Article Synopsis
  • Mycophenolate mofetil (MMF) is commonly prescribed for kidney transplant patients, and the study focused on understanding how its active form, mycophenolic acid (MPA), behaves in both plasma and peripheral blood mononuclear cells (PBMCs).
  • A pharmacokinetic model was built using data from 78 kidney transplant recipients, revealing that MPA concentrations in PBMCs were effective in inhibiting key enzymes, but no link to graft rejection was found.
  • Factors like creatinine clearance, human serum albumin, and a specific genetic polymorphism were identified as influencing MPA levels in the body, highlighting the role of P-glycoprotein in the drug's cellular transport.

Infection of host cells by the respiratory syncytial virus (RSV) is characterized by the formation of spherical cytoplasmic inclusion bodies (IBs). These structures, which concentrate all the proteins of the polymerase complex as well as some cellular proteins, were initially considered aggresomes formed by viral dead-end products. However, recent studies revealed that IBs are viral factories where viral RNA synthesis, i.

Objectives: This phase 2 proof-of-concept study (NCT02610543) assessed efficacy, safety and effects on salivary gland inflammation of seletalisib, a potent and selective PI3Kδ inhibitor, in patients with moderate-to-severe primary Sjögren's syndrome (PSS).

Methods: Adults with PSS were randomized 1:1 to seletalisib 45 mg/day or placebo, in addition to current PSS therapy. Primary end points were safety and tolerability and change from baseline in EULAR Sjögren's Syndrome Disease Activity Index (ESSDAI) score at week 12.

Positive predictive values of CMV-IgM and importance of CMV-IgG avidity testing in detecting primary infection in three different clinical settings. A French retrospective cohort study.

J Clin Virol

November 2020

AP-HP. Université Paris-Saclay, Hôpital Paul-Brousse, Service de Virologie, 94804 Villejuif, France; Groupe de Recherche sur les Infections pendant la Grossesse (GRIG), France; Université Paris-Saclay, 94804, Villejuif, France; INSERM U1193, 94804, Villejuif, France.

Background: Diagnosis of Cytomegalovirus (CMV) primary infection during pregnancy or in immunocompetent patients relies on serology with detection of specific CMV-IgG and IgM. In case of positive CMV-IgM in pregnant women, CMV-IgG avidity is now widely recommended, but in general population it is not currently performed.

Objective: In this study, we aimed to determine CMV-IgM positive predictive values (PPV) in different clinical settings.

Background: Patients with disorders/differences of sex development (DSD), especially those possessing the Y chromosome, have a higher risk of gonadal germ-cell tumours (GCTs). We aimed to examine the incidence of different types of gonadal neoplasia and associated risk factors.

Methods: A total of 1040 DSD patients aged ≥16 years participated in a cross-sectional multicentre European study (dsd-LIFE).

Prospects for improved glomerular filtration rate estimation based on creatinine-results from a transnational multicentre study.

Clin Kidney J

August 2020

Department of Public Health and Primary Care, KU Leuven Campus Kulak Kortrijk, Kortrijk, Belgium.

Background: The Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) creatinine equation is routinely used to assess renal function but exhibits varying accuracy depending on patient characteristics and clinical presentation. The overall aim of the present study was to assess if and to what extent glomerular filtration rate (GFR) estimation based on creatinine can be improved.

Methods: In a cross-sectional analysis covering the years 2003-17, CKD-EPI was validated against measured GFR (mGFR; using various tracer methods) in patients with high likelihood of chronic kidney disease (CKD; five CKD cohorts,  = 8365) and in patients with low likelihood of CKD (six community cohorts,  = 6759).

Purpose: Sarcopenia or adipose tissue distribution within obese and overweight renal transplanted have been poorly evaluated. Our objective was to evaluate morphometric markers to predict surgical complications in kidney transplantation.

Methods: We retrospectively included patients with a BMI > 25 kg/m undergoing kidney transplantation from 2012 to 2017.

Clinical Investigation Center for Biotherapies and Inflammation-Immunopathology-Biotherapy Department (i2B), AP-HP.Sorbonne Université, Pitié-Salpêtrière Hospital, 83 Bd de l'Hôpital, F-75013, Paris, France.

Article Synopsis
  • The study focused on the effects of low-dose IL-2 on activating regulatory T cells (Tregs) in children with recently diagnosed type 1 diabetes, aiming to improve immune regulation over a year-long treatment.
  • Conducted as a Phase I/II clinical trial, 24 children were randomly assigned to receive varying doses of IL-2 or placebo, with results measured primarily by the change in Tregs after 5 days.
  • Results showed no serious side effects, with mild reactions mostly at the injection site; higher doses of IL-2 were associated with significant increases in Tregs compared to placebo.

Septic shock, defined as the combination of sepsis, a requirement for catecholamines to maintain systolic blood pressure above 65 mmHg and a serum lactate level above 2 mmol/L despite adequate volume resuscitation is a life-threatening condition. The Quick Sepsis-related Organ Failure Assessment (qSOFA), which can be used by all nurses with a high-risk patient presenting with infection, enables the patient to be transferred rapidly to specialist care units.

Article Synopsis
  • * The analysis included 18 studies, revealing that chronic smoking negatively impacts cognitive abilities such as attention, working memory, learning, and executive function, while no significant effect was found on delayed memory and language.
  • * Findings suggest that age and the severity of schizophrenia symptoms influence the extent of attention impairment related to smoking, highlighting the need for better management of smoking and cognitive health in these patients.

COVID-19 pandemic is rapidly spreading all over the world, creating the risk for a healthcare collapse. While acute care and intensive care units are the main pillars of the early response to the disease, rehabilitative medicine should play an important part in allowing COVID-19 survivors to reduce disability and optimize the function of acute hospital setting. The aim of this study was to share the experience and the international perspective of different rehabilitation centers, treating COVID-19 survivors.

Cancer immunotherapies have changed the landscape of cancer treatment during the past few decades. Among them, immune checkpoint inhibitors, which target PD-1, PD-L1 and CTLA-4, are increasingly used for certain cancers; however, this increased use has resulted in increased reports of immune-related adverse events (irAEs). These irAEs are unique and are different to those of traditional cancer therapies, and typically have a delayed onset and prolonged duration.

Predicting immunogenicity for biotherapies using patient and drug-related factors represents nowadays a challenging issue. With the growing ability to collect massive amount of data, machine learning algorithms can provide efficient predictive tools. From the bio-clinical data collected in the multi-cohort of autoimmune diseases treated with biotherapies from the ABIRISK consortium, we evaluated the predictive power of a custom-built random survival forest for predicting the occurrence of anti-drug antibodies.

Background: Morbid obesity, based on body mass index (BMI) and/or clinical examination, can be a temporary contraindication (TCI) of kidney transplantation. However, BMI alone does not evaluate the intra- or extra-peritoneal distribution of fatty tissue, and clinical examination alone is subjective. The objective was to evaluate the interest of morphometric criteria to ensure reproducible and consensual decision of TCI.
