Disentangling the contributions from fluctuations in the nuclear overlap size (geometrical component) and other sources at a fixed size (intrinsic component) remains a challenge. Convincing evidence for the efficacy of ablation as first-line therapy in paroxysmal AF (PAF) and its clear superiority to medical therapy for rhythm control in both PAF and persistent AF (PsAF) has generated considerable interest in the optimal timing of ablation. Based on this data, there is a widespread view that the principle of 'the earlier the better' should be generally applied. However, the natural history of AF is highly variable and non-linear, and for this reason, it is difficult to be emphatic that all patients are best served by ablation early after their initial AF episodes.

This review demonstrates the value of central pharmacodynamics (PD), including positron emission tomography (PET) and computerized cognitive testing, to supplement pharmacokinetic (PK) and peripheral PD for determining the target dose range for clinical efficacy testing of emestedastat, an 11β-hydroxysteroid dehydrogenase 1 (11β-HSD1) inhibitor. Combined data from 6 clinical trials in cognitively normal volunteers and patients with Alzheimer disease included a population PK model, endocrine PD, a human PET trial (11β-HSD1 brain imaging), and computerized cognitive testing. PK and PET findings were similar in volunteers and patients with Alzheimer disease.

Objective: Though there is a rich psychoanalytic tradition investigating the content and phenomenology of dreams, the clinical use of this has fallen into widespread disuse. We have undertaken a narrative review of the clinical significance and utility of dream content.
Findings: Dream content may have useful clinical and prognostic value in a number of neurological and psychiatric conditions, including schizophrenia, borderline personality disorder, temporal lobe epilepsy, REM sleep behaviour disorder, dementia, the culture-bound syndrome of Latah, and substance use.

Ecosystem restoration has historically been viewed as an ecological endeavor, but restoration possesses significant, yet largely untapped, potential as a catalyst for personal and social transformation. We highlight the opportunity for restoration to enhance community resilience by increasing agency and collective action and countering the pervasive perception that we are powerless witnesses to environmental decline. In this perspective, we take a "bright spots" approach and highlight successful examples of ecosystem restoration that have helped to nurture a sense of place, foster optimism, and cultivate stronger and more diverse social networks.

Background: Advance care planning (ACP) is potentially helpful for older adults, however, the rate of uptake in community aged care settings is low. Previous pilot studies suggest that holistic, person-centered ACP approaches may be effective for older adults who experience functional impairment but do not necessarily have life-limiting conditions with clear prognoses. This paper describes the protocol of a randomized trial to test the effectiveness of combined life story work and facilitated ACP in promoting ACP engagement among older adults receiving community aged care services.
