Heart failure with reduced ejection fraction is a syndrome consisting of symptoms (dyspnoea, fatigue, swelling) and/or signs of congestion (pulmonary crackles, oedema). It is caused by structural and/or functional pathologies, most commonly ischaemic heart disease, entailing elevated cardiac filling pressures and can result in low cardiac output. Medical treatment has evolved during the recent decades as outlined in this review, and a 4-pillar treatment strategy is recommended including a renin-angiotensin-aldosterone system blocker or sacubitril/valsartan, a betablocker, a mineralocorticoid antagonist, and an SGLT2 inhibitor.

Chronic coronary syndromes (CCS) are associated with reduced quality of life, an increased risk of cardiovascular events and mortality, and substantial healthcare costs. Diagnostic testing algorithms and treatment strategies for this condition have evolved significantly. Importantly, the results of several randomised controlled trials assessing invasive versus medical management of patients with CCS have recently been reported.

Pure alexia is a rare neuropsychological condition in which a structural lesion in the occipital lobe renders the patient unable to read without affecting writing or the sensorimotor function. We present two cases in which alexia was the sole symptom of stroke: a 72-year-old previously healthy female and an 80-year-old male with diabetes, ischaemic heart disease and renal insufficiency. Each patient presented with difficulty reading without coinciding with aphasia or sensorimotor deficits.

In this case report, a nine-week-old girl was referred to the ear-, nose- and throat department because of decreased oral intake, swelling of the left tonsil and the left side of the neck. An MRI was performed due to the atypical presentation, which revealed a massive peritonsillar abscess communicating with the swelling on the neck. Treatment consisted of an incision, drainage and intravenous antibiotics.

Article Synopsis
  • - Up to 350,000 patients over 65 in Denmark visit emergency departments annually, with 40% suffering from hyperosmolar dehydration (HD), which complicates existing health issues.
  • - HD negatively impacts mortality, morbidity, cognitive and physical functions, and overall quality of life for elderly patients.
  • - The review suggests the use of calculated serum osmolarity from standard blood samples as a diagnostic tool for HD, emphasizing that better recognition and treatment can enhance outcomes for older patients.

Diabetic retinopathy (DR) is the most common complication in patients with diabetes, and screening for sight-threatening end-stages is indicated to avoid severe visual loss. Screening of DR is nationally implemented in Denmark according to evidence-based national guidelines, which includes the use of individualised screening intervals. Treatment is indicated for proliferative DR and diabetic macular oedema, and treatment modalities include retinal laser therapy, intravitreal angiostatic therapy, and vitrectomy.

  • An unusual left atrial chorda was found, which was affecting the mitral valve's function and worsening the insufficiency.
  • She underwent surgery to remove the problematic chorda and received neochordae and a mitral ring annuloplasty, resulting in successful valve function post-operation.

Introduction: In recent years, different factors such as population aging have caused escalating demand for hip and knee arthroplasty straining already limited hospitals' resources. To address this challenge, focus is put on medical and operational efficiency improvements. This includes an increased use of machine learning (ML) to predict duration of surgery (DOS) and length of stay (LOS) for total knee and total hip arthroplasty, which can be utilized for optimizing resource allocation to satisfy medical and operational limitations.

The term placebo poses challenges in definition, language and use in research and clinical settings. Placebo extends beyond inactive substances, and modern exploration has revealed complex neurobiological, psychological, and social factors influencing placebo mechanisms. With knowledge of the placebo effect, clinicians can assess and enhance the effectiveness of the most optimal treatments.

This review presents a simplified model to understand better how disease-modifying anti-inflammatory drugs (DMAIDs) work in immune-mediated inflammatory diseases (IMIDs) with a focus on rheumatology, dermatology, and gastroenterology. In this model, IMIDs are listed on a spectrum from autoinflammatory to autoimmune characterised by the involvement of either mostly the innate or the adaptive immune system. DMAIDs specifically target these immune components and have shown efficacy in distinct IMIDs.

Infections in pregnancy are common, and although often not harmful to the woman or her unborn child, some infections may be associated with severe adverse outcomes. Pregnancy causes physiological and immunological adaptations which may make pregnant women more susceptible to infections and at increased risk of more severe illness. Infections in pregnancy represent a challenging area of concern for many specialists and general practitioners.

Article Synopsis
  • A study was conducted to create a data-driven method for predicting why older adults (≥65) in Denmark are prescribed antiepileptic drugs based on secondary data from registries.
  • The research focused on new users of these medications from 2005 to 2017, analyzing a total of 8,609 participants with a confirmed epilepsy diagnosis.
  • The algorithm developed achieved a sensitivity rate of 65.3%, meaning it accurately predicted the reason for drug prescriptions for about 6 out of 10 users in this group.

Telemedicine is emerging and has both clinical, scientific, and political interest. In this review, we present the present literature on implementation and describe the economic considerations when implementing telemedicine in chronic obstructive pulmonary disease (COPD). The use of telemedicine in COPD is well-received by patients, but there is little evidence to suggest its superiority to standard outpatient care or to support better outcomes for patients.

Bronchoscopy has a low risk of complications when diagnosing peripheral lung lesions suspected of malignancy, however the procedures do not always determine a diagnosis. Several modalities have been invented to improve the diagnostic yield, including radial endobronchial ultrasound and electromagnetic navigation, which are currently used by several departments in Denmark. Augmented fluoroscopy, CT-guided bronchoscopy and robotic bronchoscopy are not yet available in Denmark, but may improve the diagnostic work-up, as argued in this review.
