Background: Evidence indicates that central sensitization (CS) plays a role in subacromial impingement syndrome (SIS). Reduced treatment response has been associated with pretreatment pain sensitization features, such as CS-related symptoms.

Methods: Patients who received subacromial steroid injection were evaluated before the injection, at the first and third months.

Background: Accurate prediction of short-term mortality in patients with acute pulmonary embolism (PE) is critical for optimizing treatment strategies and improving patient outcomes. The Pulmonary Embolism Severity Index (PESI) is the current reference score used for this purpose, but it has limitations regarding predictive accuracy. Our aim was to develop a new short-term mortality prediction model for PE patients based on deep learning (DL) with multimodal data, including imaging and clinical/demographic data.

Machine learning for prognostic prediction in coronary artery disease with SPECT data: a systematic review and meta-analysis.

EJNMMI Res

November 2024

Machine & Hybrid Intelligence Lab, Department of Radiology, Northwestern University, 737 N. Michigan Avenue Suite 1600, Chicago, IL, 60611, USA.

Background: Single-photon emission computed tomography (SPECT) analysis relies on qualitative visual assessment or semi-quantitative measures like total perfusion deficit that play a critical role in the non-invasive diagnosis of coronary artery disease by assessing regional blood flow abnormalities. Recently, machine learning (ML) -based analysis of SPECT images for coronary artery disease diagnosis has shown promise, with its utility in predicting long-term patient outcomes (prognosis) remaining an active area of investigation. In this review, we comprehensively examine the current landscape of ML-based analysis of SPECT imaging with an emphasis on prognostication of coronary artery disease.

J Ophthalmol

November 2024

Ophthalmology Department, Sultan Abdulhamid Han Training and Research Hospital, University of Health Sciences, Istanbul, Türkiye.

Diving is an intense physical activity under hyperbaric and hyperoxic conditions. Pulmonary hypertension (PH) is a progressive vascular disease characterized by elevated pulmonary arterial pressure and resistance in the pulmonary vascular bed. It is associated with high morbidity and mortality. Although right heart catheterization (RHC) is the gold standard for diagnosis, noninvasive methods, such as echocardiography, are essential for early detection and management.

Background: Pulmonary hypertension (PH) involves increased arterial stiffness and reduced vascular tone, affecting pulmonary arterial wave reflections. The Reflection Index (RI) may provide insights into these changes.

Objective: This study examines the utility of RI in PH patients by correlating it with key right heart catheterization (RHC) parameters.

  • Geriatric patients with brittle type 2 diabetes often face complications like hypoglycemia, and Automated Insulin Delivery (AID) systems may improve their glycemic control.
  • A study involving 34 elderly patients switching from multiple-dose insulin to AID systems showed significant reductions in HbA1c levels and total insulin doses after six months.
  • Results indicate that even frail patients, including those undergoing chemotherapy and steroids, can benefit from AID systems, leading to better diabetes management.

Laparoscopic surgery is recommended as the standard approach for bilateral inguinal hernia repair. There are few studies in the literature comparing laparoscopic transabdominal preperitoneal (TAPP) and laparoscopic total extraperitoneal (TEP) approaches for bilateral inguinal hernia repair. This study aimed to compare the surgical outcomes and effectiveness of laparoscopic TAPP and laparoscopic TEP methods applied in bilateral inguinal hernia repair.

There is growing interest in the association between COVID-19-related stressors and quality of life. This study aimed to extend current knowledge by investigating the potential mediating roles of COVID-19 anxiety and COVID-19 burnout in the association between fear of COVID-19 and COVID-19 quality of life in nurses. A total of 300 nurses were selected by convenience sampling approach from a training and research hospital serving as a pandemic hospital in Turkey.

  • The study investigates the prognostic factors for C3 glomerulopathy (C3G), focusing on the total chronicity score (TCS) derived from a new histological index (C3G-HI).
  • 74 patients from 20 medical centers were analyzed to determine factors affecting kidney health outcomes, with a particular emphasis on hemoglobin levels and their correlation with patient survival and kidney function.
  • Results indicated that low hemoglobin levels were linked to worse outcomes in C3G patients, and a TCS of 4 or higher correlated with significantly lower kidney survival rates over three years.

MiRNAs have been reported as biomarkers with diagnostic, prognostic, and predictive value for many different diseases. Therapeutic agents targeting some miRNAs are currently being developed. We aimed to compare BC-NEFs (carcinoma of the breast with neuroendocrine features) with IDC (invasive ductal carcinoma) cases without neuroendocrine features in terms of the level of miRNA expression known to show the oncogenic (miR-21) and tumor-suppressor effects (miR-let7f) and the clinicopathological features.

Background: The objective of this study was to assess the accuracy of VO measurements in predicting long-term major adverse cardiac events (MACEs) in patients with high cardiovascular risk.

Methods: Based on a 10-year atherosclerotic cardiovascular disease risk score, 333 patients with high cardiovascular risk were included in this retrospective analysis. The study endpoint was MACEs, comprising all-cause mortality, cardiovascular mortality, non-fatal myocardial infarction or stroke, and coronary revascularization.

Objective: Pregnancy causes physiological, hormonal, and hemodynamic changes that affect the aortic wall dimensions and elastic properties. Multiple pregnancies increase the risk of aortic enlargement and reduce aortic elasticity. The aortic stiffness index (ASI) and aortic velocity propagation (AVP) are markers of elasticity.

  • Fine-needle aspiration biopsy (FNAB) is the best method to evaluate thyroid nodules, but non-diagnostic results can lead to delays, unnecessary procedures, and stress for patients.
  • * The study aimed to analyze the relationship between patients' anxiety levels, pain perception, and the occurrence of non-diagnostic cytology results during FNAB.
  • * Results indicated that higher anxiety and pain scores were significantly associated with non-diagnostic outcomes, suggesting that alleviating these factors could improve diagnostic accuracy.

Introduction: Sarcopenia, characterized by progressive skeletal muscle loss, has emerged as a significant public health concern, with a global prevalence of 10-27%. While traditionally studied in geriatric populations, recent evidence indicates its impact on individuals aged 40 and above, with early manifestations of muscle decline. Primary care settings play a pivotal role in the early identification and management of sarcopenia, facilitating timely diagnosis and intervention.

Objective: In the hepatitis C virus (HCV) diagnostic algorithm, an anti-HCV screening test is recommended first. In countries with low HCV prevalence, anti-HCV testing can often give false-positive results. This may lead to unnecessary retesting, increased costs, and psychological stress for patients.
