Purpose: Osteoporosis, affecting over 200 million individuals, often remains unrecognized and untreated, increasing the risk of fractures in older adults. Osteoporosis is typically diagnosed with bone mineral density (BMD) measured by dual-energy X-ray absorptiometry (DXA). This study aims to develop DeepDXA-Hand, a deep learning model using the efficient CNN-based deep learning architecture, for opportunistic osteoporosis screening from hand radiographs.

Cholangiocarcinoma is a malignant tumor that affects the bile ducts and is usually aggressive with poor prognosis. The treatment of cholangiocarcinoma depends on the stage and location of the tumor as well as the patient's overall health status. Systemic therapy, such as chemotherapy using gemcitabine and cisplatin, is the first choice for patients with cholangiocarcinoma who were inoperable.

Background: Necrotizing soft tissue infections (NSTI) are rapidly progressing and life-threatening conditions that require prompt diagnosis. However, differentiating NSTI from other non-necrotizing skin and soft tissue infections (SSTIs) remains challenging. We aimed to evaluate the diagnostic value of the biochemical analysis of soft tissue infectious fluid in distinguishing NSTIs from non-necrotizing SSTIs.

Background: Severe hypoglycemia (SH) poses a significant challenge in the management of type 1 diabetes (T1D); however, the factors that offer protection other than diabetes technologies are under-studied. The primary objective of this study was to examine the association between hypoglycemia problem-solving (HPS) abilities and severe hypoglycemic events in adults with T1D using Poisson regression analysis.

Methods: In this cross-sectional study, 287 adults with T1D (mean age: 50.

  • Motor-sparing local infiltration analgesia (LIA) improves recovery in hip and knee surgeries but can cause local anesthetic systemic toxicity (LAST), prompting the use of rescue lipid treatments.
  • A study involving 1,621 patients was conducted to evaluate the impact of pretreating patients with lipid emulsion on the incidence of LAST after LIA.
  • Results showed a slight decrease in severe LAST events with lipid pretreatment, a reduction in bradycardia and arrhythmias, but an increase in postoperative opioid use, indicating mixed effectiveness and a need for further research.

Article Synopsis
  • This study examined the effectiveness and safety of combining nivolumab with chemotherapy for treating advanced HER2-negative gastric cancer (GC).
  • A total of 27 patients participated, with a notable response rate of 55.6% and a disease control rate of 74.1%, especially in patients with a combined positive score (CPS) of 5 or higher.
  • The findings suggest that this treatment approach could be a new standard for first-line therapy in patients with advanced HER2-negative GC, while the majority experienced manageable side effects.

Chronic kidney disease (CKD) is a widespread condition often resulting from multiple factors, including maternal influences. These risk factors not only heighten the likelihood of developing CKD but increase the risk of a preterm birth. Adverse events during nephrogenesis can disrupt kidney development, leading to a reduced number of nephrons.

Background: The combination of anti-angiogenic therapy and immune checkpoint inhibitors has revolutionized the management of unresectable hepatocellular carcinoma (uHCC). While an early-phase study demonstrated promising outcomes for lenvatinib plus pembrolizumab (L+P) in treating uHCC, the LEAP-002 trial did not meet its primary endpoint. However, the comparative efficacy between L+P and atezolizumab plus bevacizumab (A+B) as first-line treatment remains a topic of uncertainty.

Papaya leaves have been used as food and traditional herbs for the treatment of cancer, diabetes, asthma, and virus infections, but the active principle has not been understood. We hypothesized that the anti-inflammatory activity could be the predominant underlying principle. To test this, we extracted papaya leaf juice with different organic solvents and found that the ethyl acetate (EA) fraction showed the most outstanding anti-inflammatory activity by suppressing the production of nitric oxide (NO, IC = 24.

Divergent interpretations of child abuse in legal judgments: perspectives from clinicians and forensic experts.

Arch Public Health

October 2024

Division of Pediatric Critical Care Center, Department of Pediatrics, Chang Gung Memorial Hospital, Chang Gung University, 5 Fu-Shin Street, Kwei-Shan, Taoyuan, 333, Taiwan.

Background: Child abuse in Taiwan is a major societal concern that severely affects the well-being of children. Despite the complexity in detecting abuse, reports of child abuse are increasing, evidenced by a rise in cases and heightened awareness. This study utilizes judicial judgments as a lens to understand the varied interpretations of child abuse by clinical and forensic experts and explores the broader epidemiological trends of such abuse within the declining youth population of Taiwan.

Article Synopsis
  • Small-vessel occlusion, or small subcortical infarctions (SSIs), are responsible for about one-third of ischemic strokes and can be challenging to distinguish from other conditions based on size alone.
  • This study analyzed data from patients with acute SSIs to explore the relationship between infarct size and outcomes, particularly focusing on early neurological deterioration (END) and functional recovery.
  • Findings indicated that larger infarct sizes (15 mm or larger) were linked to a higher risk of END, with significant predictive thresholds identified, highlighting the importance of infarct size in assessing patient outcomes.

Amphiregulin Upregulation in Visfatin-Stimulated Colorectal Cancer Cells Reduces Sensitivity to 5-Fluororacil Cytotoxicity.

Biology (Basel)

October 2024

Division of Colorectal Surgery, Department of Surgery, Kaohsiung Chang Gung Memorial Hospital, Kaohsiung Medical Center, Kaohsiung 833, Taiwan.

Colorectal cancer (CRC) has become a prevalent and deadly malignancy over the years. Drug resistance remains a major challenge in CRC treatment, significantly affecting patient survival rates. Obesity is a key risk factor for CRC development, and accumulating evidence indicates that increased secretion of adipokines, including Visfatin, under obese conditions contributes to the development of resistance in CRC to various therapeutic methods.

A M-PEEK rod system to stabilize spinal motion after graded facetectomy: a finite element study.

BMC Musculoskelet Disord

October 2024

Department of Orthopedic Surgery, Heping Fuyou Branch, Taipei City Hospital, Taipei City, 100, Taiwan.

Background: Resecting the facet joint to relieve nerve pain can lead to spinal instability, deformity, and abnormal pressure on the anterior of the intravertebral disc. To mitigate these issues, surgeons often limit the amount of bone removed during facetectomy or stabilize the spine by fusion to maintain lumbar stability. This study aimed to assess how a M-PEEK rod system influenced the stability of the lumbar spine during graded facetectomy.
