Transient neurological symptoms not fulfilling the criteria for TIA are a problem often encountered in clinical practice. In a large cohort from the Rotterdam study, it was found that these 'transient neurological attacks' (TNA) increase the risk of future cardiovascular events and dementia. Imaging using ultrasonography, spiral CT, MRI and 18F-fluorodeoxyglucose positron emission tomography (FDG-PET), plays a major role at two situations during the management of patients with colorectal liver metastases: (a) at the time of the diagnosis and treatment of the primary colorectal tumour, and (b) during the follow-up for the detection of liver metastases and assessing the resectability of these metastases. At the time of the diagnosis and the treatment of the primary tumour, imaging comprising spiral CT or MRI to detect and characterize liver lesions is considered to be the modality of choice. Due to their low prevalence, imaging for the evaluation of lung metastases may be limited to conventional chest radiography.

Three women, aged 21, 28 and 37 years, respectively, were diagnosed with interstitial pregnancies. In the past so years, uterine artery embolisation has gained popularity in the treatment of uterine fibroids compared with traditional surgical interventions. This was based on promising results from a few non-randomised studies, which reported success rates of 95 to 98%. Were these rates accurate? Two recent randomised studies (the Dutch EMMY trial and the Scottish REST trial) showed that, in patients with symptomatic uterine fibroids, the time to recovery and return to work was significantly shorter after embolisation than after surgery (hysterectomy or myomectomy).

Antithrombotic medication plays a key role in the treatmentand secondary prophylaxis of atherothrombotic conditions. Numerous studies have found that acetylsalicylic acid is effective in reducing the incidence of (recurrent) acute vascular events, such as myocardial infarction or ischaemic stroke. For coronary atherothrombosis, anticoagulation with vitamin K antagonists was shown to be slightly superior to acetylsalicylic acid in high-risk patients.

Despite improvements in resuscitation techniques, the prognosis for patients who experience cardiac arrest outside of a hospital remains relatively poor. This is mainly due to brain damage that occurs as a result of global cerebral ischaemia. In 2002, two prospective randomised multicentre studies demonstrated that induced mild hypothermia can increase the chance of good neurologic recovery after out-of-hospital cardiac arrest by at least 40%.

Most patients with post-anoxic coma after resuscitation have a poor prognosis. Reliable prediction of poor outcomes (death or vegetative state after 1 month; death, vegetative state or severe disability after at least 6 months) at an early stage is important for both family members and treating physicians. Poor outcome can be predicted with 100% reliability in the first 3 days after resuscitation in about 80% of patients using pupillary and corneal reflexes and motor response from the neurological examination, cortical responses from somatosensory evoked potentials and EEG.

A 65-year-old man with a 15-year history of 'leukemicised' low-grade lymphocytic B-cell non-Hodgkin lymphoma with a low-titre of IgM kappa paraprotein was admitted with severe anaemia and reticulocytopenia. The revised practice guideline from the Dutch College of General Practitioners offers straightforward instruction on the diagnosis and treatment of thyroid dysfunction. Most cases of hypothyroidism can indeed be managed by general practitioners, and indications for referral to specialists are made clear. For substitution therapy with levothyroxine in otherwise healthy subjects the guidelines advocate the 'start low, go slow'-principle under the adage 'better safe than sorry'.

Long-term substantial development aid has not prevented many African countries from being caught in a vicious circle in health care: the demand for care is high, but the overburdened public supply of low quality care is not aligned with this demand. The majority of Africans therefore pay for health care in cash, an expensive and least solidarity-based option. This article describes an innovative approach whereby supply and demand of health care can be better aligned, health care can be seen as a value chain and health insurance serves as the overarching mechanism.
