The purpose of this study was to determine the frequency with which atherosclerotic lesions of renal arteries are detected during diagnostic angiography in patients presenting with peripheral arterial disease. To this was added the assessment of the risk factors for the development of atherosclerotic renal artery lesions (i. e.

A medial flexion elbow contracture is characterized by the presence of a fold which has a semi-lunar shape, the sheets of which are scars. The shortage in skin length and the excess in width occurs in the sheets. To estimate the size and form of length deficiency in the fold is the section from the top of the fold to the rotation axis of the joint.

The paper analyzes outcomes of 12 internal carotid artery (ICA) graftings for repeat stenosis after carotid endarterectomy (CEA) in 11 patients. In 6 cases restenosis occurred after eversion CEA, in 5 - after conventional patch CEA, and in 1 case - after ICA patch angioplasty for post-CEA restenosis. Critical restenosis >75% in reconstruction area was an indication for surgical intervention.

The paper presents the data on the use of laser Doppler flowmetry (LDF) for microcirculation assessment in patients with lower limb angiodysplasias. Preliminary results suggest that LDF can be successfully used both for the quantification of microcirculatory disturbances and for the diagnosis of arterio-venous micro fistulas. LDF was used as a monitoring tool for compressive therapy effectiveness in patients with arterio-venous and venous forms of lower limb angiodysplasias.

[Endovascular closure of defects of the interatrial septum and patent foramen ovale].

Angiol Sosud Khir

October 2007

Department of Roentgenovascular Surgery, A. V. Vishnevsky Institute of Surgery, Russian Academy of Medical Sciences, Moscow, Russia.

Endovascular occlusion of defects of the interatrial septum and patent foramen ovale has increasingly been gaining ground in surgical practice. This is predetermined by a shortened hospital stay of the patients and the lack of any cosmetic defects left, as compared with the conventionally performed surgical management. The present review analyses both Russian and foreign literature related to the problem concerned.

The article is dedicated to analyzing the outcomes of an open prospective study of efficacy of Liprimai administered in the postoperative period. The study comprised a total of 30 patients presenting with atherosclerotic lesions of the arteries of the carotid bed and aorto-femoral segment, who were subjected to reconstructive vascular operations. The aim of the present study was to look into a possibility of preventing cardiac complications in patients after endured reconstructive vascular surgery.

Allergy to iodine-containing contrast agents is the main contraindication for angiographic examinations of patients. This paper reviews the works of Russian and foreign specialists describing an experience gained with the use of magnetic resonance gadolinium-containing agents possessing radioabsorption properties for digital subtraction angiography and CT, angiography. The authors provide a comparative evaluation of the efficacy of the use of magnetic resonance contrast agents with varying gadolinium concentration.

[Present-day classifications of congenital vascular malformations (angiodysplasias)].

October 2007

Department of Vascular Surgery, A.V.Vishnevsky Institute of Surgery, Russian Academy of Medical Sciences, Moscow, Russia.

The article deals with the previously existing classifications of congenital vascular developmental abnormalities (angiodysplasias, malformations), with an emphasis put on the main classificatory features, as well as on the basic differences between angiodysplasias and haemangiomas. Due coverage is given to specifying certain problems related to the apparently synonymous technical terms and words widely used in the clinical practice concerned. Also, carefully described herein are two most frequently occurring in the practice of cardiovascular surgery classifications of dysplasias, namely that of Hamburg, and V.

[Diagnosis and management of visceral arteries aneurysms].

Angiol Sosud Khir

September 2007

Department of Roentgenendovascular Surgery, A V Vishnevsky Institute of Surgery, RAMS, Moscow, Russia.

Visceral arteries aneurysms appear to be of particular clinical importance in surgical practice. Approximately 22% of the reported cases of visceral arterial aneurysms are complicated by a rupture with a high risk of an eventually lethal outcome. Endovascular embolization of the aneurysmal cavity and closure of the arterial wall defect by means of stent-grafts are increasingly gaining ground in surgical practice, which may reasonably be attributed to the minimally invasive nature of the procedure as compared with the conventional and routinely performed surgical intervention.

Micronized diosmin (Detralex) for vein-related trophic ulcers: European experience.

Angiol Sosud Khir

September 2007

Chair of Faculty Surgery, Department of Continuing Education, Russian State Medical University, Moscow A V Vishnevsky Institute of Surgery, RAMS, Moscow, Russia.

The paper presents a meta-analysis of 5 large European studies (N - 723) on the micronized diosmin administration to patients with venous trophic ulcers (CEAP class VI). With the use of evidence-based approach, it was shown that micronized diosmin has a statistically significant clinical effect on middle-sized trophic ulcers (5 - 10 cm(2)) that persisted for 6 - 12 months. The study concludes that polyvalent venotonics are an appropriate and valuable addition to complex therapy of complicated chronic venous insufficiency.

Tissue reaction to implantation of light polypropylene meshes.

Bull Exp Biol Med

December 2006

Department of Pathology, A. V. Vishnevsky Institute of Surgery, Russian Academy of Medical Sciences, Moscow, Russia.

Comparative histological and electron-microscopic studies of tissues at the site of implantation of Vypro and Ultrapro polypropylene mesh endoprostheses and the meshes coated by fibroblasts were carried out. Tissue reaction to implantation of meshes coated by fibroblasts was more pronounced, particularly during the early period, and the process of connective-tissue capsule formation and implant growth was more rapid. The meshes integrated with the adjacent tissues at the site of implantation, which recommends them for clinical use.

Radioautographic study of palatine tonsillar cells.

Bull Exp Biol Med

March 2006

Department of Pathological Anatomy, A. V. Vishnevsky Institute of Surgery, Russian Academy of Medical Sciences, Moscow.

Proliferative and functional (protein producing) activities of palatine tonsillar cells were studied by light radioautography (semithin sections). 3H-Thymidine incorporation demonstrated the dynamics of renewal of palatine tonsillar cell populations in various pathological states. 3H-Uridine labeling objectively evaluated functional activity of different cells and more accurately showed distribution of destructive processes in the organ.

Radiology Department, A. V. Vishnevsky Institute of Surgery RAMSc, Bol. Serpukhovskaya Street, 27, Moscow 115093, Russia.

Background: Pancreatic cancer is a devastating disease whose early detection remains difficult. There is no 100% reliable imaging test to diagnose and stage pancreatic cancer. We assessed the surgical value of contrast-enhanced spiral computed tomography (CT) in predicting the resectability and survival rates of patients who had pancreatic head cancer.

This paper describes the first in the world experience with the treatment provided in 1990 to a female patient suffering from hypereldosteronism associated with adenoma of the left adrenal. The treatment consisted in endovascular destruction of the adrenal, which was performed by means of acute occlusion of the adrenal venous bed using a sclerosing agent. The intervention resulted in a decrease of arterial pressure (AP) from 240/140 to 130/80 mm Hg.
