Context: The coronavirus disease 2019 (COVID-19) pandemic raised concerns about the safety of laparoscopy due to the risk of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) diffusion in surgical smoke. Although no case of SARS-CoV-2 contagion related to surgical smoke has been reported, several international surgical societies recommended caution or even discouraged the use of a laparoscopic approach.

Objective: To evaluate the risk of virus spread due to surgical smoke during surgical procedures.

Medical treatment for benign prostatic hyperplasia: Where do we stand?

Urologia

August 2019

1 Urologic Section, Department of Human and Paediatric Pathology 'Gaetano Barresi', University of Messina, Messina, Italy.

Male lower urinary tract symptoms are frequently due to benign prostatic enlargement. Medical therapy is strongly recommended in patients with moderate to severe symptoms. Lower urinary tract symptoms may require a different medical approach using drugs with different mechanisms of action.

Late Arteriovenous Fistula After Partial Nephrectomy in Solitary Kidney.

J Endourol Case Rep

May 2019

Urologic Section, Department of Human and Pediatric Pathology "Gaetano Barresi, " University of Messina, Italy.

Arteriovenous fistula is a relatively frequent complication after partial nephrectomy. The timing of this complication is generally limited to the perioperative period. In this study, we describe a case of arteriovenous fistula after imperative right partial nephrectomy occurred 2 years after surgery.

Objective: To simplify the original Preoperative Aspects and Dimensions Used for an Anatomical (PADUA) classification of renal tumours, generating a new system able to predict equally or better the risk of overall complications in patients undergoing partial nephrectomy (PN); and to test if the addition of the contact surface area (CSA) parameter improves the accuracy of the original PADUA and new Simplified PADUA REnal (SPARE) nephrometry classification systems.

Patients And Methods: We analysed the clinical records of 531 patients who underwent PN (open, laparoscopic and robot-assisted) for renal tumours at five tertiary academic referral centres from January 2014 to December 2016. The ability of each variable included in the PADUA classification to predict overall complications was tested using binary logistic regression analysis.

Context: In patients treated for prostate cancer, a rising serum prostate-specific antigen (PSA) level is a first sign of relapse, but imaging is needed to determine the localization of the recurrence, which may be local, in lymph nodes, and/or metastatic. With the increasing success rate of earlier salvage therapy, the diagnosis has become pertinent when the recurrent PSA level is still very low.

Objective: To systematically review the literature on the role of the existing imaging techniques in patients with early recurrent prostate cancer.

Introduction: the aim of this work is to assess whether renorrhaphy with the sliding-clip technique improves perioperative and early functional outcomes compared with traditional renorrhaphy in a contemporary series of patients undergoing open partial nephrectomy (PN).

Material And Methods: A single-center single-surgeon cohort of 107 consecutive patients treated with open PN between July 2013 and March 2017 was analyzed. A study group of 48 patients undergoing renorrhaphy with the sliding-clip technique was compared with a control group of 59 patients receiving traditional renorrhaphy.

Objectives: To evaluate the ability of original tumour contact surface area (CSA) to predict postoperative complications and renal function impairment in a series of patients who underwent elective partial nephrectomy (PN) for renal masses.

Materials And Methods: We analysed the clinical records of 531 consecutive patients who underwent elective PN because of a suspicion of kidney cancer at five academic, high-volume centres between January 2014 and December 2016. Each participating centre evaluated prospectively the radiological images to evaluate the CSA and to assign a PADUA score.

Will Image-guided Metastasis-directed Therapy Change the Treatment Paradigm of Oligorecurrent Prostate Cancer?

Eur Urol

August 2018

Division of Oncology, Unit of Urology, Urological Research Institute, IRCCS Ospedale San Raffaele, Milan, Italy; Vita-Salute San Raffaele University, Milan, Italy. Electronic address:

Although recent studies support the role of image-guided metastasis-directed therapies in the oligo-recurrent setting, several issues including the risk of possible underestimation of the true tumor burden at imaging, patient selection and the lack of studies comparing different approaches limit their implementation in the clinical practice. As such, metastasis-directed therapies should still be considered as investigational treatment options in oligo-recurrent prostate cancer patients.

Objective: The aim of the present anatomic and radiologic study was to evaluate the location, extension, and characteristics of the Brödel's plane and eventually define its different patterns.

Materials And Methods: We evaluated 15 human normal kidneys sampled from unembalmed cadavers without clinical history or anatomical evidence of renal diseases. Kidneys with the surrounding perirenal fat tissue were removed en bloc with the abdominal segment of the aorta.

Context: In bladder cancer (BCa) patients, accurate local and regional tumor staging is required when planning treatment. Clinical understaging frequently occurs and leads to undertreatment of the disease, with a negative impact on survival. An improvement in staging accuracy could be attained by advances in imaging.

Efficacy and Safety of Hexanic Lipidosterolic Extract of Serenoa repens (Permixon) in the Treatment of Lower Urinary Tract Symptoms Due to Benign Prostatic Hyperplasia: Systematic Review and Meta-analysis of Randomized Controlled Trials.

Eur Urol Focus

December 2016

Academic Medical Centre Hospital "Santa Maria della Misericordia", Urology Unit, Udine, Italy; Department of Experimental and Clinical Medical Sciences, Urology Unit, University of Udine, Italy. Electronic address:

Context: A recent Cochrane Collaboration meta-analysis of randomized controlled trials (RCTs) evaluating the efficacy of different extracts of Serenoa repens in relieving lower urinary tract symptoms (LUTS) due to benign prostatic hyperplasia (BPH) concluded that these extracts were no more effective than placebo. However, among all Serenoa repens extracts, Permixon (Pierre Fabre Medicament, Paris, France) has the highest activity and the most accurate standards of drug preparation and extraction.

Objective: To evaluate the efficacy and safety of Permixon in the treatment of LUTS/BPH.

Risk stratification for benign prostatic hyperplasia.

Urologia

August 2017

Department of Surgical and Gastrointestinal Sciences, Urologic Unit, University of Padua, Padua - Italy.

Benign prostatic hyperplasia (BPH) represents an important public health problem in ageing men due to frequently associated lower urinary tract symptoms (LUTS), which may impair quality of life. BPH is also a progressive disease, mainly characterized by a worsening of LUTS over time, and in some patients by the occurrence of serious outcomes such as acute urinary retention and need for BPH-related surgery. The management of BPH and LUTS in men should move forward its focus on symptom control only.
