological processes, including cancer, and the methyltransferase SETD3 is identified as being regulated by the cell cycle, peaking during the S phase.
  • The degradation of SETD3 is mediated by the β-isoform of the tumor suppressor FBXW7β, which recognizes SETD3's specific phosphodegrons, with GSK3β being the kinase responsible for phosphorylation that leads to degradation.
  • Elevated levels of SETD3 correlate with increased liver cancer cell proliferation and malignancy, suggesting that the GSK3β-FBXW7β pathway is vital for regulating SETD3's role in liver tumorigenesis.

Modified technique for intrascleral fixation of posterior chamber intraocular lens without scleral flaps.

J Cataract Refract Surg

February 2017

From the Departments of Ophthalmology, Huashan Hospital affiliated with Fudan University (Zhang, Li, Wang), and Ninth People's Hospital affiliated with Shanghai Jiaotong University School of Medicine (He, Jiang), Shanghai, China. Electronic address:

We present a modified technique for intrascleral fixation of a posterior chamber intraocular lens (IOL) without scleral flaps and assess its long-term safety and efficacy. The haptics of the IOL are externalized with a 23-gauge forceps and fixated in a scleral tunnel. Two clockwise scleral incisions are made using a 23-gauge needle at an angle 30 degrees above the sclera.

In circadian rhythm sleep-wake disorders precision medicine is less developed than in other medical disciplines mainly because homeostatic sleep and circadian timing have a very complex phenotype with multiple genetic regulation mechanisms. However, biomarkers, phenotyping and psychosocial characteristics are increasingly used. Devices for polysomnography, actigraphy and sleep-tracking applications in mobile phones and other consumer devices with eHealth technologies are increasingly used.

Sudden infant death syndrome (SIDS) is the unexplained death, usually during sleep, of a baby younger than 1-year-old. Even though researchers have discovered some factors that may put babies at extra risk, SIDS remains unpredictable up until now. One hypothesis is that impaired cardiovascular control may play a role in the underlying mechanism of SIDS.

Int J Biometeorol

May 2017

Department of Otorhinolaryngology, Shanghai Children's Medical Center, affiliated with Shanghai Jiaotong University School of Medicine, Shanghai, 200127, China.

Meteorological and environmental factors influence the pathogenesis of allergic rhinitis (AR). An understanding of the risk factors will facilitate the development of diagnostic and preventative tools for AR children and improve their quality of life. However, research on the impact of these factors on subjective symptoms in AR children remains scarce.

Article Synopsis
  • Individuals with Smith-Magenis syndrome (SMS) experience disrupted sleep patterns, prompting a study on sleep issues among new patients at a sleep clinic.
  • Caregivers of 50 children and nine adults with SMS were surveyed regarding their sleep habits and melatonin use, revealing that some individuals were using exogenous melatonin while many were not.
  • The study found that participants struggled with night waking and early awakenings, and children specifically had abnormal salivary melatonin levels, suggesting a complex relationship between endogenous melatonin, age, gender, and sleep disturbances in SMS that requires tailored treatment strategies.
