Background/aims: Acute decompensation (AD) is defined as the development of complications related to portal hypertension and liver dysfunction that affect the progression of chronic liver disease (CLD) or liver cirrhosis (LC). Variations exist in patient demographics and prognostic outcomes of AD based on the aetiology of CLD, encompassing LC. However, limited research has been conducted to analyse these discrepancies across aetiologies.

Background And Aims: We investigated associations between body mass index (BMI) and hepatocellular carcinoma (HCC) in patients with hepatitis B (HBV) C (HCV) virus infection, alcoholic liver disease (ALD), non-alcoholic fatty liver disease (NAFLD), and liver cirrhosis (LC).

Methods: We followed 350,608 Korean patients with liver disease who underwent routine health examinations from 2003-2006 until December 2018 via national hospital discharge records. Multivariable adjusted hazard ratios (HRs) per 5-kg/m2 BMI increase (BMI ≥25 kg/m2) for HCC risk were calculated using Cox models.

Obesity due to excessive body fat accumulation remains a global problem. Patients with obesity have high cortisol levels, and its dysregulation is caused by increased 11β-hydroxysteroid dehydrogenase type 1 (11β-HSD1) levels. The effects and mechanism of J2H-1702, an 11β-HSD1 inhibitor, on nonalcoholic steatohepatitis (NASH) were explored.

Reducing the risk of urethral strictures after transurethral surgery for patients with bladder cancer requires effective strategies. We compared the clinical outcomes of a novel drug-injectable urethral catheter set (NIUS) with hyaluronic acid (HA) with those of the conventional intraurethral HA injection method. This six-center, prospective, randomized, single-blind trial included 192 male patients aged ≥ 20 years scheduled to undergo transurethral surgery.

Radiofrequency ablation (RFA) is a minimally invasive treatment modality used as an alternative to surgery in patients with benign thyroid nodules, recurrent thyroid cancers (RTCs), and primary thyroid microcarcinomas. The Korean Society of Thyroid Radiology (KSThR) initially developed recommendations for the optimal use of RFA for thyroid tumors in 2009 and revised them in 2012 and 2017. As new meaningful evidence has accumulated since 2017 and in response to a growing global interest in the use of RFA for treating malignant thyroid lesions, the task force committee members of the KSThR decided to update the guidelines on the use of RFA for the management of RTCs based on a comprehensive analysis of current literature and expert consensus.

Screening tests for specific immunoglobulin E (sIgE) to food allergens, such as the multiple allergen simultaneous test (MAST), are widely used in patients with suspected food allergies in South Korea. We evaluated whether MAST could effectively screen wheat-dependent exercise-induced anaphylaxis (WDEIA) and α-gal syndrome (AGS). We retrospectively reviewed patients with WDEIA and AGS diagnosed with unequivocal history and positive sIgE results for omega-5 gliadin and α-gal using ImmunoCAP, respectively.

Impact of Routine Follow-Ups After Curative Gastrectomy in Elderly Patients with Early Gastric Cancer.

Ann Surg Oncol

December 2024

Department of Clinical Epidemiology and Biostatistics, Asan Medical Center, University of Ulsan College of Medicine, Seoul, Republic of Korea.

Introduction: Elderly patients with early gastric cancer exhibit a relatively shorter life expectancy and lower recurrence risk, prompting consideration of whether the regular follow-up strategy should apply to this demographic. This study was designed to determine the effect of routine postoperative follow-up on the elderly patients, specifically those pathologically diagnosed with stage I gastric cancer.

Methods And Materials: This retrospective analysis was conducted at a single tertiary center and enrolled patients aged ≥ 75 years who underwent curative gastrectomy for stage I gastric cancer between January of 2007 and December of 2016.

Article Synopsis
  • - The study investigates the effectiveness of durvalumab maintenance therapy in patients with unresectable locally advanced non-small cell lung cancer (NSCLC) after receiving concurrent chemoradiotherapy (CCRT), comparing those with specific genetic mutations to those without.
  • - A total of 339 patients were analyzed, revealing similar progression-free survival (PFS) rates of 21.4 months for wild-type and 21.0 months for mutant groups, while significant differences in PFS were noted based on programmed death-ligand 1 (PD-L1) expression levels.
  • - The findings suggest that while genetic mutations did not significantly alter outcomes, PD-L1 expression levels had a notable impact on PFS, highlighting the importance

Department of Pulmonary and Critical Care Medicine, Asan Medical Center, University of Ulsan College of Medicine, 88 Olympic-ro 43-gil, Songpa-gu, Seoul, 05505, Republic of Korea.

Article Synopsis
  • Diabetes mellitus (DM) and alcohol consumption are both risk factors for tuberculosis (TB), prompting a study on their relationship specifically in individuals with type 2 DM (T2DM).
  • The study followed over 2.4 million T2DM patients from 2009 to 2018, identifying the effects of alcohol consumption on TB development, with findings categorized into non-drinkers, mild-to-moderate drinkers, and heavy drinkers.
  • Results indicated that mild-to-moderate drinkers had a lowered risk of TB, while heavy drinkers showed an increased risk, especially those consuming 20 g/day or more; additionally, current smokers also faced heightened risks, making the relationship complex.

Article Synopsis
  • - This study aimed to determine if tegoprazan is as effective as lansoprazole in healing erosive esophagitis (EE) within four weeks, with a specific focus on rapid mucosal healing and symptom relief by week two.
  • - In a randomized trial of 218 patients, results showed that tegoprazan had significantly higher healing rates compared to lansoprazole at both two weeks (88.4% vs. 82.6%) and four weeks (95.2% vs. 86.2%).
  • - The findings suggest that tegoprazan is superior to lansoprazole for treating EE, but more research is needed to fully understand its advantages, especially for severe cases

What are the factors contributing to symptomatic local recurrence in metastatic spinal cord compression after surgery?

J Orthop Surg Res

November 2024

Departments of Orthopedic Surgery, Asan Medical Center, University of Ulsan College of Medicine, 388-1, PungNap-2-dong, SongPa-gu, Seoul, Republic of Korea.

Article Synopsis
  • The study aimed to identify risk factors for local recurrence after surgery in patients with metastatic spinal cord compression (MSCC), as this had not been well researched before.
  • A retrospective analysis of 304 patients revealed that local recurrence occurred in 16.4% of cases, with higher rates linked to certain surgical methods, tumor types, and the presence of multiple metastases or pathologic fractures.
  • Key findings indicated that decompression alone had a significantly higher recurrence rate compared to other surgical techniques, and factors like renal cell carcinoma, extensive spinal metastasis, and smoking were also associated with higher local recurrence risk.

JACC Asia

October 2024

Division of Cardiology, Department of Internal Medicine, Heart Vascular Stroke Institute, Samsung Medical Center, Sungkyunkwan University School of Medicine, Seoul, Republic of Korea.

Background: Coronary computed tomography angiography-derived fractional flow reserve (FFR) is a per-vessel index reflecting cumulative hemodynamic burden while coronary events occur in focal lesions.

Objectives: The authors sought to evaluate the additive prognostic value of the local gradient of FFR (FFR gradient) in addition to FFR to predict future coronary events.

Methods: The current study included 245 patients (634 vessels) who underwent coronary computed tomography angiography within 6 to 36 months before the index angiography, of which 209 vessels had future coronary events and 425 vessels did not.
