Italian Expert Opinion on Chronic Hand Eczema: from Guidelines to Clinical Practice.

Dermatol Ther (Heidelb)

November 2024

Section of Dermatology and Venereology, Department of Medicine, University of Verona, Verona, Italy.

Introduction: Chronic hand eczema (CHE) is an inflammatory skin condition characterized by different pathomechanisms, clinical presentations, and prognoses. Treatment is often challenging because of limited approved drugs, and severe CHE is associated with reduced quality of life (QoL) and poor overall health measures in terms of psychological, functional, and occupational challenges. This study aims to describe the real-life management practices of Italian dermatologists who frequently treat patients with CHE, compare these practices with existing guidelines, and propose practical clinical recommendations for the management of these patients.

Article Synopsis
  • A study was conducted to assess current treatment practices for extensive-stage small-cell lung cancer (ES-SCLC) among Italian clinicians, especially regarding the use of radiation therapy.
  • The survey involved 225 cancer care professionals, with 90 responding, mainly radiation oncologists with significant experience; the results highlighted a preference for combining chemotherapy with immunotherapy and the common use of prophylactic cranial irradiation (PCI).
  • The findings revealed variability in treatment approaches for ES-SCLC, indicating a need for further clinical trials and improved multidisciplinary strategies for managing these patients.

Atopic dermatitis (AD) is a chronic, relapsing inflammatory skin condition that can have a negative impact on a patient's quality of life. Long-term effectiveness is required to manage the symptoms of AD (skin inflammation, eczematous lesions, and itching). Because some of the systemic immunosuppressants used to treat AD have been associated with serious adverse events (AEs), other safer, more effective options, including dupilumab, have been proven effective long-term for treatment of adult and adolescent patients with moderate-to-severe AD.

Long-Term Outcomes After Edge-to-Edge Repair of Secondary Mitral Regurgitation: 5-Year Results From the EuroSMR Registry.

JACC Cardiovasc Interv

November 2024

Department of Cardiology, Department of Medicine I, LMU University Hospital, Ludwig Maximilian University of Munich, Munich, Germany; German Center for Cardiovascular Research, Partner Site Munich Heart Alliance, Munich, Germany. Electronic address:

Background: Mitral valve transcatheter edge-to-edge repair (M-TEER) reduces secondary mitral regurgitation (MR) in heart failure and impacts survival in selected patients as demonstrated in the COAPT (Cardiovascular Outcomes Assessment of the MitraClip Percutaneous Therapy for Heart Failure Patients with Functional Mitral Regurgitation) trial. However, long-term outcome data after M-TEER under real-world conditions are lacking.

Objectives: This study sought to assess long-term efficacy and survival after M-TEER in a large real-world registry.

Hidradenitis suppurativa is a chronic skin disorder characterized by painful inflammatory nodules and abscesses, significantly impacting patients' quality of life. Current treatment strategies, including topical antibiotics, often yield limited efficacy and pose risks of antibiotic resistance. Photodynamic therapy has emerged as a potential option, with RLP068/Cl (ELKOFAST®, non-sterile formulation) showing promising efficacy due to its broad-spectrum bactericidal activity.

Diagnosing hypermobile Ehlers-Danlos syndrome (hEDS) and hypermobility spectrum disorders (HSD), common overlapping multisystemic conditions featuring symptomatic joint hypermobility, is challenging due to lack of established causes and diagnostic tools. Currently, the 2017 diagnostic criteria for hEDS are used, with non-qualifying cases classified as HSD, although the distinction remains debated. We previously showed extracellular matrix (ECM) disorganization in both hEDS and HSD dermal fibroblasts involving fibronectin (FN), type I collagen (COLLI), and tenascin (TN), with matrix metalloproteinase-generated fragments in conditioned media.

Background: Recently, the PORT-C and LUNG-ART trials, which evaluated the role of postoperative radiation therapy (PORT), have significantly altered the treatment landscape for NSCLC pN2 patients who previously underwent surgery. In response, the Italian Association of Radiotherapy and Oncology Thoracic Oncology study group has initiated an observational multicenter trial to assess both acute and late toxicities of PORT in pN2 NSCLC patients treated with modern techniques.

Methods: Data on NSCLC patients submitted to PORT after radical surgery treated between 2015 and 2020 in six Italian Centers were collected.

Article Synopsis
  • Researchers want to find better ways to heal the intestines of people with inflammatory bowel diseases (IBD), but there are many different methods to choose from.
  • They studied a special protein called claudin-2 to see if it can help doctors tell if someone has active IBD.
  • Their results showed that claudin-2 is a good marker for IBD, and if more tests confirm this, it could help doctors diagnose and treat patients more easily.

Background: Right ventricular impairment is common among patients undergoing transcatheter edge-to-edge repair for secondary mitral regurgitation (SMR). Adherence to guideline-directed medical therapy (GDMT) for heart failure is poor in these patients.

Objectives: The aim of this study was to evaluate the impact of GDMT on long-term survival in this patient cohort.

Background: Inflammatory bowel disease (IBD) includes Crohn's Disease (CD) and Ulcerative Colitis (UC). Correct diagnosis requires the identification of precise morphological features such basal plasmacytosis. However, histopathological interpretation can be challenging, and it is subject to high variability.

Article Synopsis
  • Malnutrition significantly impacts the prognosis of patients with secondary mitral regurgitation (SMR), with high rates of moderate to severe malnutrition detected among these patients undergoing transcatheter edge-to-edge repair (TEER).
  • A study involving 658 patients revealed that moderate-severe malnutrition was linked to a higher risk of all-cause mortality and hospitalization due to heart failure, demonstrating its harmful effects on clinical outcomes.
  • The findings suggest that nutritional status should be prioritized in the management and treatment of patients with SMR, as it remains an independent predictor of poor prognosis regardless of other factors.

Article Synopsis
  • - The study investigates the link between eosinophilia and ocular vascular issues, highlighting the procoagulant effects of eosinophils, which can lead to various cardiovascular problems and eye conditions.
  • - A retrospective, multicenter analysis of 57 patients revealed that the majority experienced eye issues as the first sign of eosinophil-related diseases, with specific conditions like central retinal artery occlusion being common among them.
  • - The findings indicate a poor visual prognosis for these patients, with only 10% achieving full recovery, suggesting the need for urgent treatment and further research on eosinophilia's impacts.

Effectiveness and tolerability of chlormethine gel for the management of mycosis fungoides: a multicenter real-life evaluation.

Front Oncol

January 2024

Dermatology Unit, Fondazione IRCCS Ca' Granda Ospedale Maggiore Policlinico, Department of Pathophysiology and Transplantation, University of Milan, Milan, Italy.

Background: Topical chlormethine (CL) is recommended as a first-line treatment for early-stage mycosis fungoides (MF) and in 2017, the European Medicines Agency approved the CL gel formulation to treat adult patients. More recently, to increase patient compliance and adherence, clinicians have developed flexible protocols that allow the concomitant use of CL gel with topical corticosteroids in daily practice regimens. Therefore, sharing real-life data on CL gel use and side effects management may help improve the use of this agent.
