Background: Failure, persistent knee instability, and reinjury rates after anterior cruciate ligament (ACL) reconstruction are still concerns. Biomechanical investigations have highlighted the role of the anterolateral ligament (ALL) as a crucial knee stabilizer, and clinical outcomes after combined ACL and ALL reconstruction appear to indicate the success of the procedure.

Purpose: To compare the functional outcomes, return-to-sport (RTS) rate, and complications between combined ACL and ALL reconstruction and isolated ACL reconstruction.

We developed a flow cytometry (FC) assay enabling the rapid and accurate identification of bacterial and viral infections using whole blood samples. The streamlined flow cytometry assay is designed to be user-friendly, making it accessible even for operators with limited experience in FC techniques. The key components of the assay focus on the expression levels of specific surface markers-CD64 on polymorphonuclear neutrophils (PMN) as a marker for bacterial infection, and CD169 on monocytes (MO) for viral infection.

Article Synopsis
  • A study investigated weight changes in patients with chronic rhinosinusitis with nasal polyps (CRSwNP) receiving Dupilumab therapy, focusing on the link between weight gain and improved sense of smell over a year.
  • Results showed significant weight gain and olfactory improvement in patients who had mild smell dysfunction or normal smell before treatment, while those with complete loss of smell did not experience significant weight changes.
  • The findings suggest that improvements in nasal function from the treatment may impact weight, but further research is needed to clarify the relationship between olfactory changes and weight regulation in CRSwNP patients.

Introduction: This study investigated the interaction with membrane mimetic systems (LUVs), bacterial membranes, the CD spectra, and the bactericidal activity of two designed trematocine mutants, named Trem-HK and Trem-HSK. Mutants were constructed from the scaffold of Trematocine (Trem), a natural 22-amino acid AMP from the Antarctic fish , aiming to increase their positive charge.

Methods: The selectivity of the designed AMPs towards bacterial membranes was improved compared to Trematocine, verified by their interaction with different LUVs and their membranolytic activity.

Recalcitrant frontal sinusitis in patients with chronic rhinosinusitis and nasal polyps (CRSwNP) has a negative impact on their quality of life due to frontal pain and a high risk of sinus occlusion, thus necessitating antibiotics, systemic corticosteroids, and multiple surgeries. The aim of this study was to assess the efficacy of dupilumab in reducing frontal pain and the need for rescue treatments for recalcitrant frontal sinusitis in patients with CRSwNP. We enrolled a cohort of 10 patients with severe uncontrolled CRSwNP and concomitant recurrent frontal sinusitis associated with severe facial pain measured by MIDAS score who were treated with dupilumab 300 mg every 2 weeks and followed for at least 12 months.

Pre-asthma: a useful concept? A EUFOREA paper. Part 2-late onset eosinophilic asthma.

Front Allergy

May 2024

Department of Otorhinolaryngology-Head and Neck Surgery, University Hospitals, Leuven, Belgium.

The concept of pre-diabetes has led to provision of measures to reduce disease progression through identification of subjects at risk of diabetes. We previously considered the idea of pre-asthma in relation to allergic asthma and considered that, in addition to the need to improve population health via multiple measures, including reduction of exposure to allergens and pollutants and avoidance of obesity, there are several possible specific means to reduce asthma development in those most at risk (pre- asthma). The most obvious is allergen immunotherapy (AIT), which when given for allergic rhinitis (AR) has reasonable evidence to support asthma prevention in children (2) but also needs further study as primary prevention.

Introduction: Management of severe chronic rhinosinusitis with nasal polyps (CRSwNP) has changed significantly in recent years, with different treatments now available including biologics and endoscopic sinus surgery (ESS), although there are still few comparative studies. We aimed to compare 1-year outcomes of patients with severe CRSwNP treated with dupilumab or ESS plus intranasal corticosteroids (INCS).

Methods: In this retrospective, real-life, observational, cohort study, we enrolled 101 patients with severe CRSwNP who were treated with INCS and either ESS (n = 49) or dupilumab (n = 52).

Article Synopsis
  • Imposter syndrome (IS) is a common psychological issue affecting many professionals, particularly in competitive fields like neurosurgery, where a European survey found 94% of young neurosurgeons show symptoms of IS.
  • The survey involved 232 participants and assessed demographics, IS levels using the Clance Imposter Phenomenon Survey (CIPS), and possible compensatory behaviors.
  • Results indicated that IS was linked to experience level and sex, with male neurosurgeons being less likely to experience intense IS, while reading more than five articles weekly increased the risk, highlighting the need for tailored interventions to support affected individuals.

Article Synopsis
  • - Malignant pleural mesothelioma (MPM) is a highly aggressive cancer linked primarily to asbestos exposure, but recent trends show a growing number of cases without clear asbestos links, suggesting genetic factors may also play a significant role in susceptibility.
  • - About 20% of MPM cases could be influenced by genetic predisposition, particularly involving the BAP1 gene, where those carrying specific variants tend to experience less aggressive forms of the disease and have different treatment needs.
  • - The review summarizes research on both genetic (germline) and non-genetic (asbestos-related) causes of MPM, highlighting the importance of genetic testing and surveillance for at-risk individuals.
