Article Synopsis
  • Overuse of antibiotics in nursing homes often leads to negative outcomes and the rise of resistant bacteria, prompting the need for better antimicrobial stewardship interventions.
  • This study conducted interviews with prescribing physicians in nursing homes in France to determine factors influencing antibiotic prescribing, resources used, and necessary interventions.
  • Key findings revealed that perceived risks, difficulties in obtaining microbiological samples, and staff shortages contribute to inappropriate prescribing practices, while tailored guidelines and enhanced multidisciplinary communication could improve antibiotic use.

Article Synopsis
  • Chronic ischemic lesions (CILs) are found in about 14.2% of young adults with embolic stroke of undetermined source (Y-ESUS), particularly more common in males, older patients, and those with vascular risk factors like hypertension and diabetes.
  • The study involved 535 patients under 50 years old from various stroke research centers, revealing that those with CILs had different clinical profiles compared to those without CILs.
  • CILs were linked to a lower likelihood of having a pathogenic patent foramen ovale (PFO) and were associated with a reduced risk of stroke recurrence, suggesting their potential significance in understanding stroke mechanisms in young adults.

Incidence and survival of transthyretin amyloid cardiomyopathy from a French nationwide study of in- and out-patient databases.

Orphanet J Rare Dis

November 2023

APHP, Department of Genetics & Department of Cardiology, Referral Center for Hereditary or Rare Cardiac Diseases, Pitié-Salpêtrière Hospital, Paris, France.

Background: Precise data about ATTR-CM incidence rates at national level are scarce. Consequently, this study aimed to estimate the annual incidence and survival of transthyretin amyloid cardiomyopathy (ATTR-CM) in France between 2011 and 2019 using real world data. We used the French nationwide exhaustive data (SNDS database) gathering in- and out-patient claims.

Introduction: Poor food intake is common among elderly living in nursing homes, leading to micronutrient deficiency (MD). There are no recommendations for the management of MD in malnourished older adults.

Methods: We conducted a single arm, open-label, multicenter interventional study in institutionalized malnourished older adults to describe the effect of a 4-week daily energy and protein dense oral nutritional supplementation (ONS, 600 kcal, 30 g protein per unit) containing 50% of the recommended daily micronutrient intake on micronutrient status.

Background: The respective benefits of high and low doses of dexamethasone (DXM) in patients with severe acute respiratory syndrome coronavirus 2 (SARS-Cov2) and acute respiratory failure (ARF) are controversial, with two large triple-blind RCTs reaching very important difference in the effect-size. In the COVIDICUS trial, no evidence of additional benefit of high-dose dexamethasone (DXM20) was found. We aimed to explore whether some specific patient phenotypes could benefit from DXM20 compared to the standard of care 6 mg dose of DXM (DXMSoC).

Postpartum anal incontinence is common. After a first delivery (D1) with perineal trauma, follow-up is advised to reduce the risk of anal incontinence. Endoanal sonography (EAS) may be considered to evaluate the sphincter and in case of sphincter lesions to discuss cesarean section for the second delivery (D2).

We report 21 cases of trichogerminoma harbouring previously undescribed FOXK1::GRHL1/2 or GPS2::GRHL1/2/3 in-frame fusion transcripts. Microscopic examination of a preliminary set of five cases revealed well-delimitated tumours located in the dermis with frequent extension to the subcutaneous tissue. Tumours presented a massive and nodular architecture and consisted of a proliferation of basaloid cells.

Remdesivir and dexamethasone are the only drugs providing reductions in the lengths of hospital stays for COVID-19 patients. We assessed the impacts of remdesivir on hospital-bed resources and budgets affected by the COVID-19 outbreak. A stochastic agent-based model was combined with epidemiological data available on the COVID-19 outbreak in France and data from two randomized control trials.

Aortic Sources of Embolism.

Front Neurol

January 2021

Department of Neurology and Stroke Center, APHP Bichat Hospital, Paris, France Université de Paris, LVTS, Inserm U1148, Paris, France.

Article Synopsis
  • Aortic arch atheroma is common in ischemic stroke patients, but its exact role in causing strokes or as a sign of atherosclerosis is still unclear.
  • Transesophageal echocardiography is the best method for spotting it, with CT angiography as a strong alternative; other imaging techniques may help assess plaque risk.
  • While the best antithrombotic treatment is not yet determined, intensive lipid-lowering therapy is recommended, and the review aims to provide guidance on diagnosis and treatment based on current research.

Establishing a prognosis at hospital admission after stroke is a major challenge. Inflammatory processes, hemostasis, vascular injury, and tissue remodeling are all involved in the early response to stroke. This study analyzes whether 22 selected biomarkers, sampled at admission, predict clinical outcomes in 153 stroke patients treated by thrombolysis and mechanical endovascular treatment (MET).

New roles for prokineticin 2 in feeding behavior, insulin resistance and type 2 diabetes: Studies in mice and humans.

Mol Metab

November 2019

Université de Paris, Paris, France; Unité de Biologie Fonctionnelle et Adaptative, CNRS UMR8251, Paris, France. Electronic address:

Objective: Prokineticin 2 (PROK2) is a hypothalamic neuropeptide that plays a critical role in the rhythmicity of physiological functions and inhibits food intake. PROK2 is also expressed in the main olfactory bulb (MOB) as an essential factor for neuro-and morphogenesis. Since the MOB was shown to be strongly involved in eating behavior, we hypothesized that PROK2 could be a new target in the regulation of food intake and energy homeostasis, through its effects in the MOB.

Background: The efficacy of venovenous extracorporeal membrane oxygenation (ECMO) in patients with severe acute respiratory distress syndrome (ARDS) remains controversial.

Methods: In an international clinical trial, we randomly assigned patients with very severe ARDS, as indicated by one of three criteria - a ratio of partial pressure of arterial oxygen (Pao) to the fraction of inspired oxygen (Fio) of less than 50 mm Hg for more than 3 hours; a Pao:Fio of less than 80 mm Hg for more than 6 hours; or an arterial blood pH of less than 7.25 with a partial pressure of arterial carbon dioxide of at least 60 mm Hg for more than 6 hours - to receive immediate venovenous ECMO (ECMO group) or continued conventional treatment (control group).

The epidemiology of Candida species infection has changed over recent decades, influenced by local hospital-related factors, patient predisposing conditions and type of antifungal agents administered. A shift from Candida albicans as the predominant pathogen towards an increasing prevalence of the species Candida glabrata and Candida parapsilosis amongst critically ill patients has been documented. Changes in Candida species distribution may impact treatment recommendations due to differences in susceptibility to antifungal agents among species.
