How should the medical team approach care for a very preterm infant with a significant painful and life-limiting condition when the parents wish to pursue all life-sustaining therapies? Here, we discuss a case of an infant born at 28 weeks' gestation with a diagnosis of Carmi syndrome (junctional epidermolysis bullosa and pyloric atresia). While the medical team felt that a do-not-resuscitate order and redirection to comfort care were appropriate, the family held on to hope for recovery and wished to continue with full intensive care measures.

Background: Systemic corticosteroids are vital to critical asthma management. While intravenous methylprednisolone is routinely used in the pediatric intensive care unit (PICU) setting, recent data supports dexamethasone as an alternative. Using the Pediatric Health Information System (PHIS) registry, we assessed trends and variation in corticosteroid prescribing among children hospitalized for critical asthma.

Article Synopsis
  • The study aimed to investigate ethnic differences in facial measurements among infants to inform surgical approaches for correcting bilateral cleft lip/nasal anomalies, questioning whether characteristics like columella length are predominantly European traits.
  • The research involved 33 infants from diverse ethnic backgrounds (African American, Hispanic, and European descent), using 3D imaging to measure 18 different facial distances, with statistical methods applied to analyze ethnic variances.
  • Results showed that while African American infants had wider noses and longer lips, there were no significant differences in nasal projection or columella length, suggesting that standard surgical corrections should be applied across all ethnic groups without modification based on these measurements.

The coronavirus pandemic and in-person contact restrictions necessitated rapid implementation of telehealth, specifically videoconferencing, to provide essential care to patients. This study surveyed 25 pediatric behavioral health providers at a single center during their first month of utilizing telehealth during coronavirus disease 2019 (COVID-19). Twenty-one participants completed a pre-questionnaire distributed prior to telehealth service delivery, and 23 providers completed a post-questionnaire approximately three weeks later.

Degree of Asymmetry Between Patients With Complete and Incomplete Cleft Lips.

Cleft Palate Craniofac J

May 2021

Division of Plastic and Reconstructive Surgery, 7582Johns Hopkins All Children's Hospital, St. Petersburg, FL, USA.

Introduction: Surgical outcomes for patients with complete cleft lips are not as ideal as those achieved for milder phenotypes. We hypothesized that in addition to the greater width of the cleft, patients with complete cleft lip and palate exhibit a greater degree of hypoplasia and asymmetry.

Methods: Stereophotographs of 14 infants with unrepaired unilateral complete and 14 with incomplete cleft lips were measured using Vectra imaging software (Canfield Imaging).
